On 7/19/2022, I conducted an unannounced inspection to the child care center. I interviewed program director, Layal Alkayal. She said has been 2 working as the program director at the facility for six months. Child A attends the center once a week when his mother works a full day. Child care staff member (CCSM), Jennifer Davis is a floater at the center and the mother of Child A. Child A stays in the classroom with the four- and five-year-old children while Ms. Davis works. She has not had any complaints regarding Child A. Ms. Alkayal was not aware that the terms of the license are from age birth to six. On 7/19/2022, I interviewed Ms. Davis. Child A graduated from the preschool program but attends once a week when she works. The children in the preschool room gravitate toward him because he is bigger and taller. The children like climbing on him and he tells them to stop. Most of the parents in the classroom are aware he is her son because he attended for years. She has received no complaints about Child A being in the classroom. On 9/7/2022, I interviewed Child B's (5-year-old, male) Father. Child B was enrolled at the center in the kindergarten readiness program. He has no concerns with staff or the children in the classroom. On 9/7/2022, I interviewed Child C's (5-year-old, male) Mother. Child C was enrolled in the program from infant through kindergarten readiness. The staff is wonderful and she has never had any concerns with staff or the children in the classroom.
Child A (8-year-old, male) is in a classroom daily with four-and five-year-old children with no communication to parents. The infant room is operating out of ratio. The program director asked staff to lie about ratio to the parents.

On 7/19/2022 and 9/19/2022, I interviewed Ms. Alkayal. The infant room was never out of ratio. She hired a third person to assist in the infant room for two days a week. Former child care staff member (CCSM), Samantha Perez complained once because she did not feel comfortable with a new person helping in the infant room. The staff person quit after three days because she felt unwanted. The last day infants were present was 7/15/2022. Three children were enrolled but the parents were made aware weeks ahead that the classroom was temporarily closing. Both staff members quit at the same time leaving her no choice 3 but to close the room. Ms. Alkayal has never asked any staff to lie about the ratio to parents. She had no reason to do such a thing. She worked above the required ratio in the infant room. She worked as a third person in the classroom to assist in maintaining ratio. On 7/19/2022, during my inspection, the infant room was not open. I reviewed infant's sign in sheets for week 7/11/2022 through 7/15/2022. Each day the center maintained the required ratio. On 7/11/2022, the CCSM to child ratio was 2:6. On 7/12/2022, the CCSM to child ratio 3:6. On 7/13/2022, the CCSM to child ratio was 2:5. On 7/14/2022, the CCSM to child ratio was 2:5. On 7/15/2022, the CCSM to child ratio was 2:4. On 9/7/2022, I interviewed Child D's (1-year-old, female) Mother. She has no concerns with the infant room or staff. She was really bummed when the classroom had to close but understood there were no staff to cover the room. She never witnessed the classroom out of ratio. On 9/7/2022, I interviewed Ms. Perez. She was the lead caregiver in the infant room. She was told by Ms. Alkayal to lie to the parents if they are out of ratio. She never lied to the parents. She also never operated out of ratio. Ms. Perez assistant resigned and was replaced by someone else. The new person did not have any personal experience with the babies. Ms. Alkayal thought because she had her own children that it accounted for experience. On 9/7/2022, I interviewed CCSM, Kathleen Sheehan. Ms. Sheehan resigned from working in the infant room. She started out working two days a week and increased to four days during the last two weeks employed. She never worked out of ratio and was never instructed by Ms. Alkayal to work out of ratio. She heard that it occurred but never happened with her. Ms. Sheehan refused to work out of ratio and never witnessed others work out of ratio. On 9/19/2022, I interviewed former CCSM, Lisa Bame. Ms. Alkayal asked her to work out of ratio. She also asked her to lie to parents if they ever asked about ratio. Ms. Bame explained under no circumstances would she ever work out of ratio or lie to the parents. She did ask a parent if he could wait a few minutes during drop off until another staff person arrived to her classroom. The room was never out of ratio. 4
