On May 4, 2023, 1 received an email from program director and licensee designee, Ms. Kerry McShane. Ms. McShane self-reported an incident that occurred today at the center. Child A (female, 22 months old) was left outside on the gated toddler playground for approximately eight minutes. On May 5, 2023, I received an incident report. According to the report, "On Thursday, May 4, 2023, Child A, was left outside in the gated toddler playground when her class went inside. At this time, the Preschool class and the Young Preschool class was occupying the playground areas on both sides of the toddler playground. It was noticed by preschool teacher, child care staff member (CCSM) 5, that Child A was outside by herself. CCSM 5's co- teacher, CCSM 6, brought Child A inside to the office. Child A's Mother and Child A's Father were both contacted to communicate details of the incident. CCSM 4, admin team member, called Child A's Father, at approximately 5:10 pm. Child A's Mother was also contacted by Ms. McShane, at 5:11p m." 3 On May 9, 2023, I completed an unannounced on-site inspection. I interviewed Ms. McShane, program director, Ms. Kelly Plummer, and CCSM 4, CCSM 6 and CCSM 7. CCSM 1, CCSM 2, CCSM 3, and CCSM 5 were absent. Ms. McShane and Ms. Plummer both said they were not at the child care center when the incident occurred. CCSM 4 called and informed them that Child A was left outside on the playground. The incident occurred at approximately 4:39 pm. Ms. McShane and Ms. Plummer both came back to the center. Ms. McShane said CCSM 1, CCSM 2 and CCSM 3 were outside on the toddler playground with ten children. CCSM 3 left when two children were picked up from the playground and the class was in ratio. CCSM 1 and CCSM 2 were on the playground with eight children. Ms. McShane said when it was time to come inside the building, CCSM 1 took four children and left the playground. CCSM 2 followed shortly with three children. Child A was left on the playground. Ms. McShane said the child care center has training videos on how to complete name-to-face head count. Ms. McShane said CCSM 1 and CCSM 2 will be re- trained. CCSM 1 had the iPad, and she took it inside the building with her when she left the playground. CCSM 2 didn't have an iPad. Ms. McShane said they have ordered more iPads so each classroom will have at least two iPads. The child care center uses the Tadpole system which tracks the children's arrival and departure times. Each classroom has an iPad tablet that child care staff members use to track supervision and attendance throughout the day. The Tadpole system allows child care staff members to move a child from one location to the next. I interviewed CCSM 4 who said she works in the classroom as well as the office. CCSM 4 said she was working in the office when CCSM 6 brought Child A to the office around 4:45 pm. CCSM 6 said Child A had been left outside on the playground. According to CCSM 4, she asked CCSM 6 what had happened and CCSM 6 said she, CCSM 5 and the preschool children were outside on the preschool playground. CCSM 6 said they were outside for at least 15 minutes before CCSM 5 noticed Child A standing alone on the toddler playground. CCSM 4 said Child A was not upset or crying. CCSM 4 called Ms. McShane and informed her of the incident. I interviewed CCSM 6. CCSM 6 said she was outside on the playground with CCSM 5, the preschool children and a new child care staff member who was in training. CCSM 6 said CCSM 5 took two children inside to use the bathroom. When CCSM 5 came back outside, CCSM 5 looked over at the toddler playground and saw Child A. 4 CCSM 6 said Child A was standing in the corner by the fence. Child A was talking to a couple of the preschool children. Child A was not crying nor appeared to be upset. CCSM 6 said she thinks Child A didn't understand what was going on or what had happened. On December 21, 2022, licensee designee, Ms. Jamie Miller, called at 4:56 P.M. and left a message. She self-reported an incident that occurred at the child care center at 4:00 P.M. Child A (male, 16 months old) was left outside unsupervised for less than a minute. Ms. Miller indicated that she was contacting Child A's parents and that she will submit a written report of the incident. On December 23, 2022, I received the incident report. According to the incident report, 3 "On December 21, 2022, at 4:07 P.M. it was brought to Ms. April Cangialosi's (admin team) attention via phone call from a parent, Child B's Mother, that a child was left outside the building unattended. Child A is currently enrolled in the Toddler classroom. At the time of the incident, the Toddler class was transitioning from the outdoor playground to the classroom. There were three teachers (Ms. Lily Sultana, Ms. Emilia Bonas, and Ms. Shelby Moore) in charge of 11 children at the time. The classroom performed a head count at the building wall on the sidewalk and all children were accounted for. They then walked inside the building with one teacher in front, one in the middle of the group, one at the end and were in the process of doing another headcount at the hallway wall when a Toddler Too teacher, Marisa Richardson, brought Child A to them, saying he was left outside. The total time apart from the group was approximately less than 30 seconds. Immediately following the incident, April notified Jamie Miller (owner) and Kelly Plummer (interim director) about the situation. April contacted Child A's parents to inform them of the details of what occurred. Child A returned to school as planned the following day, Thursday, December 22nd, 2022. Jamie Miller reported the incident to licensing via phone message within the 24 hours." On January 3, 2023, I called and spoke with interim program director, Ms. Kelly Plummer. Ms. Plummer said she was not at the center at the time of the incident. Ms. Cangialosi called and informed her that the Toddler Classroom was coming in from outdoor play when Child A was left outside. Ms. Plummer said Ms. Richardson is a child care staff member in the Toddler Too Classroom. Ms. Richardson had gone outside to the playground to retrieve one of the children's gloves. Child B's Mother stopped Ms. Richardson and informed her that Child A was left outside on the sidewalk. Ms. Richardson picked up Child A and brought him inside the building to the Toddler Classroom. She believes Child A was left outside on the sidewalk for 30 seconds. Ms. Plummer said Child B's Mother was picking up Child B (male, 3 years old) '/2 when she witnessed the incident. Child B's Mother called and reported the incident to Ms. Cangialosi. I asked if there were any security cameras, and Ms. Plummer said there are security cameras on the side of the building, but she does not believe that there are any in front of the building. Ms. Plummer said Ms. Cangialosi called and spoke with Child A's Mother and Child A's Father. Ms. Cangialosi also followed up with them the next day. Ms. Plummer said Ms. Sultana, Ms. Bonas, and Ms. Moore were all disciplined and re-trained on head counting. IXL created a head count training video that covers policy and protocol. Ms. Plummer said to prevent this from happening again, Ms. Sultana, Ms. Bonas, and Ms. Moore are required to take smaller groups of children outside. Each child care staff member is responsible for four children. They must 4 stop and complete a verbal head count at the office when they leave and when they come back in from outdoor play. On January 3, 2023, I received a telephone call from Ms. Plummer. Ms. Plummer said there is a security camera located at the front of the building. She will send the video footage to me.
