I spoke with the RS on 08/03/2023. They advised that there are numerous medications accessible on Jamie Stockton’s desk in the infant room. The medications have been accessible for days. The RS texted photos to me, one included the medication on the desk. During the onsite inspection on 08/04/2023, Jamie Stockton advised that medication was left that she forgot to grab. She had it “bagged up to go home” and left it overnight. CCSM 1 found it and put it away in the medicine cabinet. I reminded Jamie Stockton that all medications needed to be out of reach of the children at all times. While onsite on 08/04/2023, Alan Wiese denied that any medications were accessible to children. All medications are stored above children’s heads in a medicine cupboard near the toddler bathroom. 11

I spoke to the RS on 08/03/2023. They expressed concern that there is not a curriculum used with the children of any age. The center allows children to free play all day. There is no structure or any learning occurring. The children can watch movies all day or play with toys. There is no circle time or small groups occurring. During the inspection on 08/04/2023, Jamie Stockton said they do have programming but do not use a specific curriculum. They haven’t been able to get High Scope yet but plan to get it and use with children. The center uses various curriculum books and does activities with the children to keep them busy. They have large groups, small groups, music, and large muscle activities. She gave examples of running around outside, playing, reading books, using flash cards, and activities with manipulatives such as play dough and sensory tables. They do math and science experiences in small groups doing science projects and math flashcards. While onsite on 08/04/2023, Alan Wiese advised the children do have programming and it is not just free playing all day. He provided examples such as flash cards for math including addition and subtraction, but they still need to get division, puzzles that spell words, and a binder of math activities. They do science experiments with the older children. The children also color, draw, participate in large and small groups, and take walks to the park daily. When interviewed onsite on 08/04/2023, Child A advised that the CCSM’s play with them. They do science project every now and again. They watch movies. They have to watch the first movie but then they have the option to read, color, or they can start playing. The movies are all on Disney Plus. Child B, Child C, and Child D’s Mother said they agree and are not sure what the children are doing during the day. They seem to only do arts and crafts and color. The television is always on, and the children can do whatever. There was a structure outlined in the paperwork provided by the center at enrollment, but they have not observed any sort of structure at the center. It seems to be all free play and no programming. It is free play until they eat and then take a nap. Then, the children can wake up whenever. They play more and then go home. There are not arts or crafts or anything creative being sent home. Child E’s Mother was not aware of any programming occurring at the center. Child G’s Father was unsure about programming offered at the center. 13 ANALYSIS: The center does not use a curriculum but did provide examples of daily learning experiences for children that include opportunities for physical, social, emotional, and cognitive development. CONCLUSION: VIOLATION NOT ESTABLISHED

I spoke to the RS on 08/03/2023. They advised that raw ground beef was sitting out on the counter to thaw for lunch the next day. It was not put into a refrigerator and then served to children. During the onsite inspection on 08/04/2023, I asked Jamie Stockton how the center thaws foods intended for use. She said all food is thawed in the refrigerator. Even at home, meat would not be left out on the counter and if it was, then it would be thrown away. Jamie Stockton was not aware of any ground beef left out overnight on the counter. While onsite on 08/04/2023, Alan Wiese advised that there has not been any meat left out to thaw overnight to their knowledge. If something was left out, it would have been thrown away. The center has been working on a menu. He did not know where a menu was located. While onsite on 08/04/2023, CCSM 8 advised that all food is thawed in the refrigerator. They have never left meat out to thaw on the counter. Child B, Child C, and Child D’s Mother was not sure about the food handling practices at the center. She does not have a menu or know of one. Child E’s Mother was unsure if raw meat was left out but that it is gross if that is happening. Child G’s Father was not sure about the food or the center practices. He saw to-go food containers of Culver’s on the floor accessible to children under the age of 3 years. The children were picking fries out of the containers and eating them. Child G’s Father witnessed one child for sure eat fries out of the container from the floor. It was between 3:00 PM and 3:30 PM, well after lunch was served. Child H and Child I’s Father did not know how the center handles food. 15 three small pieces of an apple with cinnamon. Another example is one small taquito and one small scoop of rice with three frozen strawberries. The infants get half a taquito. There was a child under 1 year of age that got a taquito. The snack is approximately five animal crackers and older children get one small bag of fruit snacks. During the onsite on 08/04/2023, Jamie Stockton said the center participates with the Child and Adult Care Food Program (CACFP). They use specific portion sizes based on the ages of the children being served per the CACFP requirements. The infants and toddlers get smaller amounts otherwise they spread it around. The center always offers seconds. There is one CCSM that gives too much food and then tries to make the children eat it all and it is too much food for them. Jamie Stockton did not know who their contact was at the food program. During the interview onsite on 08/04/2023, Alan Wiese advised that CCSM 8 makes and serves food. Alan Wiese has cooked food before but is never responsible for serving to children. The center always provides seconds and thirds if children want more food. I asked Child B, Child C, and Child D’s Mother about portion sizes on 08/16/2023 during the telephone interview. She could believe that to be true since when picking up their children, they all act like they haven’t eaten all day. One is a picky eater, so if that child comes home hungry, it is usually because they did not like the offerings. But all of them seem hungry. Child B has said they eat spaghetti, pizza, mac and cheese, and hot dogs. Child E’s Mother was unsure about portion sizes served by the center. Child E seems hungry and cranky when they get home. Child G’s Father said they were providing powder formula for the center to mix initially, but then were told it needed to be in a pitcher they would refrigerate. There were no other concerns noted. Child H and Child I’s Father said the children are always hungry when they come home. The center usually sends home information on what was eaten. Examples of foods given to the children include fried chicken and nachos with cheese. There are no concerns about the quality of food being provided. The children have not complained about the food or that they did not get enough. He was not sure what is being served daily, there is not a m

During my interview with RS it was said a child’s parent brought yogurt bites and a banana. Jamie Stockton shared the yogurt bites and broke up the banana and fed to all of the children in the classroom. Jamie Stockton also ate some of the banana. Jamie Stockton said that a child’s parent did provide some snacks including yogurt bites and a banana. The mother was not sure if the child would eat the snacks and said to give the child either the snacks she provided or whatever the other children were getting. Jamie Stockton was trying to get the child to eat the snacks and was encouraging them by sharing with the other children. She shared the yogurt bites and banana between all the children. There was one piece of the banana left that the children did not want, so she ate it. Jamie Stockton said the parent did not care if the snacks were shared. I reminded her that all food sent by parents must be labeled with the first and last name of the child and the date. The food should only be served to the child it is labeled. Alan Wiese said that some parents send food. One child will only eat pizza, so the parent sends that for the child. They have offered other foods, but the child doesn’t want anything else. The center asked the parent to stop sending food because there is not enough to share with all of the children, and it is not fair. They have children keep candy bars and stuff like that in their cubbies. If parents want the food shared, the center will use gloves when handling the food. I advised Alan Wiese that food provided by a parent for a specific child could not be shared with other children at the center and needed to be labeled with the first name, last name, and date. Child B, Child C, and Child D’s Mother does send food for the children. Usually, it is in the morning and includes animal crackers, granola bars, and pizza. There has not been a known concern with the center sharing food. Child E’s Mother does not usually send food to the center and has Child E leave it in the car most of the time. There is the occasion Child E has gone in with a snack but there have not been any concerns. 19 Child G’s Father did not have knowledge of what was being served to children. He did express a concern regarding the Culver’s to-go boxes being accessible to children under 3 years on the floor. I asked about the Culver’s to-go food. CCSM 1 advised it was their lunch and it was on a table. CCSM 1’s child was also eating the food. Jamie Stockton advised the food containers were pulled onto the floor and CCSM 1’s child was eating the food from the containers on the floor. I advised that children should not have access to food on the floor and children should be sitting, preferably at a table, while eating and drinking. CCSM 1 said the child’s mother did bring food for the child including yogurt bites and a banana. The mother said they were for the child, but the snack could be shared with other children. Jamie Stockton was feeding to the children in the classroom. No one wanted the last bite of banana, so Jamie Stockton ate it. Doran Stockton has not seen a CCSM or anyone else eat food intended for a specific child. They do not allow the children to eat off other children’s plates during mealtime and CCSM’s are right there to supervise. During the exit conference, I reminded them that food brought for children needed to be labeled with the first and last name of the child the food is intended for and then only fed to that specific child. I advised the CCSM’s could not consume the food brought by parents for a specific child.

Licensee*ddesi.. ee Doran Stockton, arrived at the center during the onsite inspection. and I interviewed Jamie Stockton, CCSM 1, Alan Wiese, an Stockton while onsite. Jamie Stockton advised that Alan Wiese did not child room. She said the child could have been his who has been control. She stated there is no discipline for infants, they talk to the toddlers, the older kids get separated or they talk to them and make them apologize. A timeout lasts about two minutes. He knows the rule is one minute per age of the child. On 06/28/2023, and I contacted CCSM 2 over the telephone. She works with an Wiese at the center. She denied that she has seen him child by their . She would not be okay with anyone doing that and would contact the State o ichigan to file a complaint. She has not observed anything concerning and thinks Alan Wiese is good with children. On 06/28/2023, and I contacted Child B, Child C, and Child D's Mother over"teReppone. She did not have any concerns regarding appropriate care and supervision. Sometimes the children are rowdy and might have bumps sometimes. She is aware the Alan Wiese is a caregiver. She has not seen anything physical occur, Child B has been put in time out. She is not sure what happens with the younger children. On 06/28/2023, and I contacted Child E's Guardian over the telephone. She has not "oseErvveIRSIVIs being physical with children but has observed them being "verbal" or sounding more annoyed. There was a younger male CCSM "really yellingat a kid one day." It was concerning but she thinks it was one of hi . He was yelling at the child to stop doing something. She did not see or hear the child's response. On 06/28/2023, and I contacted Child F and Child G's Mother over the telephone. She knows Alan Wiese. He works in all the classrooms. If the cream is a medication, it will need a medication permission form. The form can be located on the Child Care Licensing Bureau's website under forms. Jamie Stockton advised that they check children in diapers every two hours or if they feel they need to be checked. They are changed if the diaper is soiled. Jamie Stockton said the center documents diaper changes for children up to 12 months old. I advised that infants and toddlers require daily sheets including diapering information be provided to parents. They provide this information to parents on a piece of paper. I encouraged the center to keep a copy on file for reference. I asked to review the daily sheet and provided technical assistance. The daily sheet included eating, sleeping, and diapering but did not include unusual behaviors or developmental milestones as required. I also encouraged the center to document the primary caregiver on the daily sheet. While onsite on 06/23/2023, CCSM 1 advised that Child A does have a Child A arrived at the center with . She does not remember 6 Child A's Mother mentioning it when she dropped Child A off and she thought it when they him. She told Child A's Mother at pick up that she was concerned about th Child A's Mother took Child A to the doctor. She could not remember if Child A went to the doctor before or after she talked to Child A's Mother. CCSM 1 said that on 0612212023, Child A had a . She noticed Child A wa and told Child A's Mother. On 06/28/2023, and I interviewed Child B, Child C, and Child D's Mother; Child E's Guardian; Child F and Child G's Mother; and Child H, Child I, and Child J's Mother over the telephone. During the onsite inspection on 06/23/2023, Jamie Stockton advised that Doran Stockton would never be alone at the center. He has never been in the building alone with children, there is always another CCSM present. Someone else would have been present at the center with him and must have been coming in from outside or the kitchen. The other CCSM could have been getting cots for the children and ready for nap time. The cots are stored in the back of the center. She denied there is ever a time when anyone is alone with children at the center. They always have at least two CCSMs present. and I interviewed CCSM 1 while onsite on 06/23/2023. She R"ise t at Doran Stockton has never been at the center by himself. There is always another CCSM present when he is working. and I interviewed Alan Wiese while onsite on 06/23/2023. Reooesnnot recall ere every being only one CCSM present at the center. He has cared for a group of up to 12 children at a time, but he was not the only CCSM at the center. He advised that Jamie Stockton and CCSM 1 open the center and are at the center most of the time. and I interviewed Doran Stockton while onsite on 06/23/2023. ReaM vise that he works at the center frequently. He worked yesterday. He worked at the center and then left for a bit and came back later in the day. He has never been alone with children at the center. He is never even alone in a classroom with children. He does not work alone and is usually working with Jamie Stockton. There are always two CCSMs at the center. and I spoke with CCSM 2 over the telephone on PRPRFe does work with Doran Stockton. She does not remember a time when there was only one CCSM present at the center. No one is ever left alone. On 06/28/2023 and I interviewed Child B, Child C, and Child D's Mother; Child ['S F and Child G's Mother; and Child H, Child I, and Child J's Mother over the telephone. 10 Child B, Child C, and Child D's Mother has observed approximately 12 children with two CCSMs present. There were at least two infants in care. She does not believe there is enough CCSMs present for the number of children they are caring for but are trying to hire more CCSMs. Child E's Guardian has never observed only one CCSM to be present at the center alone. It does seem a little overwhelming sometimes because there are so many babies around. Child F and Child G's Mother did not have concerns regarding ratio. Child H, Child I, and Child J's Mother advised the center has always been fully staffed when she is there with generally two or three CCSMs present at a time. She does not have any concerns regarding ratio. On 08/02/2023, I spoke with CCSM 5 over the telephone. She stated that she has not observed Doran Stockton to be out of ratio but has seen other CCSM's out of ratio. She advised that today, CCSM 6 was alone with 15 infants and toddlers outside. A CCSM called in and so they were short staffed. The center mixes children of all ages including infants with school age children. She has observed the center operating out ratio every day this week. I reviewed investigation report. interviewed Child A's Mother's on 06/24/2023. Child A's Pot"er stated that i e she was changing Child A's diaper before leaving the center, she observed Doran Stockton as the only CCSM with approximately 20 children sleeping on the floor and children in cribs. interviewed CCSM 7 on 06/28/2023. CCSM 7 did not have concerns regarding ratio. interviewed CCSM 3 on 06/28/2023. CCSM 8 said she had never worked alone or seen any other CCSM work alone. There was one occasion when Jamie Stockton and CCSM 1 tried to leave her with one other CCSM at the center with approximately 15 children because they said they needed to go grocery shopping. She told them she would not agree to that, and they did not leave. interviewed CCSM 4 on 06/30/2023. CCSM 4 has not "eenRe qa=one with children. She advised that CCSMs might be left alone if it is during nap time because that is when they take their breaks. During

While onsite on 06/23/2023, Jamie Stockton advised that Child A's Mother and Child A's Grandmother tell them different things on different days regarding what Child A can and cannot eat. The center does not have any medical documentation on file. Sometimes the family says not to give Child A anything but a bottle. Other times, they say Child A can have some food items sometimes. Child A's Mother sends foods including carrots and formula. Jamie Stockton denied she has ever given the wrong formula to a child. She said that a former CCSM did feed Child A another child's formula. The children used the same formula and the CCSM got the cans of formula mixed up. When Child A's Mother picked up Child A, she was given an unopened can of formula. Child A's Mother questioned what Child A was eating all day long. She was advised there was a mistake and Child A was fed from a different can of formula. and I interviewed CCSM 1 while onsite on 06/23/2023. CCSM 1 said A's Mother has told her that they are only sticking to baby 13 food because Child A is having reactions to certain foods. She has only served light baby foods such as apples and bananas. Child A's Mother puts rice in bottles for him. CCSM 1 said "I try to stick to the foods mom says." She advised that she gave Child A apples yesterday and she told Child A's Mother at pick up. CCSM 1 advised that a former employee did give Child A formula from the wrong canister when she worked there. The two children had the same formula. The canisters were on the counter and not labeled. They assumed it was Child A's can of formula and made his bottle out of it. and I interviewed Alan Wiese while onsite on 06/23/2023. Pe has never cared for Child A. He does not do any of the food preparation for children. and I interviewed Doran Stockton while onsite on 06/23/2023. Re does not prepare or feed children at the center. and I spoke with CCSM 2 over the telephone on 06/28/2023. ReRnolt think that Child A had food allergies but rather that Child A's Mother likes him to have more bottles than baby food. She does not know why. She remembers Child A's Mother "making a big stink" that he has not had enough bottles during the day, and they should not feed him any actual food but rather make sure he gets bottles all the time. She knows Child A's Mother had a problem with a former CCSM saying she was not feeding enough bottles. and I asked CCSM 2 about serving the wrong food. She qaRseRt at sheRn a knows about it." She advised that Child A's Mother had put pre-measured powder formula in containers. The center then would mix the bottles. The container with the powder formula was not used all the way. The CCSM had actually used formula from one of the canisters and not the pre-measured container provided by Child A's Mother. She stated that Child A's Mother "made a big fit" that Child A was not eating and was starving but CCSM 2 does not believe that is true. She is not aware of anyone else giving a child the wrong formula or bottle. On 06/28/2023, and I interviewed Child B, Child C, and Child D's Mother; Child E'sRuar ian; Child F and Child G's Mother; and Child H, Child I, and Child J's Mother over the telephone. Child B, Child C, and Child D's Mother. She advised that Child D was breastfed and on formula but is now drinking milk. She used to provide powder formula for the center to mix. She would also send snacks for her children to eat. The children come home hungry often but are picky eaters. Child E's Guardian provides formula with it portioned into containers for the center to mix and feed. Initially, the center was providing water. Now they want her to premix 14 the formula and so she takes a whole pitcher and one bottle for them to use and clean during the day. Child E's Guardian has not had any concerns that Child E has been given the wrong formula. When she was sending portioned containers, it was always gone. They provided a daily sheet with how much Child E was fed. She told them when she starte

I received this complaint on 08/16/2022 and had a text exchange with the reporting source (RS). The RS advised that Child A's attends Tiny Tators Childcare located on Bell Road. The RS knows the center is not licensed for child care because they checked the statewide search for licensed centers and homes and it was not listed. The RS provided contact information for Child A's Mother at her place of employment. I called Child A's Mother on 08/16/2022. I advised Child A's Mother that I was a Child Care Licensing Consultant and asked where she was obtaining child care. She has one child that attends the "center on Bell Rd." I confirmed that it was Tiny Tators Childcare located within a strip mall on the corner of Bell Rd and US 12 in Niles. The center is close to her place of employment, so it is convenient. Child A has been attending for approximately "one month or so." She drops Child A off and picks him up at the center. Child A's Mother advised that a woman named "Christina" cares for the children. I confirmed that it was Christina Kieffer. Child A's Mother said there are other children present at the center. She thinks are Ms. Kieffer's children, but she is unsure. Child A's Mother shares custody with Child A's Father. She has Child A every other week. She has paid the center $140 in cash twice for child care. She is not sure what Child A's Father has paid. I obtained contact information for Child A's Father. I advised Child A's Mother that the center on Bell Rd is not licensed for child care yet and she would not be able to utilize the center for care until it is licensed. I explained that Ms. Kieffer could care for Child A at her licensed home but not at the center on Bell Rd. Child A's Mother said she was upset that her child cannot receive care at the center, and this creates a lot of stress for her. She then hung up on me. I attempted telephone contact with Child A's Father, Ms. Kieffer, and licensee designee, Alan Wiese, on 08/16/2022. I spoke over the telephone with Ms. Kieffer on 08/17/2022. Program director, Jamie Stockton, was present at Ms. Kieffer's home and I was able to speak with both of them. Ms. Kieffer advised that she is not involved with the center and is not caring for children at the center. Someone called her asking about it and she told them it is her brother, Alan Weise, opening the center. She plans to continue to keep her home licensed for child care. Ms. Kieffer said that Child A's Mother works near the center. Child A's Mother drops off and picks up at the center, but Child A is receiving care at her home. I asked if Child A's Mother was aware that Child A was not at the center during the day and was instead at her family home. She said that Child A's Mother should know that Child A is at her home. Ms. Kieffer has permission to transport him. I advised that when I told Child A's Mother that Child A could no longer receive care at Tiny Tators 3 Childcare until it was licensed and would have to use the licensed family home, she became upset and hung up. Ms. Kieffer advised she would talk with Child A's Mother. Ms. Stockton stated that child care is not being provided at the center. Ms. Kieffer meets Child A's Mother at the center and then drives Child A to her home for care. They are still in the process of fixing up the center and getting it ready to be licensed. I reminded them that there cannot be child care provided at the Bell Rd location. I spoke with Child A's Father over the telephone on 08/17/2022. He stated that he is currently dropping Child A off at Ms. Kieffer's home for child care. He has brought Child A to the center, but he is unsure if there is care being provided there. When he was there, they all walked to a nearby park. He is not sure where Child A's Mother drops off or picks up. Child A has been attending for approximately one month. He is paying $140 per week. I advised that there could not be child care provided at the center on Bell Road until a license was issued but Ms. Ki
