This handbook can be obtained for free from the United States Consumer Product Safety Commission's website at https://www.cpsc.gov/safety-education/safety- guides/playgrounds/public-playground-safety- handbook, or from the United States Consumer Product Safety Commission, 4330 East-West Highway, Bethesda, Maryland 20814. This handbook can be inspected and obtained at no charge at the Department of Lifelong Education, Advancement, and Potential, Child Care Licensing Bureau, 105 W est Allegan Street, Lansing, Michigan 48933.
The playground inspection, dated 5/8/2021, provided during my onsite inspection does not include the large play structure on the school aged playground.

The shock absorbing surface material may be either unitary or the loose-fill type. An exception to this subrule is provided for n atural playgrounds.
During my onsite inspection, I observed the playground. There was scattered mulch under the large play structure and in many areas the ground was exposed.

Appropriate supervision was not provided when Child A and another child grabbed Child A on the ladder causing her to fall. Child A was on the school aged playground and equipment which is not intended for children under the age of 5.

Although some inappropriate contact occurred between children, there was a lack of evidence to suggest it occurred as a result of inappropriate supervision.
