On 1/24/2023 I conducted an onsite inspection at the child care facility, Busy Bee's. During the inspection I interviewed the owner/license/program director, Ms. Brooke Allman and Child Care Staff Member (CCSM)1. Ms. Allman advised that the child care center began providing care for Child A on 11/20/2022 through 1/20/2023. Since Child A began attending the child care center there had been a total of 14 incidents involving Child A. Some of the injury reports were related to normative toddler behavior, such as falling while walking, but also acknowledged that there were multiple instances in which Child A had been bitten or scratched by the other child care children. Ms. Allman reported that in her child care policy there is a "3 strike rule" where if a child bites 3 times, on consecutive days, they are terminated from the child care center. Ms. Allman reported that this was part of Child A's Guardian's confusion, as to why the children were still present at the center if he had been bitten more than 3 times. Ms. Allman reported that due to confidentiality, they did not disclose the identity of the other child care children that were biting Child A, but stated that it was not always the same child and that it never occurred over the span of 3 consecutive days. Ms. Allman discussed each incident report with me and provided me with verification of their documentation through the Brightwheel App. The incidents were as follows: • 12/7/2022 - Child A fell while walking - there were no specific details surrounding the fall, but it was reported that he had a red mark present on his head which she believed was a rug burn resulting from the fall. 3 • 12/15/2022 - Child A presented with a scratch on his torso which was discovered during his first diaper change of the day. The incident report cited the diaper change occurred at 8:53AM and he arrived that day at 7:09AM. CCSM2 noted that they believed the scratch was present on Child A upon arrival. • 12/15/2022 - Child A presented with a red mark on his leg which was believed to be a bug bite at 1:46PM. It was noted that Child A's Guardian informed CCSM2 that "they should probably get rid of the bugs." • 12/16/2022 - Child A was bitten by a friend, this occurred while the children were watching a Christmas movie. The bite was located on Child A's back. Due to the severity of the bite, Child B was sent home. • 12/16/2022 - Child A was observed with random red blotches on his face. The photographs were uploaded. It was noted that Child A's Guardian was not concerned, as the blotches were no longer present at the time of pickup. Child A did not have any allergies noted on his Child Information Card. • 1/4/2023 - Child A and Child C were running and collided, bumping heads. There was no swelling, however, Child A presented with a red mark on his cheek. • 1/9/2023 - Child A was scratched by Child D while the children were playing in the ball pit. Child A and Child D were both removed from the ball pit. It was noted that Child A's Guardian was upset and advised that the CCSM should cut Child D's fingernails. • 1/10/2023 - Child A was scratched near his ear by Child D. It was reported that the incident was observed, and the scratch was accidental. • 1/10/2023 - Child A was running and pushing a pop-it toy, tripping over on the rug which resulted in a carpet burn. • 1/11/2023 - Child A was in the infant room and was sitting on the floor in a Summer Super Seat (which sits low to the ground). Child A tipped the seat over and hit his head. Child A presented with a red mark without swelling. • 1/17/2023 - Child A was bitten by Child E. CCSM2 observed this occur and attempted to get to the children prior to the bite but was unable to get to the children in time. The bite occurred on Child A's lower back. The bite was noted to be not significant. • 1/18/2023 - Child A was running and tripped on the rug and presented with a red mark on his head which was noted to be a rug burn. Child A has received multiple injuries in the 8 weeks he has attended the child care facility which include repeatedly being bitten, deep scratches on his face, bumping heads with other children, and from falling out of a chair landing on the floor.
