On November 17, 2022, I completed an onsite inspection. I spoke with program director Jennifer Snyder. Ms. Snyder stated she was working in the classroom on November 3, 2022, when Child A was injured on the outdoor play area. She was outside with the preschool classroom, and it was the end of the day, around 2:50 p.m. It was approximately 15-20 minutes later when Child A walked up to her and he had wood chips all over him. At that time, child care staff member Nicole Wilkinson who was also out on the outdoor play area, took Child A and went to the bench. Ms. Wilkinson felt Child A's arm and looked him over. She took his sweat shirt off and checked his arm and there was no swelling, and he was not crying. Ms. Wilkinson put his sweat shirt back on and he went and played for another 15 minutes; he was climbing and sitting on the dinosaur in the outdoor play area. There were no visible signs of a major injury. This is when Child A's Grandmother arrived to pick him up. Ms. Wilkinson walked Child A over to the pick-up area to meet his grandmother. An hour after Child A was picked up, Child care staff member Michelle Ault received a message from Child A's Mother stating they took his sweat shirt off and he was crying and there was swelling in his arm. Child A's Mother was taking him to urgent care. Child A's Mother updated Ms. Ault later that Child A's arm was broken and he was scheduled to see a specialist the following week. Ms. Snyder stated neither she nor Ms. Wilkinson saw or heard what happened with Child A. She was unable to access their attendance application as no staff are allowed to use the internet until the school system resolves the recent ransomware attack. She believes there were 15 children ages 4- 5-years-old present on the outdoor play area at the time Child A was injured on November 3, 2022, and there were two child care staff members. On November 17, 2022, I spoke with Nicole Wilkinson. Ms. Wilkinson reported on November 3, 2022, she was on the outdoor play area with program director Jennifer Snyder and the children were playing. It was a beautiful day outside, so they were out longer than usual enjoying the weather. Normally when they are outside the child care staff members are always walking around, scanning, and are carefully watching the children. At around 3:00 p.m. she was standing next to Ms. Snyder and they were facing the back of the climbing/play structure. They were engaged in conversation when Child A came around the front of the play structure and he was covered in wood chips; he was not crying and he was not holding himself. The look on his face appeared startled or frightened and he said he fell off the slide. Ms. Wilkinson rushed to Child A and asked him what happened. Child A did not elaborate on the details of his fall. That day the children were rambunctious. There are two slides on that play structure and the slide that twists is a popular slide; the children run toward the slide entrance and jump into the slide. She felt Child A's extremities for injuries and asked if anything hurt, he said no for each area. She asked if she could take his pullover sweat shirt off to check and make sure he was okay; he said yes and she removed the sweat shirt. Child A was wearing a short- sleeved shirt underneath and this made it possible for her to see his arms easily. She noticed his upper left arm was red but there was no swelling. She asked Child A what hurt and he held onto his left upper arm. She touched the area, and he did not react, he made a muscle, moved his arm, and was able to lift his arm up. Child A was acting as if there was no concern other than the red upper left arm which he indicated hurt. Child A went off and continued playing until his grandmother came. Child A was injured on the outdoor play area and the injury resulted in a broken arm. Child care staff members did not see what occurred and were not aware Child A was injured until he came to them.
