On 04/20/2023, an onsite inspection was completed from 12:05 pm until 1:40 pm. The allegations listed in the complaint were discussed with program director Ms. Molly Sieplinga, Child Care Staff Member 1 (CCSM1), CCSM2, and CCSM3. Ms. Sieplinga reported that this incident occurred at the end of the day while children were outside on the playground. CCSM2 and CCSM3 were outside with the children, and CCSM1 was inside of the building. Child care staff members so the incident happen, and they responded immediately. CCSM2 brought Child A and Child B into the classroom, and they stayed with CCSM1 until pickup. The 2 incident occurred on 04/12/2023. Child A and Child B did not return to school until Child A and Child B's Mothers met with school personnel on 04/14/2023. Ms. Sieplinga reported that Child A and Child B are cousins, and they live together with their mothers. There have been some concerns with Child A and Child B including inappropriate language. CCSM1 reported that CCSM2 and CCSM3 took the children outside to play before dismissal. CCSM1 remained in the classroom to complete paperwork. CCSM2 returned to the classroom with Child A and Child B and told her what had happened. Child A and Child B remained in the classroom with her, and CCSM2 returned to the playground. CCSM1 talked to Child A and Child B about what had happened. They were picked up by their grandmother, and CCSM1 told her that she would be contacting Child A and Child B's Mothers to discuss an incident that occurred on the playground. CCSM1 reported that she met with Child A and Child B's Mothers on 04/14/2023, and Child A and Child B returned to the program. CCSM1 reported that she does not have any concerns regarding supervision on the playground. She stated that child care staff members are always moving around the playground, and if they have to discuss something, they meet in the middle of the playground. CCSM2 reported that this incident happened at the end of the day during outdoor play. Child A and Child B were standing near the edge of the playground, but the were clearly visible to child care staff members. She reported that she observed Child A and Child B drop their pants, and she immediately went over to them and pulled their pants up. CCSM2 asked them why they did it, and they said they thought it would be funny. She then brought them into the classroom where they stayed with CCSM1 until they were picked up by their grandmother. CCSM2 reported that there have been some behavioral concerns with Child A and Child B, but they have never done anything like this. There have been issues with them swearing and other minor behaviors. Child A (usually instigates the behaviors, and Child B follows along with him. CCSM2 stated that she does not have any concerns regarding supervision, and all child care staff members work together to ensure that children are appropriately supervised at all times. CCSM3 reported that she did not see this incident happen because she was signing children out as they were being picked up by their parents. She stated that she saw CCSM2 holding Child A and Child B's hands, and CCSM2 told her that she was taking them back into the building. CCSM3 reported that Child A and Child B have talked about watching shows on tv that are not child appropriate. She reported that they use inappropriate language quite a bit, and they have been talking about their private body parts lately. They have never exposed themselves to children or staff in the past. CCSM3 reported no concerns regarding supervision. While I was onsite, I observed the playground. The area is open, and there are no blind spots where child care staff members would have difficulty seeing what children are doing. 3 On 06/05/2023, I interviewed Child A's Mother. She reported that child care staff members reported this incident to her immediately, and she had an opportunity to meet with them and discuss the situation. She has never had any concerns regarding supervision, and
