The incident with Child B occurred on 5/8/2025 at 7:39 A.M. and was not reported until 5/9/2025 at 12:00 P.M. Program Director stated that she "sat on it for 24 hours" because "I just couldn't report it." She also stated that licensee designee Brandy Carey did not want her to report it until Brandy Carey saw the video. Licensee designee Brandy Carey confirmed she wanted to see the video before reporting and was out of the office the day of the incident.

Child care staff members did not provide appropriate care and supervision of Child A. Child A was left unattended in the hallway for two minutes. As Licensee Designee Jenni Bowen went looking for Child A, she left 5 children alone in the classroom.

On 04/05/2022 I conducted an unannounced on-site inspection where I interviewed the Licensee Designee/Program Director and Child Care Staff Members. CCSM Ms. Caitlynn Christiansen stated she was out of the building and received a call from another CCSM indicating that a child has closed their hand in the classroom door. She was told that a staff member closed the door on Child A's hand. Child A was taken to the sink to wash the blood off and to cover the finger. She looked at the finger and there was blood on the tip of the pointer finger. There were no pools of blood or dripping blood. However, she could not tell how deep the injury was. When she returned to the building, Child A's Father was outside waiting to pick Child A up. Pick up is at 3:35pm and the incident happened about 3:27pm. She and CCSM Ms. Brandy Carey, who was also called into the building when the incident occurred, took Child A out to her father where they notified him of the incident. She believes the child was taken to Mercy Hospital for treatment; however, the child care has not been provided with any medical documentation. CCSM Ms. Kelly Chirco stated she was in the classroom with another child care staff member. They had just woken the children up from rest time. She began taking the children's mats and blankets into the hallway. She closed the door behind her as she went into the hallway. She heard the door creek open and then slam. She heard a child screaming. She immediately turned and ran back into the room. When she went back in to the room, the other child care staff member met her at the door where Child A also was. Ms. Christiansen was contacted. She took Child A to the sink and held her hand under water. She did not apply first aid because she did not want to hurt the finger any more than it already was injured. The tip of Child A's finger was hanging but she could not see the bone. A portion of the skin and tissue was ripped. Child A's Father waited about 5 minutes before Child A was brought to him. CCSM Ms. Staci Walsama came in from another classroom and helped. She wrapped Child A's finger with a towel. Right after the finger was wrapped, Ms. Christiansen and Ms. Carey came into the classroom. None of the child care staff members made an intentional effort to keep the incident from Child A's Father. Child A has not been in care since the incident. She is not sure if Child A will be returning. CCSM Ms. Staci Walsama stated she was doing snack in another class room when the door opened and she was asked to help with an incident that occurred. Ms. Chirco and Child A were at the sink. Child A's hand was under running water and Ms. Chirco was trying to comfort her. She tried to control the bleeding with an ice pack and cloth. She saw blood and a piece of her finger hanging. The door appeared to pinch off the top of her skin. Child A was taken outside to Child A's Father shortly after she arrived. Ms. Brandy Carey, Community Ed Director of Orchard View, received a call from a child care staff member indicating that an incident took place in the toddler room. When she arrived, Ms. Christiansen was holding Child A with a cold towel wrapped around her finger. There was blood on the towel. She asked what happened and 3 after seeing Child A's hand, she pulled the cloth back up over her finger. She went to explain to Child A's Father what happened while Ms. Christiansen was taking Child A outside. She went back into the building to speak with licensee designee/program director Ms. Jenni Bowen. She typed up the incident report with Ms. Christiansen. Ms. Christiansen texted Child A's Mother about the injury trying to check on Child A. Child A's Mother requested to have a meeting with the staff. During the meeting, the center staff requested medical documentation. The center has not received any medical documents at this time. On 04/13/2022, I spoke with Child A's parents. It was stated that Child A's Father arrived around 3:15, which is when he rang the

I completed an onsite inspection at the child care facility on 7/23/2021. At that time, I spoke with child care staff member Caitlyn Christiansen. Ms. Christiansen stated that she was aware of the incident in which the complaint was about. She stated that in May she received a call from child care staff member Cooper Dahl stating that Child A was escalating when Mr. Cooper was leaving for the day and had attempted to run out of the classroom. Ms. Christiansen stated that she called Child A's Mother to see if she would be there soon or if Ms. Christiansen should head up to help the child care staff members out. Child A's Mother was on her way to pick Child A up so she did not head to the center. Later that week she received a call from licensee designee Brandy Carey who asked what had happened with Child A that day, as he told his mother that he was held down by Ms. Stutevant and Ms. Tenbrink. She had spoken with them and reported that Child A was escalating after Mr. Cooper left. He was kicking and hitting them and was also thrashing himself on the floor so they were worried about him harming himself. She stated that the caregivers reported they put their hands up to defend themselves against Child A's hits and kicks but never restrained him. On 8/11/2021 I spoke with caregivers Ms. Strutevant and Ms. Tenbrink. I left a message for Mr. Cooper. Ms. Strutevant stated that sometime in May Child A was very upset that Mr. Cooper was leaving because he is his favorite teacher. Child A began throwing toys, crawling under cribs and trying to run out the door. Ms. Strutevant worked on keeping the other kids away from Child A, redirecting his hands and blocking his hit but she never restrained him. She attempted to Facetime Mr. Dahl to see if that would help, but it did not. When Child A's Mother got there to pick him up, she explained that Child A had some behaviors today and they had also spoke with Ms. Christiansen. Ms. Tenbrink stated that herself, Mr. Cooper and Ms. Strutevant were all in the infant room because numbers were down for the day and they had condensed classrooms. Mr. Dahl was able to leave for the day due to numbers and ratio requirements. Child A became very upset when Mr. Cooper left, so Ms. Strutevant attempted to Facetime Mr. Cooper to help but it did not seem to help. Ms. Tenbrink stated that she was changing diapers and keeping the other children over by her while Ms. Strutevant worked with Child A. At one point, Child A had crawled under a crib and was kicking the bottom of the crib; they were worried he would harm himself so they moved the crib and worked with Child A to get him to the carpet area. They asked him several times if there was something he wanted to talk about or if something he wanted to do. He just kept saying no and would hit at the caregivers. At no point did they restrain Child A, they just put their arms up to block his hits. 3 On 8/11/2021 I attempted to contact Child A's Mother but the number was not correct on the child information card. I did speak with Child B's Mother and left a message for Child C's Mother. Child B's Mother stated that her children have been attending the child care center for two years and everything is going well. She has no concerns with the children's safety. I sent an email to Caitlyn Christiansen asking for a new number for Child A's Mother, she responded with a new one. On 8/27/2021 I spoke with Child A's Mother. She stated that Child A does struggle with impulse control due to prenatal trauma. They are working with mental health services to help him learn control and how to handle frustrations. When Child A expressed that he was held down by Ms. Strutevant and Ms. Tenbrink, she was concerned as a mother so she called Ms. Carey to have a discussion about what had happened. Ms. Carey scheduled a meeting with Child A's Mother and all child care staff members. Child A's Mother stated that when hearing from Ms. Struvenant and Ms. Tenbrink that they worked with Child
