    On 02/17/2023, I received a complaint stating that on 02/15/2023 CCSM 1 was holding Child A's hands tightly behind his back while he was kicking and screaming. CCSM 1 put her arm around Child A's neck and shoulder area, picked him up, and carried him outside. On 02/22/2023, I interviewed Child A's Mother. She reported that Child A has had a rough time since the beginning of the school year. Child A's Mother requested that Child A be placed in CCSM 1's classroom because she thought that CCSM 1 would work with him and give him a safe place for alone time. For the last month, Child A has not wanted to go to school because people were being mean to him. Child A's Mother asked him who was being mean to him and he stated, "Everyone. Even the teachers." Child A was previously excited to go to school. CCSM 1 has been reporting that Child A has had bad days recently. Toward the end of January, CCSM 1 reported that Child A did not want to eat lunch or participate in class activities. One day during the week of 02/13/23 to 02/17/23, when she was picking Child A up, CCSM 1 reported that Child A had a rough day, and he did not want to go outside. Child A was running around the classroom during group time. Child A's Mother noticed that CCSM 2 was sitting at a picnic table cradling Child A. CCSM 2 told her that Child A did not have that bad of a day. Child A's Mother was very upset when she was informed of the allegations because she had no idea that this incident had occurred. On 02/27/2023, I received a telephone call from Child A's Mother. She reported that she had a conversation with licensee designee Ms. Diane Aamodt regarding the incident. Ms. Aamodt told Child A's Mother that she was aware that CCSM 1 had been struggling, and she has made a point to place extra child care staff members in the classroom to help out. Child A's Mother felt that Ms. Aamodt was minimizing the seriousness of this incident based on the information that I had provided when she was interviewed last week. On 02/28/2023, I completed an onsite inspection. I interviewed Ms. Diane Aamodt, and CCSM 2. Ms. Aamodt reported that CCSM 1 has been employed at Glenside Head Start since 2005. Since 2019/2020, CCSM 1 has been dealing with some personal issues, and she has noticed that her patience level has not been what it used to be. CCSM 1 has some write ups recently. These write ups involve not serving children the amount of food required and inappropriate Crisis Prevention Institute (CPI) holds on children. CCSM 1 would typically be the only child care staff member in the room, but she has placed additional child care staff members in the room recently. On the day of this incident, CCSM 2 was in the classroom with CCSM 1. CCSM 2 reported the incident to Ms. Aamodt the next morning as it occurred at the end of the day. Ms. Aamodt met with CCSM 1, and she did not think that she had done anything wrong. CCSM 1 was placed on leave the day that the incident was reported. Ms. Aamodt reported that Child A has had some adjustment problems and he has had a lot going on this year, but he was not seen as having behavioral problems. CCSM 2 reported that she has been employed at Glenside Head Start for almost five years, but this incident happened the first day she was in Child A's classroom. Things were going well before free choice play, but then she noticed that CCSM 1 began getting irritated. Her irritation continued to escalate from that point. Three children were playing at the water table, and they spilled some water on the floor. CCSM 1 yelled at them and made them sit down for at least 15 minutes. This happened sometime between 10 and 11:00 am. Throughout the rest of the day, she was raising her voice and shouting at the children. She placed children in timeout at least 3 or 4 times. CCSM 1

    On 11/07/2022 I conducted an unannounced on-site inspection after receiving an incident report from facility indicating Child A was left unattended. I interviewed Ms. Jailenne Gutierrez the child care's nutrition assistant. She was walking down the hallway and saw Child A (4 year old, female). She asked Child A what she was doing. Child A tried to go inside of her locker. Ms. Gutierrez told her not to go in her locker and helped guide her out of the locker. At that point Child A's child care staff member arrived and told Child A she can't do that because this made her scared as she didn't know where Child A was. Child care staff member and morning lead caregiver Ms. Courtney Moe reported that the class was coming in from the playground. The children were putting their items into their lockers to then go back into the classroom. Children were visibly out of the hallway. Ms. Courtney Moe went into the classroom first and then then child care staff member Ms. Bergman went inside. She counted the children and noticed one was missing. She checked the bathroom. Ms. Bergman went into the hallway and checked the hallway and lockers, which is where she found Child A. Child care staff member and afternoon lead caregiver Ms. Kennedy Bergman indicated that they were coming in from outside. They lined up to take their coats off and put them in the locker. A few children like to stand in their lockers, so she 2 checked lockers and there where kids in them. She got them out and Ms. Moe went into the room first. She then took the remainder of the children from the hallway into the classroom. They counted children and noticed Child A was missing. She went into the hallway and checked Child A's locker and found her. She talked to her about not going into her locker because it is unsafe and that scares her teachers. They then returned to the classroom. Both child care staff members indicated the child was left alone approximately 30sec to 1 min at the longest. I spoke with the licensee designee regarding the interviews and explained to her that I would be recommending the center to go on a first provisional as Child A was left unattended.

    On 05/27/2022, I was assigned a special investigation regarding a child care staff member at Glenside Head Start School and Child A (5 yr old, male). On 05/31/2022 1 spoke with Child A's Mother. She stated that her child came from school with a mark on his arm. She asked him how he got the mark and Child A stated that child care staff member Ms. Amanda Galley squeezed him hard when she grabbed him under the arm. She sent pictures to the teacher, and no one said anything to her about the situation. . She believes the school is trying to cover something up because they closed her child's classroom early for the remainder of the year. On 06,02/2022, an unsuccessful face to face was attempted at the home of Child A and Child A's Mother by There was a knock the me. on ana no one answe 5lled Child A's Mother and there was no answer. A message was left A's Mother that we were at her home. inforrri rtg She was asked to return this call. On 06,0212022, I conducted an unannounced on-site inspection where we interviewed the licensee designee, Ms. Diane Aamodt and a child care staff member Ms. Amanda Gailey. Ms. Aamodt reported that she was made aware of the allegations by the child care's attendance worker, Ms. Melissa Worthem. She approached Ms. Aamodt on 5/25/2022 explaining that Child A's Mother was concerned about bruises on Child A and that she would not be sending him to school. Ms. Aamodt then went to Ms. Gailey to inquire about what was going on. Ms. Galley forwarded a message from Child A's Mother that stated, "So I know he's full of stories but... was there an incident today, where Child A was hitting and kicking you? And he was restrained to keep him safe? Leaving bruises on his arm and leg?" This message was received by Ms. Galley from Child A's Mother on 5/23/2022. Ms. Gailey explained to Ms. Aamodt that Child A's Mother was asked why no one told her about him having bruises. Ms. Gailey spoke to Child A's Mother after receiving the text. Ms. Galley stated that she did not know that Child A had bruises. The class was closed on 5/24/2022 due to not having enough staff. Child A's Mother reached out on 5125/2022 and spoke to Ms. Aamodt. She informed Ms. Aamodt that she was going to call the police and MDHHS. She reported that she wanted to know what was going on. Ms. Aamodt informed her that she just learned of this information from Ms. Worthem and would be looking into it. Ica Ms. Amanda Gailey stated is the lead caregiver in Child A's room. On Monday 5/23/2022, there were no concerns or behaviors while outside. Going outside was the first part of the day after Child A arrived at school. In the morning, Ms. Rylee Beauvais, social worker, was one on one in the class with Child A. Ms. Beauvais left the room before or around nap at 1:30pm. Ms. Gailey does not think that Child A took a nap that day. Sometimes he naps and other times he cannot fall asleep. She believes the classroom assistance tried rubbing his back to help him fall asleep, but he did not. The children get up from nap starting around 2:45pm. The children will wake up at different timesand from there they have snack. She does not recall there being any concerns or issues during snack with Child A. After snack they do a drawing on the white board. Then the kids will grab a learning bag to do. Child A appears to do better with structured time. After nap is not a structured timeframe and at times he will struggle with that transition. Child A will do a lot of active movements and flopping on the couch or carpet. He might throw something while playing with an object. Ms. Gailey does not fully recall what triggered Child A. During the learning bag time, Child A wanted to flop on the couch. The class has assigned couch days to give everyone a turn on the couch. It was not his couch day, and he was reminded of this by Ms. Gailey and was asked to move. She reached her hand out for him to grab it and he did. Child A did not appear to be ok with having to leave the couch. They

    I received an incident report indicating that Child A (4 year old, female) slipped on a playground structure causing her to hit her head and need medical attention. I spoke with Child A's Mother regarding the incident. She does not think the incident was handled appropriately as she was notified of the incident through ClassDojo, an online messaging service. When she received the message, she sent Child A's Father to pick her up from school. When Child A was taken to the emergency room for medical treatment where she received 5 stiches. I interviewed child care staff member Ms. Melissa Poll and licensee designee Ms. Diane Aamodt. They stated that both of Child A's parent were called as soon as the incident occurred; however, neither one answered. They messaged Child A's Mom through ClassDojo. While waiting for a reply they were going to call Child A's emergency contacts. Prior to having to call the emergency contacts, Child A's Mother called back and said she would have Child A's Father come and pick her up. During the on-site inspection, I reviewed the office call log and Child A's child information card. At 08:37 am, Child A's Mother was called. At 08:38 am, Child A's Father was called. At 08:39 am, a message was sent through ClassDojo. At 08:40, 2 Child A's Mother called the school back. I also reviewed the school accident and incident policy.
