    There is no evidence that program administrator and staff did not provided appropriate care and supervision of Child A. Child A did not show any prolonged signs of being hurt after teacher 1 looked at his hand at snack time. Child A's Father picked Child A up shortly from the center after Child A showed teacher 1 his hand at snack.

    On 10/31/2022 I received a telephone call from Program Director Miranda Kettel. She indicated that Child A was inadvertently left in a room alone for 10 or more minutes. She will be submitting an incident report in the morning. On 11/2/2022 I conducted an inspection at Victory Child Care and Learning Center. Ms. Kettel was spoken with regarding the incident she reported on 10/31/2022. She was going room to room checking in with staff to ensure everything was okay and she heard a child crying. After a few minutes of investigation, she discovered the child crying was in an approved early childhood gross motor room. The door was closed and lights were off. Child A was standing inside the room by the door crying. She then went to Child A's classroom and asked the three child care staff members, Juliann Dormer, Mallory Roethler, and Stephanie Goffnett, how many children they had in the room and they said 11. She asked them to count the children present and they only counted 10 children. Ms. Kettel returned Child A to the classroom. She reviewed video and contacted licensing. The video was reviewed. Children were seen playing in the gross motor room and at 10:48AM, they began to line up at the door. Ms. Dormer and Ms. Roethler were the staff that were in the gross motor room with the children while Ms. Goffnett was in the classroom cleaning. The video then malfunctioned for several minutes and came back online at 10:50AM. The room was dark but Child A was observed standing by 2 the door crying. Due to the camera malfunction, it is unknown how Child A was left in the room. At 11:01A M Ms. Kettel entered the room and picked up Child A to comfort her. Child A was then returned to her classroom after Ms. Kettel spoke with the child care staff. After leaving the gross motor room, the three child care staff members were observed on the classroom video camera being attentive to the children in the classroom. They were also appropriately upset about the situation when they found out Child A was left in the gross motor room. Ms. Dormer, Ms. Goffnett, and Ms. Roethler were spoken with regarding the incident. All three child care staff members were very remorseful about what happened to Child A and none of them noticed Child A was missing. Child A is reportedly a quiet, happy, and well-behaved child. This type of incident has never happened before and they know that their failure was due to not counting the children during and after the transition and not sweeping the gross motor room when they were leaving. On 11/3/2022 I spoke to Child A's mother. Child A has attended Victory for a while and there have been no issues or concerns until this incident. She has spoken to Ms. Kettel and is upset about the incident. She is also understanding that mistakes happen and she is comfortable forgiving the staff for their error and will continue to send Child A to the center. She feels the care Child A receives is exceptional up until this incident. On 11/9/2022 I conducted an exit conference with Ms. Kettel.

    On 4/22/2021 I received a telephone call from Victory Child Care Program Director Kelli McDonald. Child A's mother called her last night and indicated Child A was missing her bottom two front teeth. She has spoken with the child care staff members and watched the video from Child A's classroom and saw no indication of her being hurt or harmed by an adult. Child A's clothes had no blood and no staff at the center recalls her being upset or indicating she was hurt. She ate 2 lunch and snack without complaint. She did not sleep at rest time which is odd for her. It is known that the teeth were lost at child care because Child A's mother has a photo of her from that morning and both teeth can be seen in the photo. Child A is being held out of child care until the investigation can be done. On 4/26/2021 I received a telephone call from Mrs. McDonald. She has watched the video several times and still sees no indication of injury. The video is saved for the investigator to view. On 4/26/2021 I completed an unannounced onsite inspection at Victory Child Center. Child A is still not attending child care. Mrs. McDonald has kept in touch with Child A's mother and Child A is doing well. Child A's dentist said there are some rare conditions such as hypophosphatasia which cause premature tooth loss in children. Blood tests are being done by Child A's pediatrician. Child A seemed very happy throughout the video from the classroom. She cried during afternoon snack and fell a few times throughout the day but never hit her face on anything. Lead caregiver Alex Moulter was spoken with. She has been the lead caregiver in the toddler room for 18 months. Child A has only been in the room a couple of weeks. She is a happy child and is just learning to talk more. On the day of the incident, Child A was having a good day. She did not sleep which was odd, but she did lay down and rest. She did cry for no reason at one point but after two minutes she was fine. There were no major incidents or blood noticed in her mouth or on her clothes. They went outside in the morning but just rode in the large buggy and did not go outside in the afternoon due to poor weather. She spent some time looking for the missing teeth in the classroom after Child A's mother called, but nothing was found. Child care staff member Ashley Newhall was spoken with. She has only been working at the center for a couple of weeks. She does not remember Child A falling or being hurt at all. She was happy all day and except for her not sleeping, the day was normal. Child care staff member Ashlynn Sytek was spoken with. She has worked at the center for nine months. Child A was upbeat all day, and no major concerns were noted. There were no major incidents or injuries for Child A that day. The only thing out of the ordinary was that Child A did not nap. The only time Child A cried was during PM snack, but there did not appear to be a reason. On 4/27/2021 a telephone call was made to Child A's mother. She picked Child A up at her normal time and she seemed irritable. About 10 minutes after they got home she noticed Child A's bottom front teeth were missing. There was no blood observed on her shirt or mouth. The center was contacted, and the issue was reported. She was taken to the dentist the next day and the dentist felt as if it was caused by trauma because of the way the gums looked. Bloodwork is being done by Child A's pediatrician to rule out any other causes.
