On 11/18/2022, I received a telephone message from Tia Moore, licensee designee and education specialist for Macomb County Family Resource Center Head Start. Ms. Moore was reporting an incident regarding a child care staff member being physically abusive with a child in care. On 11/18/2022, Ms. Moore emailed the written incident report. On 11/21/2022, I conducted a telephone interview with Ms. Moore regarding the telephone call and incident report. I asked Ms. Moore to verify that the date listed on the incident report of 11/03/2022 was accurate. Ms. Moore confirmed that the incident occurred on 11/03/2022. Ms. Moore indicated that on 11/03/2022, Child A's Mother informed her that when she arrived to pick up Child A, Child A reported that Child Care Staff Member 1 had grabbed him by the face. Child A's Mother wanted to meet with Ms. Moore in person and when Ms. Moore met with Child A's Mother and Child A, Child A demonstrated to Ms. Moore how he was grabbed by the face by Child Care Staff Member 1. Child A also reported to Ms. Moore that Child Care Staff Member 1 threatened to take away his blanket and give it to her own son. Ms. Moore did not observe any marks or bruises on Child A, but Child A's Mother reported that he had been coming home with marks on him. Ms. Moore then reported the incident to management and an internal investigation was started. Child Care Staff Member 1 was still working in the classroom when I spoke with Ms. Moore on 11/21/2022. 3 I asked Ms. Moore why the incident was not reported within the required timeframes and was reported over two weeks later. Ms. Moore indicated that the week after the incident she was in and out of the office. Additionally, she and Witness 1 from Head Start administration were gathering statements to send to human resources because Child Care Staff Member 2 had also witnessed the incident. I informed Ms. Moore of the licensing rules regarding timely notification to the department and informed her that the department was not notified within the required timeframes. On 11/23/2022, a telephone message was left for Child A's Mother. No response was received from Child A's Mother. On 11/28/2022, I conducted an unannounced onsite inspection at Macomb County Family Resource Center Head Start. I met with Ms. Moore who indicated that Child Care Staff Member 1 had been removed from the classroom and placed on administrative leave pending further investigation. Child Care Staff Member 2 was not at the center during the onsite inspection. Ms. Moore explained Child Care Staff Member 2 was substituting for Child Care Staff Member 3 the day of the incident. Child Care Staff Member 3 normally works in the classroom with Child Care Staff Member 1. I asked Ms. Moore if Child A had any challenging behaviors in the classroom. She explained that Child A has some behavioral issues and receives supportive services. The behaviors include spitting, punching, kicking, and hitting children and child care staff members. Child A's behaviors are not daily but are frequent. Ms. Moore denied that she had ever received any parental complaints regarding Child A injuring other children. I asked Ms. Moore if she had ever received any complaints regarding Child Care Staff Member 1. Child Care Staff Member 1 had transferred to the center this school year, but worked with Head Start for over 17 years at other locations. Ms. Moore stated that there was one parental complaint regarding Child Care Staff Member 1's attitude and another concern by another Witness 2, another Head Start employee. On 10/21/2022, Ms. Moore spoke with Child Care Staff Member 1 about the concerns and offered her additional training. Child Care Staff Member 1 did not want the training and was upset about being offered additional training. Ms. Moore stated that after speaking with Child A's Mother on 11/03/2022 she was informed that Child Care Staff Member 2 had informed Witness 2 that she had observed an incident in the classroom with Child C
On 11/18/2022, the center self-reported that Child A (four-year-old male) was grabbed by the face by a child care staff member on 11/03/2022. Child A was also told that his blanket was going to be taken away and given to the child care staff member's own child. No bruises were observed on the child, but Child A's Mother indicated Child A has had previous unexplained bruises.

On 10/28/2022, I received a telephone call and email from Tia Moore, licensee designee and education specialist. Ms. Moore indicated that Child A's Mother had called to report that a child had touched Child A inappropriately. I asked Ms. Moore to submit an incident report for review. On 10/28/2022, I received the incident report which included a written statement from the lead child care staff member, Annabelle Altermatt. Ms. Altermatt's statement indicates that Child A's Mother reported to her that another child touched Child A's privates. On 11/01/2022, I conducted a telephone interview with Ms. Moore, Ms. Moore confirmed that Child A's Mother was reporting that Child A had said Child B touched her inappropriately. Ms. Moore did not have specifics, but indicated that Child A's Mother had spoken with Ms. Altermatt and was comfortable with Ms. Altermatt's response. On 11/01/2022, I conducted a telephone interview with Ms. Altermatt, she explained that on 10/27/2022, Child A's Mother asked to speak with her and reported that Child A was saying that another child had touched her in her private area. Child A did not say which child touched her, but Child A's Mother said it was Child B. I asked Ms. Altermatt why Child A's Mother would say Child B and she explained that Child A's Mother is aware that other parents are concerned and complaining about Child B's behaviors. There were no previous complaints about inappropriate touching, but Child B "is handsy." Child B has some developmental issues and on one occasion, he put his hands around another child's neck like he was going to squeeze that child's neck, but a child care staff member intervened before he did anything. Child B has impulsive control issues and will do things like hug other children from behind. He also has some speech delays but will repeatedly "use the b word." When the concerns are addressed with Child B's Mother, she says she will talk to him about it at home. On 11/02/2022, I conducted a telephone interview with Child A's Mother. One day while in the bath, Child A stated, "Child B touched me down there" and pointed between her legs. Child A explained they were in the block area when Child A touched her. Child A's Mother has heard multiple reports from other parents of Child B spitting on other children and acting out physically. Child A's Mother "freaked out" when Child A told her, so she asked to speak with Ms. Altermatt. Child A's Mother was very satisfied with Ms. Altermatt's response and ability to address the issue. 3 Child A's mother did not have any other concerns about the quality of care Child A receives at the center. On 11/03/2022, I emailed Ms. Moore that the incident report was being assigned as a special investigation. On 11/03/2022, I conducted an unannounced onsite inspection, and the classroom was closed due to Covid and would reopen the following week. On 11/14/2022, I conduced an unannounced onsite inspection and observed Ms. Altermatt's classroom. Child B was present during the onsite inspection screamed when he did not get his way. He was reminded three times to "use your gentle hands with friends" because he was at times aggressive and grabbed things away from other children. Child B also attempted to open the classroom door, but was quickly redirected by a child care staff member. Child B was also redirected by child care staff when using foul language. Child B required 1:1 child to child care staff member supervision and due to the class size and the fact there were two child care staff members in the classroom, Child B's needs and the needs of the other children were met. I spoke with Ms. Altermatt briefly and she indicated that Child B continues to have challenges like spitting several times a day. He has been referred for additional services. Ms. Altermatt denied ever witnessing Child B touch another child in the private area or that Child B had ever injured another child. Child A also did not report being touched by Child B to Ms. Al

On 11/09/2021, I received a telephone call from Tia Moore, licensee designee and education specialist, regarding an incident at the Macomb County Family Resource Center (FRC) Head Start. Ms. Moore explained that Head Start was investigating regarding a child that was left behind without the rest of her classroom. Ms. Moore stated that information that she received was that Child A was lingering behind the rest of the class on the way outside and when the class got outside, Child A was still inside. Morningstar Early Learning Center (ELC) staff took Child A to the office. Child A could have been inside anywhere from a few seconds to a few minutes based on reports Ms. Moore received. On 11/12/2021, Ms. Moore submitted an incident report to the department along with written summaries from child care staff members and witnesses. On 11/17/2021, I conducted an unannounced onsite inspection at the Macomb County Family Resource Center (FRC) Head Start. The center shares a building and entrance with Morningstar Early Learning Center (ELC). I was admitted into the building by Morningstar ELC child care staff members. I met with Glynnis Rumfelt of Morningstar who had submitted a written statement to Macomb County FRC Head Start regarding witnessing Child A being left outside. During the interview with Ms. Rumfelt, she indicated that on 11/09/2021 at around 9:30a.m., she was in the office when the Morningstar ELC cook, Amanda Morello, came into the office saying she had found Child A in the hallway by herself. Child A is from the Macomb FRC Head Start program and her classroom was outside on the playground. Ms. Rumfelt told Ms. Morello that they would wait a couple of minutes to see if anyone from Head Start came back in looking for Child A. No one came in and Child A started crying so after about three minutes of waiting, Ms. Rumfelt took Child A outside to the playground. When she took Child A outside to the playground, Ms. Rumfelt asked the FRC Head Start child care staff members if they were missing anyone. They quickly looked around the playground and Ms. Rumfelt told them they had left Child A inside. The child care staff members looked surprised to see Child A with Ms. Rumfelt. I asked Ms. Rumfelt to clarify where the FRC Head Start child care staff members were 3 when she walked Child A outside. Ms. Rumfelt again stated they were already on the playground and the other children were playing when she took Child A outside. I then conducted an interview with Ms. Morello. Ms. Morello stated the day she found Child A in the hallway, she was coming down the main hallway and saw the Head Start classroom heading outside. She turned from the main hallway and was walking down the Head Start hallway when she noticed Child A walking in the Head Start hallway with her coat. Ms. Morello walked Child A to the Morningstar office and told Ms. Rumfelt that she had found Child A in the hallway. They waited about four or five minutes in the office to see if the Head Start staff came back to get Child A and when they did not, they took Child A out to the playground. The Head Start classroom and child care staff members were already on the playground and the children were playing when Child A was taken outside to her class. Both Ms. Rumfelt and Ms. Morello expressed concerns that when the Head Start classroom came back in, they overheard child care staff members telling Child A that she needs to keep up and stay with the class because it was not safe for her to stay behind. They stated they felt like Child A was being blamed for being left inside when it was really the child care staff member's responsibility to make sure they had all of the children with them. I then went to the Head Start classroom and contacted the education specialist and licensee designee, Tia Moore, who was in the building.
