On 6/20/22, I made an on-site inspection and interviewed Eryn Layman, site supervisor for the PEAK program. Ms. Layman was aware of the situation where Child A was put on the school bus to go home and dropped off at her home with no one to receive her. The school transportation should not have done that as it is their policy not to leave a child when no one is at the drop off to receive them. Ms. Layman said that the school district arranged for transportation to be provided since they were providing summer school part of the day. The PEAK program was provided with a list of children that were supposed to ride the bus home from the program. Ms. Lyman thought that the school had the written permission forms from the parents. She did not have any permission for any kind of transportation on file for the children; this included field trips. The only type of 2 permission that they had was a statement in the handbook stating that they would be taking field trips during the program. The parents of the children signed verification that they received the parent handbook and read it. Ms. Lyman contacted the program director, Mary LaChance. Ms. LaChance stated that they did not have permission on file from parents for the routine transportation or the field trip transportation. They were just trying to assist the parents and the school arranged this since they were offering summer school for a couple of hours. The children will get dropped off at the PEAK summer program go to the summer school for a couple hours and the teachers will drop them back to the program. In the afternoon, the busses would take some of the children home and some would stay until the end of the day when parents pick them up from the program. Monday was the first day of the program. They received a list of students that the school provided them as to which students would be taking the bus home. Child A was one of those students and she was put on the bus. They were notified that Child A was dropped off at her home without anyone to receive her. She resides in an apartment where there are only a couple other apartments. The door to the apartment complex is locked and she could not get into to her apartment. On 6/27/22, I interviewed Child A's Father. He received an email before the summer program started from the school that stated transportation department from the school would contact them prior to the start of the PEAK program so they can sign up for transportation. The transportation department's policy is that no child under the age of ten will be left alone if someone were not at the stop to receive them from the bus. He had not heard from the transportation department so he transported Child A to the program himself on the first day. He planned on picking her up from the program too since it had not been arranged through the transportation department for her to ride the bus home. During drop off Child A's Father did not tell anyone that she should take the bus and did not sign any permission form for her to take the bus home. He went to leave their apartment to pick her up from the program at about 5:20 p.m. When he left his apartment, he could see Child A outside crying. She said that the bus dropped her off and she could not get into the apartment. He figures that she had to be outside for about 30-45 minutes before he found her.
Child A (9 years, female) was placed on the school bus to go home without parental permission. She was left alone outside for a period before Child A's Father found her outside unattended.
