The center did ensure that child care staff members provided aerorooriate care and supervision to Child A. Uniid A cieniea mat anyone naa ever touched ner inappropriately. She denied anyone touched her or hurt her at the center. Child A's mother reports that in June 2021, Child A disclosed to her that someone had touched her privates however said nothing more. Child A was asked by her mother who touched her and where she was touched, and Child A stated she was lying. Child A continued to tell her mother she was not telling the truth when she questioned her further. Child A never reported being afraid to attend the center. Child A never refused to attend the center. To date, Child A has not disclosed to anyone else that she was inappropriately touched by anyone.

Child A age was attending Creative Beginnings Child Development Center and became afraid of going and no longer wanted to attend for unknown reasons. Child A recently disclosed being inappropriately touched by an unknown perpetrator. Child A age - has not disclosed a perpetrator. ■ 3 On 11118/21 I attempted to contact Child A's mother by telephone and there was no answer. A voice mail message was left requesting a return telephone call. On 11/18/21, I conducted an on-site inspection at Creative Beginnings Child Development Center. I met with the licensee designee/program director Michele Colores and an interview was conducted. UnIld As mother never informed anyone at e center that Child A haddisclosed she had been inappropriately touched by anyone at the center or at home. Child A never informed anyone at the center that she had been inappropriately touched by anyone at the center or at home. Child A's mother never told anyone that Child A was afraid or refusing to attend the child care center. Child A never displayed any fears when entering the center. Due to Covid the parents drop the children off at the door and staff are present at the door. Child A never refused to enter the building and usually would run inside to her cubby upon arrival. There were several incidents with Child A's behaviors while she attended the center. Child A would often start screaming for no reason and throw temper tantrums. Child A appeared to have sensory issues especially pertaining to her clothing. Child A would pull on her pants and once tried taking her pants off and had to be redirected that they don't take their clothes off in the classroom. Ms. Colores had to walk Child A to the bathroom and Child A was refusing. Child A always kept another change of clothing at the center as she had issues with urinating on herself. There were times when Child A wanted to change her clothes for no apparent reason other than she didn't like the way the clothes felt on her skin. Child A was old enough to go into the bathroom alone and staff would wait outside while she changed. Child A's mother reported she has had those problems for several years and was surprised Child A had never defecated on herself. She also reported that Child A had a long history of having urinary tract infections. There were times Child A was pre-occupied with the feel of her clothing )ot upset, she would rub ar on her skin or bite herself to the point she turned red. At times the child care staff would have I:0 move Child A outside the area of the other children fearful she would harm another child. Ms. Colores usually would be the person who would take Child A across the room and give her space to have her temper tantrum and she would try soothing her with soft words. I then met with child care staff member Morgan Zenz and an interview was conducted. Ms. Zenz does the secretarial work however does fill in as a child care staff member when needed. Her desk is just outside of the preschool area, and she CI ChildA's mother never informed anyone at e center that Child A haddisclosed she had been inappropriately touched by anyone at the center or at home. Child A never informed anyone at the center that she had been inappropriately touched by anyone at the center or at home. Ms. Zenz has remained in contact with Child A's mother as there is still a balance on the account and she has never disclosed any concerns about Child A being inappropriately touched or any concerns with the center. She just spoke with Child A's mother last week when she made a payment on her account. There were several incidents with Child A's behaviors while she attended the center.
