    R 400.8125 · R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.

    On 07/28/2023, I received an incident report from Lizzy Stritt, program director regarding Child A being injured by a metal bungee cord that was attached to a cage used for storing outside play equipment. I called Child Care Staff Member 1 (CCSM 1) on 08/01/2023. CCSM 1 stated that she was helping children make pizzas in a group of around 18 children along with another small group of 18 children totaling to 36 children in the gym at the time of the accident. CCSM 1 explained that Child A wanted to know how far the bungy cord could stretch. CCSM 1 explained that the children are not allowed to open the equipment cage due to the bungee cord being “dangerous” because of the two metal hooks that are located on 2 each end. CCSM1 stated that the children are aware that they are not allowed to go into the equipment bin without a Child Care Staff Member. CCSM1 did not see the accident with Child A due to her leading a small group but she did instruct CCSM 2 to take Child A to the office after she viewed his eye after the accident. On 08/01/2023 I interviewed Child A’s Mother. Child A’s Mother informed me that Child A suffered an injury to the Iris that was filled with blood at the time of the incident. There was no loss of vision and Child A will be out for at least a week per doctor’s orders. Child A’s Mother stated that this was a complete accident, and she was notified in a timely manner. Child A’s Mother did not have any concerns and spoke highly of the summer program. On 08/09/2023 I completed an on-site inspection, but the children were out for the day on a field trip. I interviewed Program director Lizzy Stritt. Mrs. Stritt stated that she was not in the gym when the accident occurred. Mrs. Stritt stated that the bin was kept outside but brought in due to the weather. Mrs. Stritt also explained that the kids could not open the cage due to the number of children in the room which will cause a huge mess. When viewing the bin, I noticed the metal hooks that were used to secure the cage which were used to keep it locked were missing, Mrs. Stritt informed me that the hooks were confiscated by EMS and were not returned. I called Child Care Staff Member 2 (CCSM 2) on 08/29/2023. CCSM2 stated that he was in the gym with CCSM 1, both Child Care Staff Members were leading projects with each having a group of 18 children, for a total of 36 children in the gym at this time. Child A went to play in another section within the gym after the project. After a few minutes CCSM 2 stated that he saw Child A “freaking out”. Child A kept asking CCSM 2 if his eye was swollen but he continued to cover his eye with his hand. CCSM 2 finally was able to get Child A to uncover his eye. CCSM 2 stated that when Child A uncovered his eye and opened it CCSM 2 began to see blood fill the eye. CCSM 2 reached out for help to by CCSM 1 who rushed over to assist. CCSM 1 walked Child A to the office. When CCSM 1 asked Child A what happened, he told her that he was hit in the eye with the bungee cord from the “equipment bin”. The program director Mrs. Stritt called 911 and Child A’s parents immediately after the eye was observed. While waiting for EMS an ice pack was given to Child A to place on his eye. 3
