Teacher 1 and Teacher 2 did not provide proper care and supervision of Child A at all times. When Teacher 1 stepped away from Child A to assist another child, and Teacher 2 was prepping for snack in kitchen area, Child A, who has bitten other children 12 times since 9/09/2025, bit Child B.

On July 4, 2022, I spoke to the Complainant. The Complainant stated that one month age Child A stated that while being picked up by another child she was dropped on her head. The Complainant did not take Child A to receive medical attention, as no injuries, marks or bruises were observed. The Complainant was informed two weeks later by Child A that she fell on the playground and hit her lip. The Complainant observed a small cut on the inside of Child A's lip. The Complainant stated that Child A did not inform child care staff members that she hit her lip. On July 7, 2022, I completed an unannounced on-site inspection at the child care center. I individually interviewed program director, Christie Reissig and child care staff members Alexa Ortiz, Jessica Signorello, Bobbie Druyor and Stacy Welch. Ms. Reissig denied that any child was dropped on their head while being picked up by another child. She stated that child care staff members do not allow children to pick one another up. She also stated that she has cameras in the center and would have observed if a child was dropped on their head. Ms. Reissig stated that she did not observe nor was she informed of a child falling on the playground and hitting their lip. Ms. Reissing stated that parents are given an incident report of all injuries and incidents. Parents sign the form, and a copy is kept on file at the child care center. An incident report is also forwarded to child care licensing for all serious accidents, injuries or illnesses. Ms. Ortiz and Ms. Signorello both denied observing a child being dropped on their head while being picked up by another child. They both stated that children are not allowed to pick up one another. Ms. Ortiz and Ms. Signorello both denied observing a child falling on the playground and hitting their lip. They both stated that incident reports are completed for all injuries and incidents and given to parents to sign, and a copy is kept on file at the child care center. Child care staff members Ms. Druyor and Ms. Welch both work with the infant and toddlers. They both denied observing a child being dropped on their head while being picked up by another child. Both stated that children are not allowed to pick up one another. They also both denied observing a child falling on the playground and hitting their lip. On July 17, 2022, I spoke to child care staff member Aimee Brubaker. Ms. Brubaker denied observing a child being dropped on their head while being picked up by another child. She stated that they do not allow children to pick up one another. She did not observe a child fall on the playground and hit their lip. Ms. Brubaker stated that incident reports are completed for all injuries and accidents "even if they get a 3 splinter". Parents sign the incident report, and a copy is kept on file at the child care center. Christie Reissig provided me a copy of the policy. I also reviewed copies of signed incident reports. On July 18, 2022, I spoke to Child C's Mother, Child D's Mother and Child E's Father. They all stated that they have received incident reports to sign when their children have sustained minor injuries while playing.

On July 4, 2022, I spoke to the Complainant. The Complainant stated that approximately three to four weeks ago Child B (female, age 2) was observed to have a diaper rash. The Complainant informed the child care staff members to "wipe" Child B after every diaper change. The diaper rash cleared up on the weekend while Child B was home and would reoccur on Monday when she returned to the child care center. The Complainant stated that this occurred "on and off for two weeks before clearing up". On July 7, 2022, I completed an unannounced on-site at the child care center individually interviewed program director, Christie Reissig and child care staff members Alexa Ortiz, Jessica Signorello, Bobbie Druyor and Stacy Welch. Ms. Reissig stated that she was not aware of any child having a reoccurring diaper rash approximately three to four weeks ago. Ms. Reissig showed me a copy of the 4 written daily records for infant and toddlers. Copies are provided to parents and kept on file at the center. Infant and toddler child care staff members Ms. Druyor and Ms. Welch both stated diapers are frequently checked at least every hour to ninety minutes, and between transition times. Both stated that children are thoroughly wiped during diaper changes. Ms. Druyor stated that she provides the primary care for Child B and completes her diaper changes, which includes thoroughly wiping her. She stated that in the past month she had not observed Child B to have a diaper rash. I observed the previous two months of written daily records noting times that Child B's diapers were checked and indicating if wet or dry. Diaper checks were observed to be completed every sixty to ninety minutes. No diaper rashes were noted. On July 18, 2022, I spoke to Child C's Mother, Child D's Mother and Child E's Father. Both Child C's Mother and Child D's Mother both stated that they did not have any concerns and were pleased with the care provided to their children regarding diaper changes. Their children have never had diaper rashes. Child E's Father stated that his child does not wear diapers, but he does not have any concerns and is pleased with the care provided by the child care center.

In addition, a copy is available for inspection and distribution at no cost at the Michigan Department of Licensing and Regulatory Affairs, Bureau of Community and Health Systems, Child Care Division, 611 West Ottawa Street, Lansing, MI 48933.
On July 4, 2022, I spoke to the Complainant. The Complainant stated that Child A and Child B are "starving" every day when picked up from child care. I asked the Complainant if Child A and Child B reported being hungry. The Complainant stated they did not but appeared to be "starving" when picked up, as they "eat a lot when they arrive home". The child care center provides meals and snacks. When asked if Complainant was informed of the meals served. Complaint stated that Child A and Child B were served vegetables, cereal, pasta, and lunch meats. I asked if Complainant discussed concern of children appearing hungry with the child care center. Complainant stated, "no, every time I address something it's dismissed". On July 7, 2022, I completed an unannounced on-site at the child care center. I individually interviewed program director, Christie Reissig and child care staff members Alexa Ortiz, Jessica Signorello, Bobbie Druyor and Stacy Welch. Ms. Reissig stated that children are served breakfast, lunch and two snacks. She has never observed any children to appear to be hungry throughout the day, as they are served more food if they request more. The child care center participates in the Child and Adult Care Food Program and food selections and portions are served according to the guidelines. Measuring spoons are utilized to ensure that appropriate portions are served according to the guidelines. Child care staff members Alexa Ortiz, Jessica Signorello, Bobbie Druyor and Stacy Welch all stated they have not observed any children to appear hungry throughout the day. Children are given more servings of food if requested. A copy of the Child and Adult Care Food Program guidelines was posted at the child care center. I observed the breakfast, lunch, and snack menus to follow the required guidelines. I also observed children eating lunch. The lunch met the requirements for quantity and food group selections. All children appeared to be satisfied and child care members were observed asking children if they had enough food. On July 17, 2022, I spoke to child care staff member Aimee Brubaker. Ms. Brubaker stated that she has never observed a child to appear hungry throughout the day. Children are given more servings of food if requested. She also stated the child care center participates in the Child and Adult Care Food Program and follow the food selections and portion sizes according to the chart. Measuring spoons are utilized to ensure that appropriate portions are served. On July 18, 2022, I spoke to Child C's Mother, Child D's Mother and Child E's Father. They all stated that their children have never reported being hungry throughout the day at the child care center or not receiving enough food. Child C's A Mother stated that child care staff members have told her that Child C asked for and was given seconds.

On July 4, 2002, I spoke to the Complainant. The Complainant stated that there was a lice outbreak at the child care center on last week. Children were sent home and parents were not notified of the lice outbreak. On July 7, 2022, I completed an unannounced on-site at the child care center. I individually interviewed program director, Christie Reissig and child care staff members Alexa Ortiz, Jessica Signorello, Bobbie Druyor and Stacy Welch. Ms. Reissig stated that she was on vacation the week of June 13, 2022. She was informed by child care staff member Aimee Brubaker on June 15, 2022, a child was observed to have lice. Ms. Brubaker called the parent to pick the child up. All other 7 parents were called and informed of the outbreak. They were informed that all bedding and clothing would be bagged up and ready to be taken home at pick up. A notice was also placed on the door of the child care center informing them of the outbreak. Child care staff members Ms. Ortiz, Ms. Signorello, Ms. Druyor and Ms. Welch stated that on June 15, 2022, parents were called and notified of a case of lice. All children's bedding and clothing were bagged up and sent home. A notice was also placed on the door of the child care center informing them of the outbreak. On July 17, 2022, I spoke to child care staff member Aimee Brubaker. Ms. Brubaker stated that on June 15, 2022, parents were called and notified of a case of lice. All children's bedding and clothing were bagged up and sent home. A notice was also placed on the door of the child care center informing them of the outbreak. The child care center was sanitized. Child care staff members checked children for lice before entry into the child care center. I observed the notice that was posted for parents. The notice informed parents of a case of lice. The notice also provided information about the symptoms and prevention measures recommended by the Us. Centers for Disease Control and Prevention. On July 18, 2022, I spoke to Child C's Mother, Child D's Mother and Child E's Father. They all stated that they were called and informed of a case of lice. They also stated that they were given their children's bedding and clothing to take home. They also observed a notice on the door informing parents of the case of lice. Children were checked for lice before entering the child care center.

During the investigation, I completed two unannounced on-site inspections to the center with no concerns. I observed the entire center to be following the required caregiver to child ratio requirements during both visits. I interacted with Staff Member 1 during both onsite inspections. Staff Member 1 provided the children with appropriate care and supervision. I did not smell any alcohol. Staff Member 1 actively interacted with the children. The children approached Staff Member 1 for care and attention. I interviewed Ms. Reissig, licensee. She denied the allegations. She stated that the center maintains the required caregiver to child ratio requirements. In addition, she does not count herself in the ratio requirements so that she can be an extra person present at the center. She also can bring in additional staff members from her neighboring center, as the staff members are hired to work at both locations. Ms. Reissig allows the staff members to leave the building for lunch if there are enough staff members to accommodate the ratio requirements. Ms. Reissig stated that she and Staff Member 1 are friends outside of work, and they sometimes go to lunch together. This does not occur every Wednesday. They have not eaten at an Applebee's restaurant in over a year. She has "never" observed Staff Member 1 consume alcohol while on lunch and then return to work at the center. She has never smelled alcohol on any of the staff members. Ms. Reissig has never had any concerns regarding Staff Member 1's care and supervision of the children. Staff Member 1 has never acted in a way that has caused her concern. She has not received any parent complaints regarding Staff Member 1. The children have not disclosed any concerns. I reviewed the center's staff and child daily attendance logs for three Wednesdays including 10/7/20, 10/14/20, and 10/21/20. The center appeared to be in compliance with caregiver to child ratio requirements. I interviewed Staff Member 1. She denied the allegations. She stated that the center maintains the proper caregiver to child ratio requirements, even during lunch time. She stated that she sometimes goes to lunch with Ms. Reissig, but that this does not occur every Wednesday, nor do they go to Applebee's. On 10/21/2020, she went to lunch with Ms. Reissig. When she left for lunch, Staff Member 4 was caring for 10 children ages 3 years and older, thus following caregiver to ratio requirements. She did not drink any alcohol on 10/21/20. Staff Member 1 stated that she would "never" drink alcohol and return to work. She stated that she is "hurt and upset" that 611 W. OTTAWA. P.O. BOX 30664. LANSING, MICHIGAN 48909 www.michigan.gov/lara . 517-335-1980 someone would accuse her of smelling like alcohol around the children. She stated that perhaps someone smelled her flavored gum through her face mask or smelled the center's hand sanitizer that smells like alcohol. Staff Member 1 believes that she provides the children with good care and supervision. I interviewed Staff Member 2. She denied the allegations. She stated that the center is "very strict" with following the required caregiver to child ratio requirements. In addition, Ms. Reissig is rarely assigned to a room, which makes her an extra staff person on-site. Staff members can the leave the building for lunch only if there are enough staff members in place to provide for proper ratio coverage in the rooms. In addition, the staff members at Ms. Reissig's neighboring center are considered employed at both locations, so they too can be brought over to provide additional staff coverage if needed. Ms. Reissig and Staff Member 1 sometimes go to lunch together, but it is not every Wednesday. On 10/21/20, Staff Member 2 interacted with Staff Member 1 before and after she returned from lunch and she had no concerns. She has never smelled alcohol on any of the staff members. Staff Member 1 has never acted in a way that caused her concern, saying that the children receive appropr
