On 5/23/2022, I completed an unannounced inspection at the center. I spoke with program director and licensee designee, Natalie Church. She stated that on 5/16/2022, Child A arrived at the facility a little drowsy. Child A's mother alerted Ms. Church that she had given him new medication that morning. Ms. Church stated that no caregivers had administered any medication to Child A that day. Child A became less responsive while in care and Ms. Church called his mother to come get him and offered to call Emergency Medical Services (EMS). Child A's mother did come and agreed to have EMS called to have Child A checked 2 out. Child A saw a doctor and was able to return to school his next scheduled day, 5/19/2022. On 5/23/2022, I observed Child A in class. He appeared healthy and well adjusted. I reviewed his medication log in his file. He was taking an over-the-counter vitamin supplement at the center up until 3/14/2022. No other medication had been administered since then. On 5/26/2022, I spoke with the complainant. The complainant did not know who gave Child A medication on 5/16/2022, but Child A was lethargic and sleeping while standing up. Ms. Church called 911. The complainant does not know the outcome of the situation. On 5/26/2022, I spoke with Child A's mother. Child A's mother stated that Child A was on a new medication, and she was instructed to increase the dose a little bit at a time over several days. She administered the medication to Child A at their home on 5/16/2022. There had not been a problem with the new medication at that point. At around 10:45 a.m. the school notified her that Child A was "out of it" and she went to the school. At around noon they decided to call 911. Child A was seen by a doctor and released. He is fine. Child A's mother thought the facility and the staff handled the situation well and she is very happy with how they cared for Child A.

On 5/23/2022, I observed two rooms being used for child care. The toddler room had four children and one staff member. The preschool room had 17 children (ages 3 and 4 years old) and two staff members. I reviewed the attendance logs for the past week (5/16/22-5/20/22). It appeared the center was in ratio the entire time for each day. Ms. Church stated that hiring has been hard lately. She had to bring a staff member from another location to work at this location, and she also disenrolled six part time families. She did this to ensure a consistent schedule and to maintain ratio until more staff can be hired. I spoke with the preschool room staff members Rose Allport and Vicky Lang. Vicky opens in the morning at 6:30 a.m. and Rose arrives about an hour later at 7:30 a.m. They both stated there is not a problem with ratio at the center. They confirmed that staff has come from another center and that some infant families were let go to keep a good staff to child ratio. On 5/26/2022, I spoke with the complainant. The complainant stated that sometimes in the mornings children would arrive before additional staff members arrived. The complainant specifically referred to 5/16/2022. I reviewed the staff attendance once more. There were two child care staff members at 6:30 a.m., three at 7:00, four at 8:30 until 8 :45. There are only 22 children enrolled. Based on schedules, only four of the five infant/toddlers enrolled are present in the infant/toddler room each day, requiring a minimum of one child care staff member. There are 17 children enrolled in the preschool room (3- and 4-year-olds) requiring a minimum of two child care staff members.

I looked through all the rooms and did not see or smell mold. There were no staff lockers. On 5/26/2022, the complainant stated that there was mold near the side entrance that the preschoolers use. The mold has been there since 2019. I reviewed the most recent environmental health inspection dated 2/1/2021. The facility received an "A" rating and there was no mention of mold.
