The complainant was not interviewed since there was no contact information provided. I made an unannounced on-site visit to the center on 9/29121 and interviewed CCSM 1 who learned of COVID diagnosis on Saturday, 9/25/21 and that Assistant Director Allie Savage notified all parents whose children attend classroom. They were notified the same day or on Sunday (9/26121). last day at the center was Wednesday, 9122/21. in the classroom on the main level. Then re is another classroom on the lower level. The children who attend it have no contact with children on the main level except that some may walk through the room upon arrival. The classes on the main and lower level are kept separate except during before and after care when _ is not present. CCSM 1 said they are keeping the children outdoors as much as possible and use a large tent and pavilion as outdoor classrooms. Shortly after I arrived, program director Kristina Shaw came to the center. The content of my interview with her was substantially similar to my interview with CCSM 1. She said that they first learned on Saturday, (9/25) or Sunday (9126) and that parents from classroom on the upper level were notified of the diagnosis on Sunday ( Allie Savage arrived at the center as I was interviewing Ms. Shaw. She and Ms. Shaw said Ms. Savage said she notified all parents in classroom about on 9/26/21 after she first learned of thatweekend, Four of those children who attend classroom were present on the inspection date. Many parents had PcoseWno eep their child home after they received notification. Ms. Shaw said she had not yet notified the health division about the positive case. I asked Ms. Shaw to contact the health division, get the name of the person with 3 whom she spoke, and to follow their recommendations regarding any quarantine needs while I was at the center. I also wrote out these steps and asked her to keep me posted on the progress. Ms. Shaw called me after I left the center and left a voicemail saying that her call to the health division was transferred and that she was unable to obtain the information she needed from them. I was unable to reach her by phone when I returned her call but left a message telling her I would help her and sent the below link from the State of Michigan as a general resource until she received specific information from the Oakland County Health Division. She said she informed Ms. Savage of on Wednesday 9122121), not that on both days Soon after interviewing on October 1, I called Ms. Shaw for an update. She told me she still had no contacteedd the health division regarding the question of quarantine, and that the last instruction they had given her was to notify them at cd( oak ov.com which she had not done. I asked her to contact them. I also asked for clarification regarding the timeframe for learning of positive test result and for notifying arents based on who had !PiPesaid she was not sure notified Ms. Savage of the diagnosis and would need to check. She did not believe from the lower level on Monday or Wednesday, the day . None of the parents of children from the lower level were informed of e illness. I asked her to get clarification on these questions and reiterated them in an email. When I contacted Ms. Shaw by email on 10/4/21 to ask about the status, she indicated by email that the health division informed her that any quarantine period had ended. The email did not address the question regarding the timing of parent notification which was the allegation that initiated this investigation. I called Ms. Savage on 10/5/21 for clarification on this timing question and left a voicemail message requesting a return call. I called her again on 1018121. She said on 9/22/21 notified her of a likely positive COVID result and would stay home and that she would later confirm the result. Ms. Savage sai

On 09/17/2020 Child Care Staff Member 3 worked at the center before she turned 18 and she was never allowed to work alone with the children. On 09/29/2020 Ms. Sarasin stated that the center employs high school students as interns that are under the age of 18, because she wants to provide high school students with skills in the child care field so that they can build a career. On 10/05/2020 Ms. Shaw and Ms. Sarasin stated that the student interns complete a child care training program at the Milford High School. On 10/05/2020 I requested documentation from Ms. Shaw, and she stated that she would contact the teacher for documentation of the training provided to the students that work at the center. I explained to Ms. Shaw and Ms. Sarasin that students under the age of 16 are not allowed to work at the center and the student interns under the age of 18 cannot be compensated for interning at the center. On 10/22/2020 Ms. Shaw provided the course outline for the classes that are part of the vocational program provided at the Milford High school. The classes are titled 3 Education General 1- Early Childhood and Education General 2- Educational Careers for grades 10,11, and 12. Each class is one semester with the completion of one year after the completion of both classes. After the students have completed part one and two of the training, they would have the vocational educational requirements.

The team is not mandated or required to wear equipment." and "if your child or a family member tested positive for coronavirus or any other virus, please inform the director and stay home until your doctor has cleared you to return to school/work with a doctor's note. If an on-site team member has tested positive for coronavirus, parents will be notified and will have the option to self-quarantine or continue to attend." The center's child, staff, and volunteer illness exclusion policy states "Children, staff, and volunteers who may be infectious or who have physical symptoms of an illness or communicable disease will be excluded from All-Stars." Child Care Staff Member 2 stated that the parents were informed that Child Care Staff Member 3 tested positive for Covid-19. On 09/17/2020 Child Care Staff Member 3 stated that she tested positive for Covid- 19 on 07/02/2020. She had the test completed after she finished working on 06/29/2020. When the results were positive, she did not return to work. She was informed by the health department that she had been exposed to someone that tested positive for Covid-19 which was why she got tested. She informed the health department that she worked at the child care center. She informed either Lead Caregiver 1 or Ms. Shaw when she received the test results. She could not remember which one she informed. She quarantined for two weeks. She did not return to the center because she went to college. The parents were notified about Covid-19 exposure on the centers Bright Wheel app. On 09/29/2020 Ms. Sarasin stated the parents were notified immediately after she was informed that Child Care Staff Member 3 tested positive for Covid-19 on 07/02/2020. The parents received a phone call from Ms. Sarasin and MS. Shaw on A Saturday 07/04/2020 and Sunday 07/05/2020 because the center was closed for the 4th of July. There was not a written notice because the parents were informed before they returned to the center. The parents were informed in writing with the center's Bright Wheels App, parents received a phone call, and a notice was posted at the center with Covid-19 symptoms to watch for. On 10/05/2020 Michelle Fruehan stated that she did not receive an incident report from the center when Child Care Staff Member 3 tested positive for Covid-19 in July.

On 08/25/2020 Child Care Staff Member 1 stated that the child care children are allowed to play in the bounce house outside. The bounce house has been used five times this summer. There are only three children allowed in the bounce house at one time. Last Monday 08/17/2020 was the last time that the bounce house was used at the center. Ms. Shaw stated that the bounce house has been used by the child care children this summer. She has not observed the children using the bounce house recently. On 08/25/2020 the bounce house was not set-up, it was deflated, and stored behind the center. On 09/17/2020 Child Care Staff Member 3 stated that she has observed the children playing in the bounce house at least twice a week. There would be no more than five children in the bounce house at one time. On 09/29/2020 Ms. Sarasin stated the children were previously allowed to play in the bounce house outside.
