    I could not contact the complainant to get further information because no contact information was provided. On 6/10/21, I went to the home and interviewed Heather Beckman and Deborah Burton, licensees. Ms. Beckman, Ms. Burton, Kayonna Chase and Tina Miller, child care staff members were present with 12 children. Seven of the children were unrelated and five of the children were related to Ms. Beckman and Ms. Chase. The children were playing outside except for Ms. Beckman's children and Ms. Chase's child that was upstairs napping. Ms. Beckman explained that there was an incident with Child A getting out of the front door a couple of months ago. Everyone was in the backyard having outdoor time. Ms. Beckman said there was one child care staff member inside changing a child. Child A ran in the house and straight to the front door. She observed him going out the front door and she ran after him. He did not get off the porch. It was about pick up time because she recalled Child C's (female, 2 years) Mother was there at the time it happened and witnessed it. Ms. Beckman reviewed her text messages and found out that it occurred on 4/22/21. The door alarm was installed on 4/26/21. She showed me the text conversation with Child A's Mother regarding this incident and her installing an alarm on the front door as a precaution. Ms. Beckman stated that it was a scary situation and that she wanted to make sure it never happened again. She informed the parents of the situation and about the installation of the door alarm. It is turned on if everyone is outside. Ms. Beckman provided me with the attendance for the day of 4/22/21. There were 12 unrelated children in care that day. The two licensees and two child care staff members were also present. There were 5 unrelated children also present in the home. Ms. Beckman was within capacity and ratio on 4/22/21. Ms. Burton could not think of another incident where a child had gotten out of the home besides the one with Child A. She thought it was early May because she remembered it was a warm day. Everyone was outside except for Ms. Miller who was inside changing a child's diaper. Ms. Chase was not working at the time that this occurred and Ms. Miller stated that she came in from outside that day to change a diaper and did not see exactly what happened with Child A. I left two voice mail messages for Child A's Mother on 6/14/21 and 6/16/21 and did not receive a return phone call. 3 On 6/16/22, I interviewed Child B's (male, 21 months) Mother, Child D (male, 4 months), Child E (male, 3 years), and Child F's (female, 6 years) Mother, and Child G's (female, 4 years) Mother. All of them were aware of the situation of a child getting out the front door and the alarm being on the front door as a precautionary measure. No one had concerns regarding supervision and were happy with the child care the children were receiving. The children love going to the child care home. I interviewed Child C's (female, 2 years) Mother. She was sitting in the backyard with everyone when she went to pick up Child C. Ms. Miller was inside diapering a child. She observed Child A go through the home and open the front door. Ms. Beckman observed him doing this and got him before he was off the steps. There is now an alarm on the door and buzzes every time it opens. The incident was a "freak" thing. Child C has been attending the child care for over a year and she trusts Ms. Beckman with her life. On 6/25/21, I went to the home and Ms. Burton and Ms. Chase were caring for 10 children with five of them being related. I interviewed Kimberly Chapin, child care staff member. She has only worked a few times in the last couple of months. She was not aware of the details of the child getting out of the home. She just knows that there is now an alarm on the door because of it. She has never observed any child care children playing in the front yard as they are not allowed to play there.
