Upon assignment of the special investigation, I reviewed an incident report regarding Child A losing consciousness on the playground and being transported to the hospital. On 12/15/2022, I initiated the investigation by speaking 2 with Child A's parents about the incident. Child A is back at child care today with no restrictions. Her parents were very thankful for the staff who helped with Child A. Child A's mother verified that she received a call from CCSM Stephanie Seeley on the day of the incident informing her of what was happening with Child A. Child A's mother also verified that she had received a copy of the incident report. Child A's parents independently obtained the emergency medical services (EMS) call online. They said at the 16-19-minute mark on a 30-minute video, the call comes in regarding Child A. Their reporting of the EMS call matches what is reported in the incident report and by involved staff. Both parents said they remain unsure of what really happened as Child A does not typically hold her breath but has on one other occasion. A concern was raised regarding the statement in the incident report that Child A has a history of choking on food and used to see a therapist for it. Child A's parents report that they never told the center that Child A had a history of choking so they wondered why it was assumed that Child A was initially choking. A concern was also raised regarding who was giving Child A back blows and why if she was not choking. Child A has returned to the center for care and Child A's parents do not have concerns regarding the care that Child A receives. On 12/15/2022 I then went to the center and stated my purpose. I reviewed the center file for Child A. There are five documents of interest, the first being a "tour information card" dated 05/06/2022 which has a notation on it that Child A does not hold her own bottle and is "still on baby food, has problem with gag reflux". The child information card signed on 05/08/2022 by Child A's mother indicates that Child A has no special needs, allergies, or special instructions. The file contained an up to date medical for Child A dated 03/23/2022 that references Child A as a "healthy child." An updated child information card signed by Child A's mother on 07/05/2022 does indicate that Child A is allergic to amoxicillin The center provided an email from Child A's mother dated 12/05/2022 giving permission for Child A to be observed during snack time and play time to be assessed by Early On. I had LD Stephanie Beckett show me where the incident happened on the playground. We retraced the steps that were taken that day and it was less than one minute in accumulated time to get from the playground to the front desk where first responders arrived. The play area is fenced in and supports staff's accounts of what happened. I verified that all involved staff are trained in CPR. I interviewed CCSM Liz Allen who called 9-1-1 from her personal phone which she reports keeping with her at all times. She was able to show me the screen shot verifying that she called 9-1-1 on 12/12/2022 at 3:15 PM for a duration of six minutes. Liz and her class were eating a snack in a designated area of the center that overlooks the toddler playground. CCSM Faith Clark came in yelling "help, she's choking" or something along those lines. Liz went to the front desk and retrieved program director, Crystal Forsberg, who came and took Child A from Stephanie Seeley and followed the instructions given by the 9-1-1 operator who was on speaker phone. Liz said she observed Child A to be unconscious at the time she 3 entered the building; she estimates for 30 seconds or less. Liz observed Stephanie Seeley reach into Child A's mouth to clear it and to perform back blows. Child A's mouth was empty. PD Crystal Forsberg also administered back blows while taking Child A to the front desk area to wait for EMS to arrive. Liz verified the contents of the incident report and said she knows more about
