On 8/11/2025 and 8/13/2025, the licensee designee/program administrator instructed teachers in Infant Room 2 to take Child A to Toddler Room 1 as when Child A arrived there were only two teachers in the room and Child A would have made them out of ratio as there were already 8 children in the room. During nap time on 8/11/2025 and 8/13/2025 staff placed Child A on a cot/mat during rest time which is prohibited for infants.

On 8/14/2025 the Toddler Room 1 and the Two's Room were required to be at a 1:4 ratio based on the ages of the children and both rooms were out of ratio.

On 8/14/2025, Toddler Room 1 was found to be at a 1:8 ratio with five children awake. The Two's Room was found to be at a 1:12 ratio with three children awake. Based on the age of the children in the room’s ratio should be maintained at 1:4 once the first child wakes up as the required ratios in the room must be met immediately.

On 8/11/25 and 8/13/25 teachers were instructed to move Child A from the Infant 2 Room to the Toddler 1 Room to maintain ratio. The Toddler 1 Room is licensed for children ages 12 months through 24 months. Child A was only 10 months of age at the time he was moved to the Toddler 1 Room. The center did not have written permission from the department to modify the terms of the license.
