On January 5, 2023, I spoke to Child Care Licensing Consultant, Jennifer Blake, about the allegation. She stated that on January 4, 2023, she had received an emotional phone call from Cynthia Strickland describing an incident that occurred involving Child A (male, age 12 months) where a `pop`; was heard in Child A's wrist while Cynthia Strickland was holding the child. The next day, Child A was taken to the doctor and diagnosed with Ms. Blake instructed Cynthia Strickland to complete an incident report and send it to her. Ms. Blake forwarded a copy of the completed incident report to me. The incident re ort signed by both Cynthia Strickland, and her child care staff member, stated that Cynthia Strickland received a phone call on January rom Child A's Mother that stated Child A was seen at Cynthia S~tricklaannd and both denied any knowledge of injuries to Child A when he was in care on January 3, 2023. retraced Child A's entire day and stated that after drop-off, Child A was fussy, so laid him down for a morning nap. Child A cried for a little bit and then tell asleep. When he woke up, "he threw a fit" but calmed down when he was given a snack. Child A did not want to play in his bouncer and cried again. He played for a while on the floor and then was upset again at lunch time. He ate lunch and then was fussy again after lunch, so put him down for a nap and he fell asleep. When he woke up from nap, Child A was fussy until they fed him a snack. Then said Child A continued to be fussy most of the afternoon and she held him and tried to engage him in different activities. said Child A was "fine" at pick-up time until another child left out e front door. After the child left, Child A "tugged his arms" while Cynthia Strickland was holding his hands "as he was standing up and it made his wrist do a little pop and he started to cry.' The child care staff members checked him over and did not see anything wrong with his wrist and assumed he was crying because the other child went home. When Child A's Father arrived to pick up Child A, said she told him that Child A had been fussy that day. On January 5, 2023, I spoke to Child A's Mother by telephone who said that Child A began attending Cynthia Strickland's child care home on October 10, 2022. She said Child A never adjusted well to the child care home and would come home fussy in the evenings, but he was not fussy on the weekends or when other people would provide care for him. Cynthia Strickland and never provided nl many details about Child A's day and it was normal that they would have Child A ready to go home at pick-up time. On January 3, 2023, Child A's Mother said she dropped Child A off at the child care home around 7:45 AM and Child A was in a good mood after having been home over the holidays. Cynthia Strickland nor ever contacted her that day to provide any updates about how his day was going. Child A's Father arrived to the child care home between 5:00-5:30 PM to pick up Child A and the child care staff members had Child A's coat on him and "rushed them out the door." Child A was crying, and Child A's Father asked what happened, but the child care staff members did not answer him. On the way home, Child A fell asleep in the car but continued to cry and whimper in his sleep. Child As Father drove Child A around the neighborhood to let him rest until 6:00 PM when he finally went home. Child A continued to be fussy when Child A's Parents fed him dinner. They noticed during dinner that Child A did not want to use his right arm, but assumed it was because he was overly tired and had not napped well at the child care home, so they put him to bed around 7:00 PM and he slept all night. On January 4, 2023, Child A's Mother said Child A woke up around 5:00 AM and she noticed that he was still not using his right arm. He did not want to hold his bottle in his right hand and when she put him down on the floor to crawl, he collapsed on his right side and would not put any weight on it.
On January 3, 2023, Child A's Father picked up Child A (male, age 12 months) from Cynthia Strickland's child care home and Child A was crying but the caregivers rushed him out of the door and did not provide Child A's Father with any explanation. Child A whimpered and cried all evening. On January 4, 2023, Child A would not use his right arm and was taken to the doctor where he was

On January 3, 2023, Cynthia Strickland did not promptly report to Child A's Parents an incident that occurred moments before Child A's Father arrived to pick up Child A. Cynthia Strickland said she was holding Child A's wrists while he was standing between her legs, and she was holding his hands. Child A bent his knees and Cynthia Strickland felt a tiny "pop" in his wrist. Had Cynthia Strickland mentioned the unusual popping noise she heard in Child A's wrist, Child A's Parents would have known to secure medical care sooner and Child A would not have been in pain all night with his right elbow dislocated.

On January 6, 2023, Cynthia Strickland had an interior basement room locked that was in the required path of escape for infants and toddlers napping in pack-n-plays in a bedroom. The door could not be unlocked from inside the bedroom unless the split door was opened which would have required special knowledge that a child might not have. I
