On 6/20/23, I made an on-site inspection at the center. I interviewed April Schomaker, program director and Corey Nichols, assistant director. Mr. Nichols is the program director for the Head Start license in the building during the school year. He is Ms. Schomaker's assistant during the summer and in charge when she is not available. Upon my arrival the front door to the center was open. The free meal program operates at the building so the door is open during the day for a couple hours during meal times. Most of the time the door is locked, and parents must be let into the building to pick up their children. Mr. Nichols has been working during the opening and closing hours during the first couple of weeks to assist staff and to make sure the program runs smoothly. If staff do not know who the person is picking up the child, they will ask for identification. They do not let 3 people they don't know into the building when it is locked. Once they get to the classroom the child care staff have access to the child information records to check if necessary. On 6/23/23 and 7/10/23, I called and interviewed the following child care staff members: Kathryn Nock, Karrie Lumsden, Caitlin Trejo, Alyssa Forbes, Ashlee Thompson. They all said that they either ask for identification, ask another coworker, or contact Ms. Schomaker to assist in making sure that the children are leaving with the appropriate person. The newer staff would have the other child care staff that have been there for longer release the children. They had more knowledge of the parents. The first week of camp some of the children had a skyward print off from the school since not all children that came to the center had a child information card in the notebook. This print off included emergency contacts and individuals to whom the child could be released. These are completed by the parents for school purposes.

The school age summer program started and child care staff do not know who they are releasing when the child is picked up from the center. April Schomaker, program director, is not available for staff to ask questions. All child care staff are not properly trained and do not have CPR. All the school age children do not have required paperwork. Child care staff do not have cleaning supplies available to them on a regular basis.

Ms. Schomaker and Mr. Nichols stated that one of them is always available when the center is operating. Mr. Nichols has been opening and closing for the first couple of weeks to make sure everything goes smoothly. Even if he is in his office doing work for Head Start, child care staff know they can contact him. Ms. Nock, Ms. Lumsden, Ms. Forbes, Ms. Erickson, and Ms. Thompson all stated that they felt that they could get a hold of Ms. Schomaker or Mr. Nichols if they needed to do so. Most of the time one of them is in the building. Ms. Eastman is a new hire for the school age program. Ms. Schomaker has not been in the building as she was on vacation. Mr. Nichols has been there. 4

(3) Before caring for children, all child care staff members and unsupervised volunteers who work directly with children shall be trained on prevention of shaken baby syndrome, abusive head trauma and child maltreatment, and recognition and reporting of child abuse and neglect. 5
Ms. Schomaker provided me with the staff CPR certification records. Sixteen child care staff have current CPR/first aid certification. Mr. Nichols has his current certification and has been opening and closing the first two weeks of the summer program. Payton Ross-Moeller did not have CPR training within the required time frame. She has been employed with the center for over a year. There are a couple of new staff who do not have the health and safety training because health and safety courses one and two could not be found on Miregistry informed her that there was a communication that was sent out stating that the training is now modules. Mr. Nichols remembered that he did see that communication regarding the health and safety training changing to the modules. Ms. Erickson stated that she did receive a few days of training prior to the start of the school age summer program. She did not take any health and safety training. When Miregistry was reviewed Ramona Rodriguez and Payton Ross -Moeller did not have all the required health and safety training prior to working with children.

(10) All child care staff members who work directly which children are required to be trained in first aid and pediatric, child, and adult cardiopulmonary resuscitation (CPR) within 90 days of being hired. Prior to issuing a license to operate a child care center, and prior to the renewal of a license, the department shall verify that at least 50% of the child care staff members who work directly with children are currently certified in first aid and pediatric, child, and adult CPR. Each of these child care staff member's first aid and CPR certificates must be valid and retained on file in the center.
Ms. Schomaker was on vacation the first week of the summer program. Mr. Nichols is in charge when she is out of the center. Ms. Schomaker was contacted while on vacation because there was some paperwork for the children not on file. The is a summer school program that operates during the week. Some of the children who participate in this program will be with the child care center children while they wait for their turn to get tutored. As an immediate solution she had the school staff print off skyward information so that they had contact information until they received the required paperwork. She has been working on obtaining the paperwork since she returned from vacation. I reviewed the school age summer program records. Sixteen children that attended the child care program did not have child information cards. There were seven children that did not have a school age health statement. There were six children that did not have signed verification that parents received the required information.

R 400.8143 · R 400.8143 Children's records. (11) A center shall maintain an accurate record of daily attendance at the center that includes each child's first and last name and each child's arrival and departure time. Electronic records may be used. If electronic attendance records are used, then they must be available to the department at the time of an inspection. If the electronic attendance records are not available during an on-site inspection, then the center is in violation of this rule.
