On 9123/2020 a bruise was observed on Child A's leg after she had been in care at the child care facility. 7 PR and I completed an onsite inspection at the child care facility on O. We spoke with licensee Lynnmarie Kinkema, program director Kim Dezeeuw, and child care staff members Debra Mys, Brenda Hamman, and Nicole Zeerwp. Ms. Dezeeuw reported that she does not work in the infant room but does speak with the parents of the children in that room. She has not noted anyone having any concerns with the care their children are being provided and she is not aware of any infants having any bruises on their legs. Ms. Mys stated that she works in the infant room for about 10-15 minutes a day, maybe 2-3 days a week, to help out when child care staff members are trying to get all the infants fed. She was unaware of any infants ever getting injured. Ms. Hamman stated that she has been working at the child care center for approximately six week and overall, it has been going well. She has cared for Child A on occasion and she has never noticed her getting hurt. She reported that Child A's Mother mentioned 2 weeks ago or so that Child A had a bruise on her leg. Ms. Hamman looked and did notice maybe where one was healing on her calf. The only thing child care staff members could think of is that sometimes Child A's leg does stick out the crib when she rolls, but child care staff members just remove it from the slats. Ms. Hamman also thought there was a chance that during tummy time maybe another child dropped a toy on her, but she never recalled Child A crying from any incidents. Ms. Zeerwp reported that she has worked at the child care center for in She stated that she is the primary care provider for Child A and was"to that ChildA's Mother mentioned to staff a couple weeks ago that Child A had a bruise on her leg. Ms. Zeerwp had never noticed a bruise on her leg, but after it was brought to her attention she did look and saw a faint bruise on her leg. She stated that child care staff members hypothetically thought that when Child A was rolling around while falling asleep, she may have gotten her leg stuck in the slat. Child care staff members have had to move Child A's leg from the slats and maybe that is when she got a bruise, but Child A has never cried like she had been hurt. Ms. Kinkema helps out in all the rooms. She stated that Child A's Mother never mentioned the bruise to her and if something happens to the children in care the child care staff members write incident reports noting the injury. and I made phone calls to parents on 10/20/2020. We spoke to Child B's Mot"er, 's Mother, and Child D's Mother. Child B's Mother stated that Child B (male, age 5 months) has been attending the child care cente . Overall, the center is fine, but she does have some frustrations withItePReMan iness of the center. Child B has come home with scratches on his face and she is not sure how they occurred. When she drops Child 3 B off, the staff seem too busy or like they are not wanting to take him. She usually sets him down on a mat on the floor at drop off. Child C's Mother stated that she has two children at the child care center, one male that is age four and Child C (male, age 4 months). She has no concerns with the center. She feels like they treat the children like family instead of profits. Child C's eldest son has gotten a gash in his head before, but he is very active and she did not have any concerns with the injury. Child D's Mother reported that Child D (male, age 1) has been enrolled at the child care center since . She has no concerns with the supervision Child D receives at the center nor has she noted any injuries. APPLICABLE RULE R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times. ANALYSIS: There is no evidence to support that the child care staff members did not provide appropriate care and supervision to Child A or that Child A. It is u Child A is not wiped during diaper changes at the child care facility. 4 During our onsite inspection on 10/5/2020 I observed the diaper changing protocol in the infant room. It stated that when changing an infant, the child should be wiped during each diaper changing session. Ms. Dezeeuw stated that infants' diapers are checked every hour. Staff note whether the infant is wet or dry on the child's daily sheet and they change them if they are wet. She has never had any parents bring concerns to her about their infants not being wiped. Ms. Mys stated that she does not do diaper changes because she is just in the infant room to provide extra support during more demanding times. Ms. Hamman stated child care staff members check the diapers of the infants every 30 minutes to maybe two hours if they are sleeping. They right down all diaper changes and note if they are wet or have a bowel movement; not if they are dry. She wipes the infant no matter if they are just wet or have a bowel movement. Ms. Zeerwp stated that they check the infants maybe every 2 hours, sometimes every hour. For some time, Ms. Zeerwp was not wiping the infants when they were just wet, but since Child A's Mother requested that she be wiped all the time she has started always wiping at diaper changes. Ms. Kinkema reported that Child A's Mother did bring her concerns about Child A not being changed enough to her attention. The policy at the center is that the infants be checked every one to two hours and there was one occasion when the daily sheet reflected Child A was not checked for three hours. She told the staff in the infant room to make sure they are wiping the infants every time they change them. Child B's Mother did not have any concerns with the center and the changing of the diapers of Child B. Child C's Mother reported both of her children have . She reported the diaper changes do seem to be an issue because if Child changed right away, he will get a rash. Child D's Mother reported there are notes on a board when you walk into the infant room that have information on diaper changes. She does not typically look at the board because Child D is very routine when he gets changes. She has no concerns for diaper changes not occurring often enough or rashes occurring. Though Child D's Mother reported that out of all her children, Child D is the most resistant to rashes. Child D has come home in a different diaper a few times and it is not for lack of not having his diapers at the center. 5 APPLICABLE RULE R 400.8125 I Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times. ANALYSIS: Upon Child A's Mother indicating to the child care staff members that Child A needs to be wiped even when just wet, the child care staff members made the change to properly care for Child A. On 03/25/2022, I received an allegation stating that for the month prior, the two-year-old classroom has a ratio of 1 child care staff member to 7 or 8 children in the morning. The allegation states that this has been an ongoing issue and that there may not be enough staff at the center. On 03/25/2022, I interviewed the Complainant, who stated that approximately 95% of the time they drop their child off prior to 8:30 A.M., the Farm/breakfast room is out of ratio. The Complainant has read previous special investigation reports where this has been brought to the licensee designee Lynnmarie Kinkema's attention. Ms. Kinkema will come into compliance while child care licensing is involved, but then eventually classrooms return to being out of ratio. The Complainant reported dropping their child off on 03/25/2022 in the Farm room and that there were 7 children already present, with the complainant's child being the 8th. The complainant confirmed that there were children in the room under the age of 30 months. There was one child care staff member in the room. The program director, Ms. Kim DeZeeuw reported that either she or Ms. Kinkema normally arrive at 6:00 A.M. and the center opens at 6:30 A.M., which is when child care staff member Ms. Barbara Beech arrives. All of the children start the day in the two-year-old Farm room for breakfast. This room is licensed for 13 children under 30 months of age. As staff begin to arrive between 7:15 and 7:30 A.M., they take their group of children to their classroom. There are a maximum of 12 children at breakfast and by 7:30 or 7:45 A.M., the children are all in their own classrooms. Ms. DeZeeuw stated that they "try" to stay in ratio and did not know of any times where they did not meet the required child to staff member ratios. Ms. DeZeeuw reported that, in the morning, if they reach 12 children in the breakfast room, she will take the four youngest children into the infant room. During the onsite inspection, I observed eight children present in the Farm room with two child care staff members. Ms. Kinkema denied that the center is ever out of ratio. Either she or Ms. DeZeeuw arrives at 6:00 A.M, a second staff member arrives at 6:45 and a third staff member at 7:00. The center opens at 6:30 A.M. and there were two child care staff members present this morning at 6:30. Ms. Kinkema reported that attendance is down this week because of spring break. When the third staff member arrives, one staff member takes the older children to a different room. There are usually around 10 children present in the breakfast room in the morning. If any infants are present, then they only allow eight children in the breakfast room. Ms. Kinkema reported that she is "very vigilant about ratio." The children are only in the breakfast room for five to ten minutes and there are always at least two child care staff members present. Ms. Beech reported that she arrives in the morning 7:00 A.M. and denied that the center is ever over the required staff member to child ratio. Ms. Carrie Hocquard reported that she arrived the morning of the onsite inspection at 7:30 A.M. and that there were approximately six staff members present at that time. 3 Ms. Kathy Zegoski was present on the morning of the onsite inspection at 7:00 A.M. and started working at 7:30. She reported that she thinks there were two staff members and two children present at 7:00. When I arrived for my onsite inspection, there were eight children between the ages of 12 and 30 months present in the Farm/breakfast room with two child care staff members. Both staff members, Ms. Rachel Oliver and Ms. Rachel Stirrett reported that they arrived that morning at 7:30 A.M. In addition to interviewing the center staff, I also reviewed attendance records for both children and staff for multiple days. During our onsite inspection on 10/05/2020, Child B was sleeping in the swing when we walked into the child care room. I asked the child care staff members if Child B had a letter from a licensed physician stating he had a medical condition and needed to sleep in the swing.
