R 400.1924 · R 400.1924 Hand washing (1) All personnel shall wash their hands appropriately and in the following manner: (b) After all of the following: (i) Diapering. (ii) Using the toilet or helping a child use the toilet. (iii) Handling bodily fluids, such as mucus, blood, vomit, from sneezing, wiping, and blowing noses, from mouths, or from sores. (iv) Handling animals and pets. (v) Cleaning or handling garbage
The Complainant stated that while at the family home, The Complainant used the bathroom and there was no soap, there was only a half bottle of hand sanitizer. The Complainant did not tell Ms. Nixon there was no soap in the bathroom. The Complainant also stated there was not toilet paper in the bathroom. On 06/02/2023, I completed an unannounced on-site inspection at the family home. I interviewed Ms. Nixon and CCSM 1. Ms. Nixon stated that the child care children play in the hand washing soap during toileting, so the soap was moved out of reach of the child care children if they aren't using the soap. Ms. Nixon said when the toilet paper roll is empty, the toilet paper is replaced when someone needs to use the bathroom. I interviewed Child Care Staff Member (CCSM) 1. She also stated that the child care children sometimes play in the soap, so the soap is placed away from the children until they need to wash their hands. Ms. Nixon and CCSM 1 both stated the bathroom is cleaned up after the children when needed. During my on-site inspection I viewed the bathroom. The handwashing soap was sitting high on a shelf. I found the bathroom to be neat and clean, it was not dirty and there was toilet paper on the toilet paper roll. On 07/17/2023 I attempted phone calls to CCSM 2 and CCSM 3. CCSM 3 recently moved away but Ms. Nixon assured me she would be a good source to contact. As of 07/28/2023 I have not been able to get in contact with CCSM 2 and CCSM 3. On 07/17/2023 I attempted phone calls to Child A's Mother, Child B's Mother and Child C's Mother. As of 07/28/2023 I have not been able to get in contact with Child A's Mother, Child B's Mother and Child C's Mother.
The bathroom that the children use is very dirty. The bathroom did not have soap or toilet paper. The home is dirty. The kitchen floor is never swept, and always has food debris on it and there's no sanitation at meal time. The home is under construction, there is a gate to keep the child care children out of the construction area but the older children know how to open the gate and could be exposed to construction materials and equipment.

The Complainant stated while in Ms. Nixon's home, the Complainant observed dried rice, price tags, and dirt on the floor. The Complainant stated every time they are at Ms. Nixon's home the home is dirty. I interviewed Ms. Nixon. She stated that the children can be messy but it's never "dirty." During my on-site inspection, I viewed the home which did not look dirty, the child care area was filled with toys and the children seemed to be happy. I also viewed the kitchen which was being prepared for lunch time. The kitchen was viewed to be clean, and the table was set for lunch. The kitchen floor was also free of any food debris. I interviewed CCSM 1, and she stated the child care can be messy with lots of toys but it's never dirty. CCSM 1 also stated that they've had an unhappy parent and when the parent picks up, she always asks for Ms. Nixon. When CCSM 1 tells the parent Ms. Nixon isn't available, the parent begins wandering around the house in search for Ms. Nixon.

On 06/01/2023 1 received a phone call from the Complainant. The Complainant said Child A (female, age 5 years old) opens the gate that creates a barrier from the construction area along with Minor Household Member 1. The Complainant has not visually witnessed Child A open the gate, Child A told the Complainant she was able to open the gate. On 06/02/2023, I completed an unannounced on-site inspection at the family home. interviewed Ms. Nixon and CCSM 1. I went over the allegations with Ms. Nixon. Ms. Nixon stated that there is a gate which creates a boundary from the kitchen to the new construction area to keep the child care children out. Ms. Nixon stated Minor Household Member 1 is the only one who can open the gate. Ms. Nixon assured me there were never any construction materials or equipment around the child care children and the gate is up at all times. During my on-site inspection, I viewed the gate which separates the child care area and the room that was under construction. The room was empty and did not contain any construction materials or equipment at the time of my on-site inspection. No hazardous materials were observed. On 06/07/2023 I received an email from Ms. Nixon with a picture of the newly added door to the room. No hazardous materials were observed in the picture. I interviewed CCSM 1. CCSM 1 also stated that the only child who can open the gate is Minor Household Member 1, who is at school during the day. If the child care children are in the kitchen the gate is moved so the children cannot access the new addition.
