I made an unscheduled onsite inspection at the Clippers Before and After School Program on 9/13/2023 and spoke with licensee designee Samantha Ball and program director Debi Veld. There were only three days of school during the first week of the child care program. Debi Veld showed me the attendance record for 8/22/2023 through 8/24/2023. Child A attended 8/22/2023 and was signed out by his grandmother at 4:39 p.m. Child A was signed out by his mother on 8/23/2023 at 5:50 p.m. There was an early release from school on 8/24/2023 due to excessive heat and there was no after school care that day. Debi Veld informed me that she did not reach out to Child A and Child B’s Mother because Child A was in attendance both days they were in operation that week. I spoke on the telephone with Child A and Child B’s Mother on 10/17/2023. She did not dispute that Child A attended child care the first week of school. I spoke on the telephone with Child C and Child D’s Mother, Child E and Child F’s Mother and Child G and Child H’s Mother on 10/17/2023. None of the parents interviewed expressed any concerns with the care provided by Clippers Before and After School Program. Child C and Child D’s Mother described the program as fantastic. Child E and Child F’s Mother and Child G and Child H’s Mother stated that their children love attending the child care program. 3
Child Care Staff did not follow their parent notification policy because Child A and Child B’s Mother was never notified that Child A did not attend the after school program during the first week of school. Child Care Staff did not follow their parent notification policy on 8/29/2023 because child care staff did not notify Child A and Child B’s Mother when Child A was missing for over 10 minutes. Child Care Staff did not follow their parent notification policy when Child A was punched by another child on two occasions and Child A and Child B’s Mother was never notified of these incidents. Appropriate care and supervision was not provided to Child B on 9/17/2023 as she was released into the parking lot without adult supervision. An operable telephone is not available or accessible during child care hours.

Martin Public Schools is a small district housing the elementary, middle and high school on the same campus. Child A is in 6th grade and is the only middle schooler attending their child care program. Middle school students are released at 3:00 p.m. and then Child A walks over to the elementary school for child care. On 8/29/2023, staff noticed that Child A was not present at the center while taking attendance. While staff were looking for Child A over at the middle school, Child A and Child B’s Mother called the elementary school office to notify them that Child A went to a friend’s house after school. Debi Veld thinks Child A’s Mother called the office at approximately 3:10 p.m. I reviewed the parent handbook. The handbook states that a parent is notified immediately if his/her child does not report to the program as scheduled. Since Child A walks over from another building, Debi Veld went looking for him prior to contacting Child A and Child B’s Mother. After this incident, it was determined that middle school staff will walk Child A over to the child care program after school. It should be noted that the handbook also states it is the parent’s responsibility to notify the Program Director that their child will not be in attendance. Child A and Child B’s Mother did not notify the program director of Child A’s absence until after the program started and child care staff went to search for him. Child A and Child B’s Mother informed me that she called the office on 8/29/2023 at 3:18 p.m. to notify them that Child A would not be attending the program that day. That is when she found out staff were looking for him. None of the other parents had any concerns regarding the child care program. 4

According to Debi Veld, Child A arrived to the program upset about being escorted by middle school staff to the after school program on 8/31/2023. He threw his backpack and then took something away from his sister. Another child in the program witnessed this interaction and pushed Child A. The child pushed Child A again after Child A threw a ball in his face. Child A refused to talk with child care staff and threatened to run if they came any closer to him. Debi Veld contacted licensee designee Samantha Ball, the school principal Becca Lynema and the school reinforcement officer Deputy Kline for assistance. Child A tried opening the gate and walked over to the baseball field. Deputy Kline went after Child A while Debi Veld contacted Child A’s Mother to come pick Child A up. Child A’s Mother refused to come get Child A and hung up on Debi Veld. Deputy Kline obtained permission from Child A’s Mother to transport Child A to her place of employment. An email was sent to Child A’s Mother after the incident terminating Child A from the program. Child A was never injured from being pushed by another child. I reviewed the parent handbook. The handbook stated that if a child becomes injured while at the Clippers Before & After School Program site, the child will be given appropriate first aid and an accident report will be completed. 5 Child A and Child B’s Mother told me that she received an incident report following the events that took place on 8/31/2023. The incident report listed Child A’s actions that day but did not mention what the other child did to him. None of the other parents had any concerns regarding the child care program.

Samantha Ball and Debi Veld informed me that parents are required to come to the door to pick up their child from the after school program. Child A and Child B’s Mother would repeatedly call child care staff requesting that they bring Child A and Child B out to the car. Her car was parked in the bus loop, right at the curb. Child care staff always escorted the children outside. Child B was never sent out of the building unattended. Child A and Child B’s Mother told me that Child B left the building unattended on more than one occasion and child care staff would come out minutes later. None of the other parents had any concerns about care and supervision provided when they would drop off or pick their child up from care. 6

When I arrived at the child care center, I noticed that there was a posting on the door with two telephone numbers. Debi Veld and Samantha Ball informed me that the center is temporarily using a cell jammer that forward calls to either Debit Veld or Child Care Staff Member 1’s (CCSM1) telephone. CCSM1 always has her phone with her and is good about responding. If she does not answer, it rings directly into the library which is the primary child care location. The phone jammer was not working properly during the first few days of school so the parents were provided the phone number for the library as well. Both of these numbers are posted at the entrance. The child care is getting a new doorbell alert system on 10/2/2023. According to Child A and Child B’s Mother, the child care had two phone lines that were not answered the majority of the time by child care staff. She was able to communicate with staff on 8/29/2023 by calling the elementary school office. Child C and Child D’s Mother informed me that there was an issue with the phone lines when the child care first opened, however, she was able to get in touch with staff by calling the school office. The issues with the phone lines have since been resolved. Child E and Child F’s Mother recalled the program having two separate phone lines prior to the program’s new telephone system. Child G and Child H’s Mother has only had to call the program on one occasion and was able to connect with staff with no issue. 7
