On 02/26/2026, a child was sick in the gym. Teacher 2 stepped out to attend to the sick child and Teacher 1 supervised two groups of children. Teacher 1 reported moving around the gym to supervise children, and separated two children who had a brief conflict. Teacher 4 reported being in the gym at this time and stated that children's needs were met appropriately at this time. On 02/26/2026 and 03/20/2026, Child A and Child B climbed the fence in the outdoor play space. In both incidents, teachers were close and retrieved the children quickly. The children were never unattended. On 03/23/2026, Child D's Mother reported having no concerns about supervision during gym time or outdoors. On 03/27/2026, Child D's Mother stated that she had no concerns about supervision in the gym or during outside time.

On 02/26/2026, there were two groups of around 16 preschool children in the gym with two teachers. Teacher 2 stepped out of the gym to assist a sick child. Teacher 1 supervised both groups of children for roughly five minutes while Teacher 2 helped the sick child and gathered cleaning supplies. Teacher 2 returned to the gym as soon as possible, which was a few minutes later. Teacher 4 was signed into the preschool classroom on attendance sheets, but it is undetermined if she was also in the gym at that time. On 03/23/2026, Child D's Mother stated that there are usually enough teachers to care for the children adequately. On 03/27/2026, Child E's Mother stated that the teachers are always compliant of ratio requirements. Attendance records confirmed there were enough teachers present on 02/26/2026 to be in compliance of ratio requirements.

Diapers are checked frequently and are changed promptly. The teachers are able to message for assistance if they need to leave the gym to change a soiled diaper. On 03/23/2026,, Child D's Mother stated that she has no concerns regarding children's diapers being changed in a timely manner. On 03/27/2026, Child E's Mother stated that children's diapers are always changed frequently.

On 03/23/2026, PA, Teacher 2, and Teacher 3 stated that Child A and Child B climbed over the fence in the outdoor play space due to the height of the snow on 02/26/2026. Teacher 2 and Teacher 3 were present and the children were never unattended. They were brought back over the fence right away. On 03/20/2026, PA and Teacher 3 stated that Child A walked over the fence and Teacher 3 followed her, telling her three times to stop. Child A was never unattended.

During interviews conducted on 03/02/2026 and 03/03/2026, Teacher 3, Teacher 4, and Teacher 5 stated that there were two teachers close to Child A when she fell. Child A was immediately comforted by teachers after the fall and was evaluated for injury. On 03/02/2026, Child B's Mother and Child C's Mother stated that they did not have any concern regarding appropriate care and supervision in the toddler classroom.

On 03/02/2026, Child B's Mother and Child C's Mother stated that they feel teachers in the toddler classrooms communicate with one another about children's needs.

On 01/12/2026, Child A and Child B were found by Program Staff 1 unattended in the outdoor play space. Teacher 1 and Teacher 2 miscounted children when coming indoors after outside time and did not know that Child A and Child B were missing. The estimated time Child A and Child B were unattended outdoors was one to four minutes.

I reviewed the center's handbook and confirmed the naptime policy. Teacher 1 stated that on 12/03/2025, Child A was picking her head up and fighting sleep, but fell asleep after a couple of minutes.

On March 3, 2025, I reviewed staff and children schedules, as well as attendance records. These records showed that on Friday, 2/28/2025 at 9:00 am, there were nine staff members present with 19 infant and toddler aged children and 22 preschool aged children. Staff and children attendance records showed that there were enough staff members in the building to accommodate the number of children within the required ratios. CCSM 1 and CCSM 2 both indicated children were shifted into different classrooms on 2/28/2025 to maintain ratio requirements for all age groups. On 3/12/2025, the PD indicated she was notified that Preschool 2 was over ratio by a CCSM on 2/28/2025 and took immediate action to shift children to maintain required ratio requirements.
