    Capacity; ratio and group size requirements (4) In each room or well-defined space, the maximum group size and ratio of child care staff members to children, including children related to a staff member or the licensee, must be as shown in Table 1: (c) Preschoolers, 3 years of age until 4 years of age, child care staff member to child ratio of 1 to 10, maximum group size of 30.

    On 05/19/2026, PA, Licensee, Program Staff 1, Teacher 2, and Teacher 4 stated that the three-year-old classroom was not over ratio on 05/13/2026. Teacher 5 and Teacher 6 stated that three-year-old children went to the two-year-old classroom to nap on 05/13/2026. Attendance records from 05/13/2026 indicated that four children moved from the three-year-old group into the two-year-old group to meet ratio requirements. The toddler group had twelve children with three teachers, there was not a need to move children from the toddler group to the two-year-old group during naptime. On 06/18/2026, additional documentation was sent from PA via email showing that two three-year-old children napped on 05/13/2026. Therefore, Teacher 1 was with the three-year-old group of ten children. Teacher 4 and Teacher 5 were with the two-year-old group of sixteen children. The program was in compliance of ratio requirements on 05/13/2026.

    On 10/24/2025, Teacher 3 stated multiple occasions where the classrooms operate over ratio and office staff do not always step in to assist with ratio compliance. Teacher 1 stated that infants are sometimes left in car seats for 15-20 minutes due to the amount of children present and not being able to meet the needs of infant and toddler-aged children. On 11/05/2025, Child D's Mother stated that there were multiple occasions when the infant classroom was over ratio. On 11/17/2025, I observed five infants with one caregiver in the infant classroom during an on-site inspection. During that inspection, Teacher 5 stated that the infant classroom was over ratio on 11/14/2025 for roughly 30-45 minutes and expressed difficulty in acquiring assistance from the office staff. Staff and child attendance records confirmed that the infant classroom was over ratio on the morning of 11/14/2025 and there is no record of if office staff stepped into the classroom to be in compliance with ratio requirements.

On 11/17/2025, I observed an infant, aged three or four months, sleeping in a crib with a blanket under his body. There was a pacifier on the child's chest that had a soft stuffed elephant attached to it.

    On the morning of 7/1/21, child care staff members left the playground on two occasions to change a child's diaper. This took about five minutes each time and could be considered a temporary absence. After the diaper change, the staff member rejoined the arouo outside.
