On 5/12/2022, an incident report was received regarding Child A leaving the classroom on Thursday, 5/5/2022. The site supervisor completed an internal report on Friday, 5/6/2022. There was additional internal documentation on Tuesday, 05/10/2022 and the report was telephoned to licensing on 5/10/2022 at 3:31 pm. The incident report stated: "At 11:03 a.m. the class entered the back door. No adult checked the back door to ensure it was latched. Child A did not want to come in from outside. Lisa put Child A on the classroom riser (steps) to help him calm. She and three other staff members then proceeded to help the other children at the cubbies and tables. Approximately 5 seconds after being brought in, Child A went back outdoors unnoticed. At 11:04 p.m. Lisa begins to count the children and realizes a child is missing. 5 seconds later Cassandra leaves out the classroom door. Seconds later, Lisa announces a child is missing. The substitute (Jennifer Erickson) was instructed to check the bathroom. Meanwhile Lisa goes to the backdoor and looks out the door window. Once Jen confirms no one is in the bathroom, Lisa then leaves out the back door and finds Child A near the sandbox/playhouse area." 2 A video recording of the classroom was provided to child care licensing. I watched the video and also received a summary report from the site supervisor, Myra Smeester. Below is the documentation provided by the center. Active supervision incident video summary and Director Response: The classroom returns to the class from outside. They enter the room at 11:03:26, no one double checks the door There are four staff present At 11:03:33 Child opened back door and went into the outside play area which is fenced in Staff were helping other children hang up coats and get ready to sit down At 11:04:24 Staff starts to count children and realizes one is missing At 11:04:29 one staff person leaves the room At 11:04:58 staff person goes outside to get child and bring them back in, then stands by the door to zone that area This is a classroom with all staff being new within the last 8 months. They have several children that show behaviors and this child has a hard time coming in from outside. This child has also started to run outside at home as well. None of the staff involved have been involved in an active supervision incident before. The supervisor has discussed and went over their Child Supervision Plan, activated door alarms that will go off when the door is opened to let staff know, and had all staff in that room review and sign off on their plan. We sent out a tips sheet regarding active supervision practices, and our standards of conduct for review. We have also added to our Daily, weekly, and monthly cleaning check list to check door bells, and ringers to ensure they are in working order daily. We have a plan to go over the video with classroom staff and have a discussion and review the CSP again to ensure that it is correct and they are utilizing all active supervision practices. On 05/13/2022, I spoke to Child A's Guardian by phone. Child A's Guardian had been properly notified about the incident by the child care center. There was a recent occurrence at home when Child A left the house, going out the door. She has obtained child handle locks for her doors. Child A's Guardian did not report this behavior to the child care center. She did not have concerns about how the center handled the incident and was relieved that it was a short time that he went to the sandbox on the fenced playground. On 05/16/2022 I conducted an on-site inspection. I interviewed staff members Lisa Salter, Cassie Popour, Christy Scott, Jennifer Erickson, and Myra Smeester. Ashley Bosanic and Tasha Howard are also staff members, but were out on the day of the incident. Ms. Smeester is an administrator at this site and is not counted in the ratio. 3 This classroom is typically staffed with four to five staff members to the 13 enrolled children. Child A was encouraged by a staff member to remain in a quiet area of the classroom because he did not want to come in from outdoor play. He was able to leave the area and was not required to be separated from the group. This action was used to promote self-control for the child.
