An incident report was received on 07/07/2023 reporting an injury of Child A (10 year old, male) on the playground. I conducted an unannounced on- site inspection and interviewed the licensee designee Sandra Ludwig and child care staff member 1 (CCSM 1). I explained to Sandra Ludwig the reason for my visit. She indicated that CCSM 1 and CCSM 2 were present on the playground. CCSM 1 stated her and CCSM 2 were outside getting set up for activities. A child was being dropped off, so she went to get the child. She was told by CCSM 2 to come back to the playground because Child A fell from the playground structure and may have broken his arm. Child A's Mother was called and she picked him up within 5 to 10 minutes to seek medical care. Child A reported to them he was trying to jump past two steps and fell. I spoke with Child A's Mother regarding the incident. Her son had a fracture in his arm as a result of jumping off the playground structure. He will have to wear a cast for 4 weeks. She reported no concerns regarding supervision. Her son has been 2 attending the program for several years. She is pleased with the program and the staff are great. I called and left a message for CCSM 2 requesting a returned phone call. At the time of this report, I have not received a returned phone call.

On 01/25/2023, I received a complaint and incident report, stating that an incident occurred between Child D (female, 5 years) and Child E (male, 5 years). The report stated that Child D and Child E were on their cots at rest time, watching a book on the screen. Both children had blankets and the room was dim but not dark. The children had their blankets over their heads, Child care staff member (CCSM) 1 told them to put the blankets down and walked over to them. They put the blankets down, stopped talking, and watched the screen. Later, it was discovered that Child D reported that Child E showed her his privates and asked her to show him her privates. Child care licensing consultant, Ms. Linda Strube, stated that she received the incident report from the center and that child care licensing consultant, Ms. Tarah Kline, also had an open investigation with the center for similar allegations. Ms. Kline reported that she currently has an investigation with the same center but a different classroom. The concerns related to her investigation are that two children went to the bathroom at the same time and showed each other their privates. She will be citing the center for supervision concerns. I completed an onsite inspection during rest time on 01/31/2023, at which point I interviewed CCSM1 and program director, Ms. Amy Brown. I observed children lying on individual cots with blankets. The cots were appropriately spaced throughout the room. CCSM1 reported that they keep the cots spaced two feet apart, but that the children often try to scoot them closer together. She reported that, on 02/15/2023, she observed Child D and Child E's cots close together. Child E was lying face down covered with a blanket, raising his butt in the air, making an inverted V shape with his body. CCSM1 told him to lie down and rest. She did not observe anything else out of the ordinary during this time. Child D's Mother picked up Child D during rest time. Child D's Mother then returned to the room and told CCSM1 that Child D had informed her that Child E was lying on his cot under his blanket, showing her his private parts and that he said, "It's your turn." CCSM1 denied that Child D showed Child E anything and stated that she did not know exactly what Child D had seen. CCSM1 reported that Child E touches his privates sometimes, and that Child E's Guardian has reported him engaging in the same behaviors at home. CCSM1 reported that, since this incident, they have moved Child E's cot away from the other 3 children and that they shadow him throughout the day. I observed Child E lying on a cot near the door, away from other children. CCSM1 reported that there were two staff members in the classroom at the time of the incident and that there are always two staff members in the classroom. CCSM1 reported that Child E recently had an incident on the bus (Dial-A-Ride, public busing service) after school of the same nature and with the same child. She denied she knew about that incident until she called Child E's Mother about the current incident. Ms. Brown reported that she has no firsthand knowledge of the incident because she was not in the room. Child E now has his own seat on the bus and that staff know to keep an eye on him. Child D's Mother reported that she picked up Child D after nap and when they got to the car, Child D reported that Child E sat on her cot, "humped" her leg, pulled down his pants, showed her his penis, and asked her to show her privates. She reported that the behavior stopped when CCSM1 came toward them. Child D reported that the staff were not looking at them when this happened. Child D's Mother reported that the week prior, Child D had pulled his pants down on Dial-A-Ride and showed other children his penis. According to her mother, Child D also has a history of inappropriate behaviors. Child D's Mother reported that she has concerns with the supervision the children are receiving due to the fact that these issues are continuing to happ

I completed an onsite inspection at the child care on 1/9/2023, during this time I spoke with child care staff members Amy Brown and Brenda Struble. Ms. Brown and Ms. Struble were getting the children ready to go outside at my arrival, so I interviewed the two of them out on the playground. Ms. Brown explained that when children arrive in the morning, they get off the bus or are dropped off by their parents to her outside. She then brings them in groups to Ms. Struble and Ms. Gibbs who are in the gym inside the school. Ms. Brown stated that she was not in the gym when the incident with Child A (male, age 4) taking Child B (male, age 4) into the bathroom to show him his private parts occurred. Ms. Brown became aware when Ms. Struble approached her letting her know that Child B had just come to her telling her that Child A took him in the bathroom and showed him his private parts. 2 Ms. Brown asked Child A what had happened with Child A. He did not really want to talk about it, but he did say he went in the bathroom and showed Child B his private parts. Supervision has been increased around Child A; an adult is to be in close proximity to him and listening to what he is saying during large group. Also, all children must ask to go to the bathroom while gathered in the gym now instead of being able to use it freely as they did previously. Ms. Struble explained that in the mornings herself and another child care staff member are stationed in the gym when the children enter. Children are typically allowed to use the bathroom that is in the gym at their own discretion. There are typically 16 child care children to two adults in this space. On 12/21/2022, Child B came up to her and said that Child A had showed him his private parts. She asked him some follow up questions about where and what they were doing. Child B explained that Child A told him to follow him in the bathroom and when they got in there, he showed him his privates. While I was on the playground talking with the child care staff members, another staff member did come out to help supervise the children; the ratio at this time was now 16 children to 3 child care staff members. While I began to interview Ms. Struble, Ms. Brown noticed that a child had left the playground area through the gate, and she had to retrieve him from around the corner. I observed the gym, the bathroom is a one stall bathroom located just behind a door in the gym, it is feasible to supervise this area while the children are gathering in the mornings. I called Child A's Mother, Child B's Mother and Child C's Mother on 2/1/2022. I left a message with Child A's Mother and have not received a call back at the writing of this report. Child B's Mother called back and left me a message stating she had no concerns with the child care. Child C's Mother stated that Child C (male, age 4) is a big Mama's boy, but he loves to go to school. She stated that he loves Ms. Amy and talks about her all the time. He typically takes the bus, so she does not observe much of the child care staff members supervising the children but when she has been at the school. She has not noted any concerns. 3
