On 12/13/2022, while at the center, Child A (5-year-old male), had a bowel movement accident. The center did not have any baby wipes and only had bleach wipes. When Ms. Korial went to use the bleach wipes on Child A's buttocks to clean him, Child A told her they were the wrong wipes and Ms. Korial said, "they are all we have, so I am using them." Child A was red in the buttocks area that evening. The next day, Ms. Adkins was told what happened. She took Child A into the room where the wipes were, and Child A showed her the bleach wipes Ms. Korial used on him the day before.

On 12/20/2022, I interviewed Child A's Mother via telephone. She stated on the evening of 12/13/2022, Child A told her that Ms. Korial used bleach wipes to clean him after he had a bowel movement accident at the center. Child A said the center did not have baby wipes and Ms. Korial told him that they only had bleach wipes to clean him. When Child A arrived home from the center, he had to urinate, but he could not. Also, his genital area was red. Child A returned to the center on 12/14/2022 but he was sent home early because he got into trouble. When Ms. Adkins called Child A's Mother to pick him up from the center, she asked Ms. Adkins about the brand of wipes that are used when cleaning the children after they have had an accident. Ms. Adkins took Child A into the classroom, and he showed her the Clorox disinfecting wipes that Ms. Korial used to clean him. Child A's mother took him to the doctor on 12/15/2022 because his genital area was red. The doctor did not see a rash or redness on his genital area. Child A's Mother informed Ms. Adkins and Ms. Crousore that she took Child A to the doctor because Child A's genital area was red after Ms. Korial used Clorox disinfecting wipes to clean him. She could not recall the date she spoke to them about Child A's doctor's visit. On 12/21/2022, I interviewed Ms. Chidsey at the center. She said she is a lead caregiver in Room 105. On 12/13/2022, Child A complained of a stomachache and then told her that he had a bowel movement accident. Ms. Korial took him to the 3 bathroom to help him get changed. Ms. Chidsey and Ms. Korial were the only two staff working in Room 105 at the time the incident occurred. On a different day, Ms. Chidsey was told by someone else that Ms. Korial used Clorox disinfecting wipes. Child A did not complain of pain or discomfort after he had been changed. Child A returned to the center on 12/14/2022 and was sent home early around 10:30 a.m. because he had been physically aggressive with three people. Child A's Mother did not inform her that Child A had been taken to the doctor or that he was in pain or discomfort resulting from the Clorox disinfecting wipes that were used to clean him. On 12/21/2022, I interviewed Ms. Korial at the center. She said she is a child care staff member in Room 105. Ms. Mati was present during the interview to assist Ms. Korial with translation. On 12/13/2022, Child A had a bowel movement accident during lunch. She helped Child A change his clothes. First, she wiped his buttocks with Clorox disinfecting wipes. Next, she wiped his buttocks with a warm wet paper towel and then a dry a paper towel. She did not realize she used Clorox disinfecting wipes until after she had already wiped his buttocks. She was moving so fast to get him clean because he was not standing still when she was cleaning him. She denied that Child A told her the wipes she was using were the wrong wipes and she denied saying, "They are all we have, so I am using them." She said she did not realize she used Clorox disinfecting wipes until after she had already wiped his buttocks, and on 12/14/2023 Ms. Crousore informed her that she was not to use Clorox disinfecting wipes on skin and to only use them to clean toys or furniture. She told Ms. Adkins and Ms. Crousore that she used the Clorox disinfecting wipes once they asked her if she had used them to clean Child A. During the interview, Ms. Korial apologized for using the Clorox disinfecting wipes. She accidentally used the Clorox disinfecting wipes. Staff usually use baby wipes to clean children after they have had an accident. There were no baby wipes in the bathroom at the time she cleaned Child A. Ms. Korial stated that Ms. Adkins called Child A's Mother to pick him up because he had diarrhea. Child A returned to the center the next day but had to be picked up early because he was physical with another child.
