On 11/04/2021, 1 made contact with Ms. Samantha Ford and Ms. Caitlin Carson to discuss the self-reported complaint against the child care facility. Ms. Ford shared on 11/03/2021, three different parents notified the office that there were children touching each other inappropriately during nap time. Ms. Carson reviewed the cameras and found the information to be true. She shared in the video footage Child A appeared to have shown Child B ■ then Child B reached over and touched Child In the footage Child A reaches back over and touches Child B but it is not sAee'sn"were Child B was touched. Child C and Child B had interaction in the corner of the room, and the footage shows Child A on top of Child C, but there is not a full view because the small space is in the corner of the room and was out of the camera view. Ms. Ford confirmed Child Care Staff Members (CCSM), Rebecca Holmes and Jessica Harrah were working in the classroom during this timeframe and both staff members were observed on their phones when she played back the video footage. We set up an onsite visit on 11/08/2021, and I asked that the footage be prepared for viewing. On 11/04/2021, I made phone contact with Child B's parent. She confirmed Child B's date of birth and age as' years old. She shared on 11/03/2021 she picked Child B up from care. Shortly after getting in the car during this drive home, Child B shared thaIn and Child A touched each other's private parts. She asked Child B when this appened and where. She was told it happened that same day during nap time. shared "Ms. Becca" was there and let them do it. I confirmed Ms. Becca was s. Rebecca Holmes. Child B's mother shared she immediately called the child care facility and brought it to their attention. She shared the way the facility handed it made her feel comfortable. They were responsive and called her back several times after gaining additional information and watching video footage. I asked if Child B has ever shared similar concerns regarding inappropriate touching or supervision. She denied Child B has ever shared concerns and shared Child B enjoys going to care and has never made a complaint. On 11/08/2021, I completed an onsite inspection. I spoke with Ms. Ford and Ms. Erica Tank. 3 Ms. Ford sat down with me to observe video footage from 11/03/2021. I observed approximately 20 minutes of the footage on site. Due to the number of incidents within that short time period I requested to have a copy of the footage for a more thorough viewing of the footage. Within the 20 minutes, I observed CCSM, Ms. Holmes on her cellphone throughout the entire time, many times never looking up. I observed children walking around the classroom, rolling on and off their cots, playing with stuffed animals. I observed two incidents where Child B touched what appeared to be Child which could be confirmed from the interview with Child B's mother whoA w'sa"to by Child B that this took place. A copy of the video footage was provided via a USB drive. Ms. Ford shared the two small blind spots were immediately removed. Ms. Ford shared Ms. Holmes and Ms. Harrah were immediately placed on a one- day suspension and placed on a probationary status as well as a requirement to complete additional hours of supervision training. Ms. Tank shared the complaint was immediately brought to her attention when the parents called to share the concerns. She watched the video footage and was able to confirm that the CCSM's in the room were on their phones. She said to her knowledge they were inputting information into the Bright Wheel app. Their personal phones were allowed to be used during this time due to the tablet for that classroom being broken. I asked if there was a cellphone usage policy and requested a copy of the policy. She shared there is a cell phone use policy which prohibits cellphone use in the classroom unless pre-approval is granted by the director.
