On September 4, 2025, Staff did not provide appropriate care and supervision when they left Child A unsupervised in the Toddler Room for approximately 5 minutes.

The primary teacher is responsible for direct care, verbal and physical interactions, primary responses to the child's physical and emotional needs, and continued interaction with the child's parents regarding the c hild's experiences.

On September 4, 2025, a primary care system had not been established in the Toddler room. No staff member had been assigned to Child A to ensure his safety and well-being.
