On January 8, 2€ 21 1 initiated my investigation by interviewing Child A's Father. Child A's Father stated that Child A has been attending child care at Ms. Kaffenberger's child care home from to present. Child A's Father _ expressed that from his knowledge Child A likes to attend and there have been no issues until recently. Child A's Father of his and his children their mother. Child A's Father stated that he for of the Child A's Mother is the primary conttahcte wirit=h s. rbUeraerwsiitnce she is the one that drops off and picks up the children at child care daily. Child A's Father explained that mid-October 2020 he received a • for Child A indicating that According to Child A's Father this incident occurred on ,and e was never notified by Ms. Kaffenberger. Child 's Father stated that Child A's Mother also never notified him that Child A got injured while at the child care home until he received the and inquired. Child A's Mother stated to him that Ms. Kaffenber er contacted er when the injury occurred, and she left work immediately to take to the Child A's Father stated to Ms. Kaffenberger that he unders ands he MnRo e primary contact person for child care but wishes moving forward to get notified because Child A's Mother does not notify him and feels that it is unprofessional for Ms. Kaffenberger to only notify one parent when it is Child A's Father does not believe that Ms. Kaffenberger follows licensing ~rues because he did receive notification of Child A's injury. On January 14, 2021 1 conducted an unannounced onsite inspection and arrived at 1:45 p.m. I interviewed Ms. Kaffenberger during the onsite inspection. Ms. Kaffenberger stated that Child A was playing with one of the plastic toy fire trucks and the ladder accideritlye hit head that caused a small gash. The incident occurred on 9:40 a.m. and Child A's Mother was notified at 9:50 a.m. and she imme is e y ork to come pick up to take to Child A's Mother contacted Ms. Kaffenberger an explained tha ild A require and that was doing well. Ms. Kaffenberger stated that she notified Child s Mother and immediately completed an incident report to ensure that Child A's Mother was given a copy . A copy was faxed to licensing to notify that a child ineeded after an incident occurred at the child care home. Ms. ffenber er exp acne at she knows that Child A's parents are in and she notified the primary contact person and assumed that Child A's Mother would communicate incidents. I was also able to observe Child A in the child care setting and, appeared happy and comfortable. On March 1, 2021 1 interviewed Child A's Mother. Child A's Mother stated that there was an incident the first week of of 2020 where Child A has playing with a toy truck and it accidently hit head causing a cut that required Child A's Mother stated that s e was immediately contacted by Ms. Kaffenberger and she left work to pick up to take to . While at the There were no issues or concerns and Child A was happy. s Mother explained that Ms. Kaffenberger has always been very good about communicating to her and the other parents about anything regarding the children. Child A's Mother further expressed that she and her children love Ms. Kaffenberger and have no concerns about parent notifications. On March 1, 2021 1 interviewed Child B's Mother. Child B's Mother stated that her children have been attending Ms. Kaffenberger's child care home for■ years and very happy with the care provided. Her children are very happy and comfortable, and Ms. Kaffenberger has always provided proper and timely notification to her parents regarding anything about the children. On March 1, 2021 1 interviewed Child C's Father. Child C's Father stated that they have never had any incidents or a situation where their child was sick while at child care. Child C's Father stated that his child has and Ms. Kaffenberger contacts them regularly to inform them of flare ups and updates. Ms. Kaffenberger calls and texts parents all the time and h

On January 8, 2021 I initiated my investigation by interviewing Child A's Father. Child A's Father expressed that he was never notified when there was a COVID-19 exposure at the child care home when another parent tested positive mid-October 2020. On January 14, 2021 I conducted an unannounced onsite inspection and arrived at 1:45 p.m. I interviewed Ms. Kaffenberger during the onsite inspection. Ms. Kaffenberger also stated that she follows her COVID-19 safety protocol and notified the parents when there was an exposure with a child care parent testing positive. This occurred in the middle of 2020 and she notified all the child care families, as well as followed all recommendations made by the Macomb County Health Department and Licensing. I was able to review and obtain copies of the incident report for Child A's accident dated and a co y of the letter sent to the child care families of the COVID-19 exposure dated , 2020. On March 1, 2021 I interviewed Child A's Mother. Child A's Mother stated that Ms. Kaffenberger has been following her COVID-19 policy and has notified the parents when there were COVID-19 exposures. Ms. Kaffenberger informed all the parents by both phone and letter to notify them of the COVID-19 exposure. On March 1, 2021 I interviewed Child B's Mother. In _ of 2020 there was a COVID-19 situation where one of the child care parents tested positive and Ms. Kaffenberger notified all the families immediately and followed the recommendations from the Health Department. Child B's Mother stated that she has no concerns and knows that Ms. Kaffenberger communicates properly with her and all the parents. On March 1, 2021 I interviewed Child C's Father. Child C's Father expressed no concerns regarding COVID-19 exposures-- Ms. Kaffenberger immediately notified parents when there was a child care parent that tested positive in 2020. On March 1, 2021 I interviewed Child D's Mother. Child D's Mother stated that Ms. Kaffenberger has properly notified the child care families when one of the parents tested 5 positive for COVID-19 in _ of 2020 and followed the health department's recommendations.
