R 400.8266(2) · R 400.8266(2) Incident, accident, injury, illness, death, fire reporting (2) A licensee, licensee designee, or program administrator shall report to the child's parent on the same day of the incident and to the department within 24 hours, directly or via phone, fax, or email, if a child is lost or left unsupervised.

The same day of the incident, Dr. Katie Dodge, Licensee Designee reported to Child A's Mother and the department that Child A was left unsupervised. Child A 's Mother was notified in person, and the department was notified by an incident report being submitted via Child Care Hub Information Records Portal (CCHIRP).

Based on interviews with Lead Teacher 1, Teacher 1 and Teacher 2, on 10/17/2025, program staff did not provide appropriate supervision resulting in Child A being unaccounted for five minutes and seven seconds.
