On 02/16/2021, I interviewed Complainant who was visiting the facility but cannot recall the exact date. Complainant was concerned when staff were not wearing any mask and there looked to be "way too many kids." According to Complainant the director stated, "Staff are relaxed about wearing mask." Complainant did not know the ages of the children or the exact number present. There was one staff person in each classroom and possibly two in another. On 02/19/2021, I conducted an unannounced investigation at the center. The facility has four approved child care rooms. There were thirty-two children present with four child care staff members (CCSM). The breakdown of the children was as follows: Ten children were in Room 103 (3 to 4 years of age). Room 115 had three children (3 to 3'/2 years of age). Room 116 had eight children (3 '/2 to 4 1/2 years of age) and eleven children were in 113 (ages 4 and up). The staff were engaging the 2 children during the onsite inspection. The licensee Ann Bode was not present, but she immediately called me once returning to the facility. According to Ms. Bode, the facility always provide appropriate care and never has too many children. If anything, the daily attendance is currently low due to the current Covid-19 pandemic but she always has sufficient staff on duty. Ms. Bode also stated, everyone required to wear a mask is wearing one. On one occasion a staff mask was off while a visitor was in the building, but she never told the person they were not required. I requested and reviewed staff, and children, attendance records from 02/08/2021 through 02/19/2021. The documents revealed, the facility averages approximately thirty-two children and four to five CCSM's on a daily basis.
