R 400.1919 · R 400.1919 Communicable disease; immunization; mental and physical health; physician attestation; tuberculosis. (1) An applicant shall obtain a written statement, signed by a licensed physician or his or her designee within 1 year before issuance of the initial license, that attests to the applicant's mental and physical health. The attestation must be renewed at the time of subsequent license renewals.

I received a current physical from Ms. Gault attesting to her mental and physical health. Based on her medical statement she should not be directly caring for children by herself. Ms. Gault has provided a corrective action plan indicating that during the next three weeks, she will ensure that she has an assistant caregiver with her during the morning shift and afternoon shift while care for children. She will be closing her child care home on May 14, 2021.
