There is no evidence to conclude Child A was on a toddler playscape, fell and busted her chin. I received a picture of the incident and a small scratch and redness was observed. CCSM 1 and CCSM 2 stated Child A’s Mother observed her fall as she tried to walk toward her. New Rule Number: R. 400. 8213(1)(a).

Based on interviews with staff member, neither CCSM 1 nor CCSM 2 provided appropriate supervision over Child A and Child B. Neither CCSM 1 nor CCSM 2 witnessed Child B biting Child A. We are not able to determine if the red mark on Child A's arm was caused from Child B biting Child A or if it was caused by CCSM 1.

On 4/9/2021, I conducted an unannounced inspection at the childcare center. I interviewed licensee/program director Jillian Toth. She confirmed the center had a positive COVID case in the toddler room. She stated she followed protocol and contacted the health department and her consultant Ms. Byrd. The toddler classroom was closed as they had to quarantine for 10 days. She stated they had another positive case at the center and the center was closing for 10 days for quarantining and cleansing. She was waiting for a few children to be picked up. I observed the center's policy on COVID and information of COVID symptoms posted in the center accessible to both staff and parents. Ms Toth informed parents of the positive COVID cases by telephone and in person when they came to pick up their children. On 4/19/2021, I received an email from Ms Toth stating the center will be reopening and she provided me with her reopening action plan. NOTE: This was an anonymous complaint therefore the complainant could not be contacted.
