On 07/14/2023 I completed an onsite inspection at Teresa Hart's group child care home. When I arrived, I advised Teresa Hart we had received a complaint and advised her of the allegations. She asked me to step into a bedroom with her and she closed the door behind her and began to cry. She reported she has been so overwhelmed with everything going on with her She reported she is going to remove him as a child care staff member (CCSM). Following the discussion in the bedroom, I asked Teresa Hart if she was open on 07/13/2023 and asked for her attendance records for 07/13/2023 and 07/14/2023. She advised me that she did not have attendance records for either day. She then printed out attendance record sheets and began writing times and dates on them. I reminded Teresa Hart that on 07/11/2023 she had reported to the Child Care Licensing Bureau that she would be closed from 07/11/23 through 07/14/2023. In her text message, Teresa Hart stated she was down state at the University of Michigan Hospital. She reported she would be closed that entire week and was hoping to return home Friday 07/14/2023. She reported she would notify the Bureau when she returns. Teresa Hart reported they returned home late at night on 07/12/2023. When asked how the parents knew to bring their children on 07/13/2023, she responded she sent them a message. After I requested to see the messages, she stated she forgot she called them instead. Teresa Hart reported she had 14 children in care on 07/13/2023. She reported that children are always supervised when they are outside. On 07/13/2023 she was outside with the older children and CCSM 1 was inside with the younger children. On 07/14/2023, I observed Teresa Hart present with 14 unrelated children, 6 related children, Child Care Staff Member 1 (CCSM 1), Child Care Assistant (CCA), and Child Care Staff Member 2 (CCSM 2). In the kitchen, I observed five children, two of which are related, CCSM1, and a CCA. In bedroom 1, I observed one child. In 3 bedroom 2, I observed one child In the porch, I observed five children, three of which are related. Outside, I observed CCSM 2 with six children, one is related. Teresa Hart reported CCSM 2 is a teacher, and she has fingerprint clearances. 07/14/2023, I interviewed Child A's Mother via telephone. She reported Child A was in care this week. When asked she reported Monday 07/10/2023 was the only day Teresa Hart was closed this week. She did not know how many children were in care on 07/13/2023. She has no concerns with supervision. Teresa Hart is always there and CCSM 2 is there most days at the end of the day as well. Child A's Mother reported parents typically get a group message from Teresa Hart regarding closures. I asked if she had the messages for the week of 07/10-07/14/2023 and if she would be willing to send me screen shots of the messages. Child A's Mother stated she would see if she had any. I interviewed Child Care Assistant's (CCA) Mother via telephone on 07/14/23. When I asked if her child was enrolled in care she responded no, CCA has been working at Teresa Hart's group child care as an assistant for the past two summers. Teresa Hart reported CCA was an assistant on 06/12/23 during her interim inspection. When I requested her file at that time, Teresa Hart said, she did not have a file because she was actually an enrolled child in care and gave me copies of her child information card and child in care statement. CCA's Mother was informed by Teresa Hart on 07/14/23 that she needs to get her fingerprints which they scheduled for the next week on 07/20/23. She also needs her TB verification which is scheduled for the week of 07/17/23-07/21/23. They got the medical clearance signed 07/14/2023. On 07/14/2023 I contacted Witness 1 via telephone to follow up on information I received from an anonymous caller on 07/13/23. When asked Witness 1 reported Teresa Hart was not closed 07/11/23-07/14/23. She was closed on 07/10/23 only. Teresa Hart intentionally reported

On 07/15/23 Teresa Hart encouraged parents to give misinformation to the Bureau when she messaged parents on 07/15/23 to advise them she told the Bureau she was closed 07/11 and 07/12 and told them they would be called and asked if she was open. She apologized for putting them in that situation. On 07/15/23 Witness 1 confirmed Teresa Hart was open 07/11/23-07/14/23 and reported to the Bureau that she would be closed because she was going to be "over her numbers". Child A's Mother, Child B, C, and D's Mother and Child L and K's Mother reported Teresa Hart was open 07/11/23-07/14/23.
