R 400.1911 · R 400.1911 Care; supervision; children. (1) A licensee shall ensure appropriate care and supervision of children at all times.
On 04/05/2021 the department received a complaint that Child A was bitten by Ms. Cook's dog while in care. Ms. Cook's original licensing study report dated 12/02/2020 addressed Ms. Cook's dogs stating, "There are two large mixed breed dogs in the home. The white dog will have limited, supervised contact with children in care. The black dog must be secured away from children in care at all times. The black dog displayed signs of aggresion and lunged at the licensing consultant when the consultant arrived at the home for the initial inspection". I contacted Child A's Mother on 04/05/2021. The incident occurred on 04/01/2021. She received a call from Ms. Cook around 8:45 am that day. Ms. Cook advised her 2 that Child A was bitten by her dog. Ms. Cook reported she was letting the white dog in from outside and Child A was standing right next to her at the door. Child A went to kiss the dog and the dog bit Child A. This was Child A's fourth day in care with Ms. Cook. Child A's Mother asked for pictures which Ms. Cook sent. Child A's Mother arranged for a family member to pick Child A up and take him to the hospital and she met them there. While at the hospital animal control came to talk to Child A's Mother. Child A's Mother was informed Ms. Cook's dog's shots were not up to date. The dog would have to be quarintined for two weeks and possibly tested for rabies. Child A's Mother was also informed Child A may have to receive treatments via injections since the dog's shots are not up to date. Child A was treated and released with a perscription of antibiotics in case of infection. Child A's Mother reported on 03/31/2021, the day prior to the incident Ms. Cook contacted her to tell her Child A was doing something and making her dog "skitish". Ms. Cook told her to have a talk with Child A about this. Ms. Cook told her the black dog was fine with Child A. When asked Child A's Mother stated the white dog was typically in a cage when she saw the dog. The dog always seemed "to be going spastic" while in the cage. The black dog was not in a cage anytime she saw it. Child A was enrolled on March 29th and the incident occurred on his fourth day in care. Child A's Mother sent me the picture Ms. Cook sent her along with additional pictures Child A's Mother took later in the evening on 04/01/2021, 04/02/2021, 04/03/2021, 04/04/2021, and 04/05/2021. The pictures showed two open cuts on the right eye lid below the eyebrow and a very small scratch under the eye. The cuts appeared to extend approximately half the width of Child A's eyelid. I completed an onsite inspection on 04/05/2021 at 1:17pm. When I arrived the two dogs were in the kitchen window pushing on the screen and barking at me as I approached the home. Ms. Cook was present. There were no children in care and Ms. Cook stated she was closed for the day. Ms. Cook reported Child A went to the hospital on the day of the incident. I asked if animal control had been there and she responded no, Deputy Allen from Alcona County Sheriff's department was the only one who had been there regarding the dog. Deputy Allen advised her that he needed to contact the Department of Health and Human Services and she needed to quarintine the dog away from "outside people". When asked Ms. Cook stated she attempted to call me to report the incident but did not leave a message. She also stated she mailed in a copy of the written incident report to the Gaylord office. I asked Ms. Cook to tell me what happened on the date of the incident. Ms. Cook stated "there's a back story" and reported that she always crates the white dog. Child A started care on Monday of that week and kept poking toys and stuff in the crate. On 03/31/2021 Ms. Cook contacted Child A's Mother and informed her about the issues between Child A and the white dog. Ms. Cook stated there have never been any issues with the black dog and Child A. Ms. Cook did not ensure potentially aggressive pets were separated from children at all times. Ms. Cook stated she spoke to Child A's Mother on 03/31/2021, the day before the incident regarding "issues between Child A and the white dog". In the original licensing study report it states the black dog was not allowed to be around children after the dog lunged at the licensing consultant during the onsite inspection. Child A's Mother and Child B and Child C's Mother stated the black dog had contact with the children in care on a regular basis.

Ms. Cook did not prevent Child A from being bit by her white dog on 04/01/2021 after reporting the white dog needed to be calmed down after being by Child A in the hall.

As of the date of this report I have not received the copy of the I incident report in the Gaylord office mail.
