Licensee designee Shawna Boyd did not report to the department that Child A was left unsupervised on the playground until after the onsite inspection on 11/20/2025. The teachers involved in the incident never reported it to LD, Shawna Boyd. An incident report was submitted by LD, Shawna Boyd on the 11/20/2025 once she was made aware of the situation. LD, Shawna Boyd was very cooperative during the investigation and ensured the department received accurate information as soon as she was made aware of the incident.

I completed an onsite inspection on 05/04/2023 at Alcona Tiny Tigers child care center. While on site I interviewed program director Ms. Desiree Renz and child care staff member (CCSM) Ms. Angela Werner. I obtained attendance records, child information cards, and staff records. I interviewed Ms. Werner. Ms. Werner reported on 04/13/2023 she was present with CCSM Ms. Brandi Schulze and 15 children in care. They were outside in the fenced in play area. The five boys involved in the incident were all in the corner on the far- right side of the play area. It was a really hot day and that was the only area with 2 shade, so the boys were sitting in the shade in that corner. Ms. Werner yelled "clean up time". Ms. Schulze was with a small group of children helping to pick up the bike track. Ms. Werner was at the shed putting stuff away as children brought it to her. While at the shed after two to five seconds, Child D came to her and reported that Child A, Child B and Child C "showed their penis". Ms. Werner asked Child D if he saw it and he responded "yes, it was disgusting". Ms. Werner went to the boys and told them that is not "proper procedure" to show their body parts to others. Child A responded, "why not" in a gruff tone and acted "stand-offish". Child D stated "its disgusting. I don't want to see that". Child B stuck his tongue out at Ms. Werner which is a typical response for him. Child E didn't say much. She asked if he was offended, and he shrugged. Child C just laughed which is fairly typical for him as well. Ms. Werner told the boys she would have to call their parents and they all said "no". They all went inside, and Mrs. Werner called Child A's parents first. She spoke to them and also had Child A tell them what happened. Child A's Mother said she would have a talk with him at home. Child C's Mother was called, and Child C explained what happened. Child C's Mother laughed and responded, 'boys will be boys". Child B was picked up from school by his uncle. Child B's Uncle was informed of what happened. He was embarrassed. Child B's Uncle asked Child B why he did that. Child B responded, "I don't know". Ms. Warner stated Child B is "a follower". Child B's Uncle advised Ms. Warner that he would talk to Child B about the behavior. Child E's Father was notified at the time of pick up. Child E's Father responded "oh great" then asked Child E if he was bothered by what happened. Child E did not respond. Child E's Father stated he would talk with Child E more at home. Child D's Mother picked Child D up and Ms. Warner told her what happened. Child D told his mother "It was disgusting mom". Ms. Warner reported the incident to licensee designee, Ms. Shawna Boyd. Ms. Boyd advised Ms. Warner to complete an incident report. When asked if anything like this had happened before Ms. Warner reported in the fall, they had some issues with Child B where he tried to pull down his pants when they played the "Gummy Bear song". They stopped playing that song after it happened a couple times. I interviewed program director Ms. Renz. She was not present on the date the incident occurred and had no first-hand knowledge of the incident. The group of children this year is more active. When asked if there had been anything like this previously in the year Ms. Renz responded, "not like this". Earlier in the year they had an issue with the "Gummy Bear song" where Child B would try to pull his pants down at one part of the song. They stopped playing that song after he tried to do that a couple of times at the beginning of the year. I reviewed the attendance records for 04/13/2023. There were 15 children present at the time of the incident with two child care staff members. I left a voicemail for Child D's Mother on 05/30/2023 requesting a return call. As of the date of this report I have not received a return call. 3 On 05/30/2023 I interviewed Child A's Mother via telephone. She reported she did not have concerns regarding supervision before the incide
