On 10/01/2025, upon arrival Licensee Darla Martin was upstairs greeting a parent while three children were downstairs in the basement alone. After coming downstairs to drop off the fourth child that had just arrived, Darla then went back upstairs to make sure the door was locked up leaving all four children downstairs alone.
