On 4/20/2023, l interviewed Child A's Mother with the Child A's Mother said that Child A was born with He doesn't = because of the n 4/12123, Child A's Mother was giving Child A his bath and noticed Wruisingg multiple places on his back. There was one large bruise extending across his back from one shoulder blade to the other. Child A's Mother contacted licensee, Ashli Everett, to ask what happened. Ms. Everett told Child A's Mother that Child A was placed in the highchair bectahuese" Whe wooas throwing fit. Child A's Mother took Child A to get a medical examination Child A's Mother showed the doctors pictures of said could have been the cause of the bruises. According to Child A's Mother, the doctors said the chair could have lined up with the bruises. Child A's Mother said that Child A loved being at the child care home. She started noticing issues happening after a diapering incident when Child A's Mother questioned Child A coming home in the same diaper that he was sent in. Child A's last day at the child care was 4/17/2023 and he came home with bruises on his forehead and by his eye. On 4120/2023. 1 made an unannounced visit to Ashli's Childcare with the Ms. Everett was alone with five children, so we set up Ranoer visit on W23 to interview Ms. Everett and Child Care Staff Member 1 (CCSM 1). On 4/26/23, I made an unannounced visit to Ashli's Childcare with the Ms. Everett said that on 4118/2023, Child A cameo the child Rcar"eomo e"anw=asverry fussy and had "a lot of small tantrums." On 4/19/2023, CCSM 1 was not present at the child care home, so Ms. Everett was by herself. Child A had a rough day with "bad tantrums", and "screaming hysterically" when she was changing other children's diapers around 10:00 AM. Ms. Everett had never seen Child A scream like he did. The change in behavior was quite extreme that week. Ms. Everett said that she was working closely with Child A's Mother to figure out what was going on. Child B broke the toilet around 11:00 A.M. so all the child care children had to use the upstairs toilet. Child A woke up from nap after 15 minutes, so Ms. Everett held him. She had to bail the water out of the toilet to try to fix it, so she placed Child A in the highchair at the table with toys for approximately 10 minutes. Ms. Everett was coming into the kitchen area frequently to check on Child A, but he started crying when she left. She took him out of the highchair after 10 minutes and sat with him to calm him down. Once he was calmed down, Ms. Everett put Child A back in the highchair with toys for approximately five minutes so she could use the bathroom upstairs. When she went back downstairs, Child A was "hysterical" and arching his back against the back of the chair. Ms. Everett took him out of the chair immediately and held him until everyone woke up. After nap, all the children went outside and played, and Child A seemed fine. When Child A was picked up at the 3 end of the day, Ms. Everett told Child A's Mother that he had a very rough day. Child A wanted to be held and threw tantrums until Ms. Everett held him. Ms. Everett was on her way to work that evening when Child A's Mother called her and sent pictures of the bruises on Child A's back. Ms. Everett realized that it was from sitting in the highchair throwing his tantrum and arching his back. Child A returned the next day, 4/20/2023. Early On came in that day to work with Child A and he was playing outside. At 10:00 A.M., the children went back inside, and Child A was "screaming and screaming" as she was changing other children's diapers. Ms. Everett texted Child A's Mother to tell her that Child A seemed tired and wanted her to hold him all the time. Ms. Everett told Child A's Mother that if Child A doesn't eat his lunch, then she will have to come pick him up. Child A did not eat lunch, so Child A's Mother picked Child A up around 1:00 P.M. Ms. Everett suggested to Child A's Mother that something may be going on medically with Child A and suggest

I interviewed Child A's Mother. She said that on 3/22/2023 she received a text to pick Child A up at 2:00 P.M. Children's diapers are changed every 1 — 2 hours. Child A's diaper was checked at 8:30 A.M., 10:30 A.M, and 12:00 P.M. and it was not wet or soiled. CCSM 1 noticed Child A's diaper was wet when he woke up from his nap, but Child A's Mother was there to pick him up, so she handed Child A to Child A's Mother before he was changed. This is only one occasion Child A's diaper did not get changed.
