On 11/12/2020, I spoke with Ms. Beck, licensee designee. Ms. Beck stated there had been an incident at Hamilton-Parsons where the Child A's whereabout were unknown by child care staff members. Ms. Beck stated she is still gathering information from everyone that is involved and would send an incident report with more information along with statements from child care staff members. On 11/13/2020, I received a copy of an incident report and statements stating Child A's whereabouts were unknown from 1:49 p.m. until 2:19 p.m. Child A was found to be at home. On 11/16/2020, I completed an onsite investigation at Hamilton-Parsons. Interviewed Ms. Beck, Andrea Hasse, principal, Karen Gran, child care staff member, Patricia Lightcap, child care staff member, and Kailee Naif, child care staff member. Ms. Beck stated I would be able to view the school video from the principal's office. Ms. Beck did attempt to contact Child A's Mother but has not spoken with her at this point. She stated she already put some changes in place to prevent what occurred from happening again. Ms. Beck stated Child A left on the school bus that arrived early (1:49 p.m.). There was no communication between the bus driver and child care staff members of the early arrival. Ms. Beck stated it appears as Child A was putting her backpack away when she saw the school bus arrive, grabbed her backpack, walked out the door and boarded the bus. The school bus typically does not arrive for pick-up until 2:30 p.m.-2:45 p.m. and a child care staff member is with Child A until she boards the bus (Child A is the only child that is taken home by bus). Ms. Gran and Ms. Lightcap were assisting other children and did not observed Child A leaving. Ms. Naif was in the cafeteria with children that had not finish eating their snacks. Ms. Beck stated she has also reviewed video and is aware that Ms. Gran and Ms. Lightcap were assisting and supervising other children. They were not positioned where they could supervise children that were putting their backpacks and other items away. She has changed how child care staff members are supervising transitions and arrivals of students. She has also spoken with the director of transportation and was waiting for how they were going to address the scheduling with the bus driver, communication, and early arrivals. Ms. Hasse stated she would be able to bring up the video of Child A leaving the building on 11/12/2020. Ms. Hasse was contacted by Ms. Gran that Child A was 3 missing and could not be located. She had office staff used the PA system calling for Child A and had school available staff looking around the school as well. Ms. Hasse stated it was not until Ms. Gran contacted Child A's Mother to inform her, they could not locate Child A that Child A's Mother told Ms. Gran that Child A was home and had been dropped off by the bus. On 11/17/2020, I reviewed video from 11/12/2020. Video started at 1:46 p.m. (video is motion sensor) I observed the following: at 1:48 p.m. I observed Child A and Child B (10-year-old boy) last two children putting their backpack away. At 1:49 p.m. in the gym, the bus could be seen arriving, I observed Child A look towards the bus, grab her backpack and head out the door. Ms. Gran and Ms. Lightcap could not be seen in the video. There were no child care staff observed supervising Child A and Child B. I observed Ms. Gran returned to the building at 1:53 p.m. searching the gym for Child A. Ms. Gran stated she has been working for the center for approximately a year. She stated on 11/12/2020, she and Ms. Lightcap transitioned with a group of children from the cafeteria to the gym. Children began to place their backpacks in the gym and wash their hands to transition to go outside. Ms. Gran stated she started to assist children in the hallway by the restrooms and doors leading to playground while Ms. Lightcap began to lead children outside. Ms. Gran was with the last four children that exited the backdoor. She did not do
