On June 15, 2021, I interviewed Child A's mother. She stated that when she picked Child A up around 4:00pm on June 10, 2021, he was on the playground. A child care staff member (CCSM) brought him to her at the gate but he needed a diaper change, so she had the CCSM take him in to change his diaper. When he was brought out, she was near the corner of where the two fences meet engaging with another child through the fence. She said that the CCSM opened the gate and Child A ran to the parking lot. Child A proceeded to run all the way around the building and over to a nearby church before he returned. She had a hard time chasing him as she was wearing heels and she felt that the CCSMs who were outside should have helped her catch Child A. Child A's mother was concerned as the center is on a busy road and Child A could have gotten to the road. She did not know the names of the CCSMs involved. When Child A was in the parking lot, she thought another parent was either arriving or departing and may have witnessed this. On June 18, 2021, I made an unannounced onsite inspection to the child care center. I spoke with program director Ashley Hill. She was in a meeting at the center and did not see Child A initially get away from his mother but did see her standing outside at the fence. She said that she came in to sign Child A out and then waited at the fence for Child A to be brought back out after his diaper change. She said the procedure is generally that a parent go in to the office and sign a child out and then step outside to the fence and a CCSM will open the gate and hand the parents the child. She states that not many children are picked up during the time that children are outside playing as it is still earlier in the day. She was walking near the infant room when she saw Child A run by. She ran out the back door and proceeded to chase him. She saw both Child A and his mother on top of the hill just outside of the playground area. Child A then ran down to an area near the building and proceeded to grab a garden hose and spray it into the playground. Ms. Hill was able to grab him and lift him over the fence, so he was back in the fence's playground area. CCSM Erika grabbed Child A from inside the playground and was able to walk him back to his mother. After the incident, she along with two of the other CCSMs wrote up what had happened. Ms. Hill provided me with the originals of these documents. She also provided me with contact information as to the person that had toured the facility and may have been the person Child A's mother recalled seeing in the parking lot. While at the center, I interviewed CCSM Asher Lafferty-McGill. She stated that she was present on the playground when the incident with Child A happened on June 10, 2021, around 4:00pm. Child A was pouring out bubbles and acting out when his mother arrived, and she seemed irritated at this. When she tried to give Child A to his mother at the playground gate, his mother asked that his diaper be changed so she took him in the building and changed his diaper and brought him back out through the fenced playground area. She believed that during this time, his mother had gone in and signed him out. She said that Child A's mother was on the other side of the fence playing with children through the fence when she opened the gate. She had Child A by the hand. 3 She did not let go of Child A's hand until his mother had taken his other hand. She said that Child A stood there for a minute and then he pulled away from his mother and ran to the parking lot. After that he ran up the hill and went behind the building. She estimated that Child A was chased around by his mom for almost 5 minutes. She did not see Child A get caught as she was dealing with children fighting in the toddler area of the playground, but she believed it was Ms. Hill who grabbed him and gave him to his mother.
