On June 2, 2023, I made an announced onsite inspection to the child care center. Program director Dawn Hardy was present. She denied not being present at least 50% of the time that children are in care and said that she usually works 9am until 3pm or sometimes 6am until 1p m. Ms. Hardy showed me her monthly attendance. It appears that Ms. Hardy is present for approximately six hours per day most days. She confirmed that previous program director Megan Rau was still listed as an additional program director as she still works in this capacity on occasion, as a back up program director. When I spoke to the complainant, she told me that she thought that Ms. Hardy just popped in for an hour or so a few times per week but has not witnessed this first-hand. On June 12, 2023, I made a second unannounced onsite inspection. Megan Rau was present and acting as program director. She told me that she covers for Ms. Hardy to ensure that a program director was present at the center at least 50% of the time children are in care. Child Care Staff Member 1 (CCSM1) informed me that Ms. Hardy is usually at the center two or three days per week. Child A's mother says that she usually sees Ms. Hardy there when she drops off or when she picks up daily. She estimated that she sees Ms. Hardy at the center approximately four days per week. Child F's mother said that Ms. Hardy is there at least half of the time she is dropping off or picking up. Child F's mother said that she usually sees Ms. Hardy there at either drop off or pick up. On July 11. 2023, I made third unannounced onsite inspection. Ms. Hardy was not initially present at the center. CCSM3 informed me that she had just left prior to my arrival. Ms. Hardy arrived within ten minutes of my arrival and stated that she had been out for a few minutes.

During the June 2, 2023, onsite inspection, there were eight children in the infant/toddler area. This area is currently licensed for up to 12 children. There were 16 children in the preschool/school-age area. This area is currently licensed for up to 22 children over the age of 2 years. 1/2 During the June 12, 2023, onsite inspection, there were five children in the infant/toddler room and 13 in the preschool area. During the July 11, 2023, onsite inspection, there were five children present in the infant/toddler room, and a total of 22 children in the preschool/school-age area. I reviewed children's attendance records for five days in June. The center did not have more than the licensed capacity of 41 children at any one time. Child A's mother said that she usually sees 8-12 children present when she drops off and picks up Child A. She did not have any concerns regarding the center operating over its licensed capacity. Program directors Dawn Hardy and Megan Rau both denied that the center has ever operated over the licensed capacity of 41 children. When I spoke to the complainant, she said that while she believed that there have been more than 12 children in the infant/toddler area, she did not believe that the center ever had more than 41 children total present at one time. Child F's mother had no concerns about the center's capacity when dropping off or picking up Child F.

At the time of the June 2, 2023, onsite inspection, two of the children present in the infant/toddler area had not been signed in. At the time of the June 12, 2023, onsite inspection, I observed that approximately 10 school-age children had not been signed out when they went to school that morning.

She did not believe that caregivers were training in safe sleep practices prior to caring for children. On June 2, 2023, I conducted an unannounced onsite inspection to the child care center. At the time of the inspection, I observed Child B (female, age 11 months) sleeping with a blanket in her crib. CCSM1 told me that she had previously had safe sleep training. CCSM2 could not recall if she had safe sleep training. During the June 12, 2023, onsite inspection, CCSM3 confirmed that she had not yet had safe sleep training. On July 6, 2023, Dawn Hardy provided me with verification that all staff had completed safe sleep training.

On July 29, 2022, I made an unannounced onsite inspection to the child care center. Program directors Megan Rau and Dawn West were not present. Child care staff member 1 (CCSM1) told me that Ms. Rau has not been present at the center daily, but that Dawn West usually works Monday through Friday. She comes in between 8am and 9am and is there most of the day. CCSM2 and CCSM3 also told me that Dawn West is there daily and that this is the only day when she hadn't been present since either of them have been working at the center, which was a few weeks for both of them. While at the center, I reviewed caregiver attendance records. It appears that Megan Rau had been present at the center for two days in July. There was no attendance record for Dawn West, however. At the time of the second unannounced onsite inspection on August 12, 2022, Neither Megan Rau nor Dawn West were present. A third unannounced onsite inspection was conducted on August 23, 2022. Megan Rau was not present, but Dawn West was present. Ms. West said that since Ms. Rau hasn't been able to work full time at the center, she has been there daily for at least 7 hours 1/2 per day. I attempted to reach the parents of 20 children currently or previously enrolled at the center. Child N's mother said that she sees Dawn West at the center on a regular basis. Child R's mother recalled seeing Dawn West present approximately 2 to 3 times since the beginning of June. Child G's mother denied seeing Ms. Rau at the center lately but that she sees Dawn West there most times at both drop off or pick up of her child. Child P's child hasn't attended since mid-June but before that, Megan Rau was always there. 4

CCSM1, CCSM2, CCSM3, and CCSM4 all denied ever placing a child in a crib for punishment and all of them denied ever seeing another caregiver doing this to a child. None of these four caregivers was working at the center in April 2022, however. When I interviewed CCSM 5, she identified Child T (male, age 4) as the child that this happened to. She said that this happened on more than one occasion and that it was CCSM 6 that did this. She did not see her do this, however. She said that her fellow co-workers at the time witnessed this. She believed that this also happened to Child U (female, age 5). The time frame that she believed that this occurred was between March and May of 2022, as CCSM5 hasn't worked at the center since May 2022. She said that CCSM 6 is a compulsive liar and will not admit to this, however. CCSM 6 confirmed that she is no longer employed at the center. She denied ever punishing a child by placing them in the darkened crib room. She said that she had witnessed Megan Rau doing this, however. No time frame was given regarding this. Megan Rau denied ever doing this and denied ever witnessing a caregiver doing this for punishment. I interviewed Child T and Child U's mother regarding this. She denied any knowledge of Child T being placed in a crib for punishment but that a former child care staff member informed her that Child U had been placed in a crib for punishment. She did not feel comfortable disclosing who this former child care staff member was that informed her of it, however. This person told her that it was CCSM 6 that did this. Child T and Child U's mother said that both children now attend another child care and that CCSM 6 now works there. Neither Child T nor Child U want to go into a classroom with CCSM 6, however, so she believes that something happened. CCSM 7 confirmed that she had previously worked at the center and was employed in May of 2022. She denied seeing anyone place a child in a crib in a dark room for punishment but that she had heard that it had happened between CCSM 6 and Child T.

On February 26, 2021, 1 interviewed the complainant. She reported Oeapproximately one month ago Child A told her that another child had when was sleeping. Child A did not identify the involved. complainant sal that she addressed this with Child A's mother, and she said that all of this had been taken care of. The complainant stated that she wanted to report it just in case but that she was not sure what to believe as Child A is a story teller and makes up a lot of things. On March 1, 2021 1 interviewed Child A's mother. She said that she had no concerns about supervision at the center and said that Child A tends to tell stories and that these stories chap e all of the time. Child A's mother said that Child A as well as Child B M, age would say something if something were to occur at the center. She ) 2 believes that Child A is talking about an incident that was previously investigated in 2020 (Special Investigation Report 2020D0521016) in which another child was disenrolled after saying inappropriate things to Child A. On March 3, 2021, I made an unannounced onsite inspection to the child care center. I interviewed child care staff member Taylor Henley, who stated that she is usually Child A's caregiver. She reported that there have been no incidents with Child A since the other child was disenrolled from the center last year (SIR 2020D0521016). She believes that Child A is always well supervised, but that Child A does like to tell stories. She denied that Child A or any other children in the center have made statements about inappropriate contact at the center. On March 3, 2021, I interviewed program director Megan Rau by telephone. She denied that there had been any incidents of inappropriate contact involving Child A and stated that Child A is well-supervised. No children have reported anything out of the ordinary regarding Child A at the center. Additional allegations regarding care and supervision at the center were previously investigated in SIR 2020D0521016 dated October 13, 2020. No new information was provided regarding these allegations.
