Upon arrival, I saw a preschool child running down the hallway away from his classroom towards toward exit doors. He was followed by a teacher who stopped him, and he was taken back to his classroom.

On January 20, 2023, I received an incident report from program director Windy Carroll. The report states on January 19, 2023, Child A (female 10 years old) was on the playground during after school programing and she was playing with a small group of children. Child B (male, 12 years old) decided to "flip" Child A as if he were doing cheerleading or gymnastics move. Child B was not strong enough and Child A fell to the ground landing on her side/ribs and the side of her face/head. Child Care Staff Member 1 went to where Child A was laying on the ground and began first aid. Child A was afraid and out of breath. Child Care Staff 2 Member 1 kept Child A laying on the ground in case there was a spinal injury and to further assess for a concussion. Child A's Mother was contacted, and she came and picked up Child A. On January 26, 2023, I spoke with Child A's Mother. Child A's Mother stated she received an incident report from Our Savior Lutheran School regarding her daughter being injured and it stated another child was doing a gymnastic flip which she thought was strange. The reason she thought this was strange is because Child A was injured in gymnastics doing a similar move and she would never allow a boy at school to "flip her." She called Witness 1 and he told her she needed to speak with Windy Carroll, the program director; but she has never heard of Program director Windy Carroll before. She got into contact with Child C (Female, 11 years old) who was on the playground when Child A's injury occurred, and Child C told her Child A was fighting with Child B and he "got rough with her and flipped her." Child C stated Child A hit her head on the ground and ended up with a concussion. On January 31, 2023, I completed an on-site inspection. I spoke with program director Windy Carroll. Ms. Carroll stated she was subbing in the after-school program the day Child A was injured as there was a child care staff member out ill; she was working with Child Care Staff Member 1. She and Child Care Staff Member 1 were outside for recess. There was a group of children on the climber, at the basketball hoops, in the gaga pit, and some that are "too cool for school" were just standing around and talking. She was standing near the basketball hoops with the children that were playing there; whichever child care staff member that stands in that area normally makes sure the ball does not go into the parking lot and that the children do not chase after a ball into the parking lot. Child Care Staff Member 1 was at the far back of the gaga pit facing toward her this way they could see everyone between them. Ms. Carroll stated she was not able to see a few children that were near the shed by the tree line which is the edge of the boundary that is as far as the children are allowed to go in that direction. She did not see or hear anything that happened with Child A and Child B. She saw Child Care Staff Member 1 as someone called his name and he hopped out of the gaga pit and ran over to the group of children by the shed. She heard Child Care Staff Member 1 yell her name and let her know he was going in that direction. Ms. Carroll stated she felt like she should not leave where she was so she held up her cell phone to let Child Care Staff Member 1 know he could call her if he needs her. She had the basketball group sit to the side and she moved to where she could see the children near the shed. Ms. Carroll stated another child came over and let her know Child A was crying, could not breathe, and she fell. She asked the other child if she knew what happened and she told her Child B tried to flip Child A. At that time, Child Care Staff Member 1 called her and let her know it appeared Child A had the wind knocked out of her and he was having Child A remain still on the ground until he could finish assessing her. On January 26, 2023, I received an incident report from program director Windy Carroll. I reviewed the report. It states Child A was playing and went to step onto a wooden box/chest when he slipped and fell, hitting his face on the box. Child Care Staff Member 2 was 10 feet away and ran over to assess, noticing Child A had knocked a tooth out. Child Care Staff Member 1 applied ice and paper towel while holding Child A to comfort him. Child A's Mother was called. On January 26, 2023, I spoke with Child A's Mother. She stated she picked Child A up, rushed him to the dentist, and he ended up losing a tooth along with the entire root. There is also a good possibility he could lose up to three more teeth due to this injury. Child A also injured the gum which is located near the top of the mouth between the two front teeth. Child A told her he was playing on a balance beam and fell on a treasure chest. The child care staff members told her they were doing a skit and that is why the balance beam and the treasure chest were so close together as they are normally on the opposite sides of the classroom. She is concerned as to how the classroom was arranged. On January 31, 2023, I completed an on-site inspection and I spoke with program director Windy Carroll. She stated she was not present in the morning on January 26, 2023, when Child A was injured. The lead caregiver is Child Care Staff Member 1 and she opens the classroom at 7:00 a.m. They get a "trickle" of children, but the majority come at 8:15 a.m. when the school day begins. Child Care Staff Member 2 was present; she walked her son next door to his classroom. This left Child Care Staff Member 1 in the classroom with approximately five to six children. The children were playing in the pretend play area as if they were stage performers and they were dressing up and singing. There is a little wooden chest in this area that has a flat top and contains colorful scarves. Child A reportedly tried to step up onto the box and she is not sure what happened, but his face hit the top of the box. Child Care Staff Member 1 ran over to Child A and his tooth had come out. Child Care Staff Member 2 returned to the classroom, and they got ice, paper towel, and had Child A hold the tooth. Child A's Mother was contacted immediately, and she turned right around and came and got him, and she took him to the dentist. I asked Ms. Carroll to email me the sign in sheets. On January 31, 2023, I received the sign in sheets from Ms. Carroll. I reviewed them and noted there were eight children present until 8:00 a.m. I requested the age of the youngest child from Ms. Carroll on March 9, 2023, and she said the age to be 3 years old. On January 31, 2023, I spoke with Child Care Staff Member 1. Child Care Staff Member 1 stated this is her sixth year working at Our Savior Lutheran School and 3 her first year in a lead caregiver role. On January 26, 2023, when Child A was injured in the classroom, she was walking sideways while watching Child A as she was headed to the front of the classroom. She saw Child A put his foot on the top of the box and his hands were on the sides of the box. It appeared Child A was going to attempt to pull himself on the box or climb/stand on the box. She said, "Child A what is your plan." And then she saw him fall. Child A hit his face on the box. It happened very fast, and she is not sure the exact area he hit his face. She ran to Child A, and she watched him fall to the floor and she watched his tooth come right out of his mouth and land on the floor. Child Care Staff Member 1 stated she got on the floor with Child A, hugged him, but did not want to move him until she could assess him. Once she assessed the situation, she moved Child A, got him a paper towel and ice, and at that point Child Care Staff Member 2 came into the classroom.
