On 4/6/22, Child A (age 23 months) had a large and significant bruise on his nose after being picked up from child care. The child care had no explanation for how the bruise occurred. The center's owners reviewed video footage from 4/6/22 and they did not observe any injuries occur, despite him not having a bruise prior to going to the child care. On 417122, Child A had a bite mark on his arm. The child care again had no explanation for the injury. It is believed that Child A would have cried because of both incidents. There are concerns that the child care is either lying about how the injuries were caused, or that the children are not being properly supervised. In addition, Ms. Dodd immediately "snatches" Child A's pacifier and will not let him say goodbye at drop off. 2 that completed an unannounced on-site inspection to the center. Interviews were conducted with r, Oicens*eeensgnee, and Ms. Robertson the center's assistant director. Child A attended the toddler room which serves up to 11 children ages 18 months to 2 '12 years. The child care staff members (CCSMs) in the toddler room include Ms. Ward, Ms. Dodd, and Ms. Gonzalez. The three CCSMs were also interviewed during the on-site inspection. Ms. Taylor and Ms. Robertson stated that there have been ongoing concerns with the relationship between Child A's Mother and Child A's Grandmother, saying there are times Child A's Mother only wants the CCSMs to speak directly to her. Child A's Mother has never expressed any concerns regarding Child A. They spoke to her on 4/6/22 about his nose being red, and she was not concerned. Child A's Mother told them that Child A falls sometimes and that she thought it looked like a rug burn. Child A's nose was only red, and no skin was broken. Despite Child A's Mother not being concerned, they received an email from Child A's Grandmother that evening saying that he had two scratches on his nose. After receiving the email, they spoke Ica with the three CCSMs in the room including Ms. Ward, Ms. Dodd and Ms. Gonzalez. None of them were aware of him getting injured while in care. None of them reported any concerns regarding the care or supervision of the children. Ms. Ward took a picture of Child A about approximately 11:00a.m. of him playing. In the picture, his nose was slightly red, but the skin did not appear broken. The picture was sent to his mother as part of their daily communication with the parents. I also observed this picture. Child A is seated in a cardboard box on the floor. The left side of his face and front of his nose are visible. There is possibly slight redness to his nose, but no other obvious sian of in Ms. i aytor ana Ms. s nose did not look like that while he was in care. Ms. Taylor and Ms. Robertson stated that on the evening of 4/7/22, they received a second email from Child A's Grandmother indicated that he had a bite mark on his arm. They both spoke with Ms. Ward and Ms. Dodd who were in the room that day. Child A wore long sleeves to the center on 4/7/22_ As such, the CCSMs would not have observed his arms directly. Ms. Ward and Ms. Dodd did not observe him get bit while in care, nor did Child A express any concerns. Neither CCSM reported any concerns regarding the care or supervision of the children. Ms. Taylor and Ms. Robertson indicated that biting can be normal for this age group. Biting can sometimes happen quickly and without any warning even when supervision is being provided. Ms. Taylor and Ms. Robertson stated that there are no children in the toddler room that are struggling with biting others. They spoke with Child A's Mother again on 418122; and she was not upset with the center, saying that she understood that sometimes things happen. Ms. Taylor and Ms. Robertson stated that they reviewed the video apes for the toddler room on both 4/6/22 and 4/7/22, and they did not observe Child A receive any injuries. While interviewing CCSM Ms. Ward, she indicated that if a child were to bite another child in the toddler room, pushed someone, was mean, or was not listening, that she would talk to the child, redirect the child, as well as rarely place the child "time out." Ms. Ward described the time out as her having a child sit down for 1-2 minutes with no other play or learning activities. There is no specific time out space in the room. If the child wanted to get up from sitting, she would tell the child that they must remain seating for the entire time out. After 1-2 minutes, she ends the time out by speaking to the child about what happened, and then the child is free to return to playing. I interviewed Ms. Dodd and Ms. Gonzalez, both stating that they do not use time out for toddlers. Both indicated that they use speaking and redirection to other activities for the children. A child may be asked to sit by them, but they are engaged in other activities I interviewed Ms. Taylor. She stated that no time out is to be used for children under 3 years of age. The toddler room CCSMs are supposed to use talking and redirection with the children. Ms. Taylor indicated that she would speak with the toddler room staff members to ensure that they are no longer using time out in the room. I interviewed Ms. Taylor a second time. She stated that all the CCCMs were trained regarding discipline and the proper use of time out for children. All the CCSMs expressed an understanding the time out for children under 3 years of age is not allowed.
