Witness 1 stated that on 06/06/2023, she observed Child Care Staff Member 1 grab Child A by the wrist and pull Child A toward her. Ms. Jackson was standing with Witness 1, and also watched the incident occur. Child A dropped to the floor and Child Care Staff Member 1 continued to hold Child A by one arm. Child A's shoulder was lifted from the ground as Child Care Staff Member 1 held her arm up. Ms. Jackson walked down to the classroom where the incident was occurring. Child Care Staff Member 1 held Child A's arm up while she laid on the floor until Ms. Jackson arrived at the door of the classroom. Witness 1 stated that the incident lasted for less than one minute. Shortly after during music time, Child A was pushed to the corner of the classroom to sit down. Child Care Staff Member 1 stood in front of her so that Child A could not leave. After less than one minute, Child Care Staff Member 1 walked away, and Child A got up. Witness 1 stated that both incidents were observed through a video feed with no sound. She added that Child A has special needs. Ms. Jackson stated that on 06/06/2023, she was in her office viewing the classroom camera feed. Child A flipped over a bookshelf and table. Child Care Staff Member 1 grabbed onto Child A's arm to stop her from being aggressive. Child A twisted around behind Child Care Staff Member 1. Ms. Jackson stated that she has trained the child care staff members that if they are holding onto a child and the child twists or drops to the ground to count to three and if the child is still in that position, to let go of the child so as not to hurt them. When Child A twisted behind Child Care Staff Member 1 while Child Care Staff Member 1 had ahold of her arm, Child Care Staff Member 1 did not let go within three seconds, so she walked down to the classroom to address the situation. When she arrived at the classroom, Child Care Staff Member 1 was no longer holding onto Child A's arm. When she arrived at the classroom, she reminded Child Care Staff Member 1 to let go of a child if they twist their body like that. Ms. Jackson stated that during the time that she was watching the video feed, Child A was on her feet the entire time; however, she stated that Child A could have dropped to the ground while she was walking to the classroom. Shortly after, Child A was ripping papers off the wall during singing time. Child Care Staff Member 1 had Child A sit in the quiet corner. Child Care Staff Member 1 bent down and seemed to be talking to Child A while she was sitting in the quiet corner. All of the children in the classroom were in that section of the classroom during singing time. After Child Care Staff Member 1 spoke to Child A, she continued singing time. Ms. Jackson stated that the child care staff members do not use time out with the children in care. The classroom has a quiet corner where kids can go if they need a break. 3 Ms. Jackson stated that Child A has several special needs. She can't hear in one ear and also has diagnosed attention deficit hyperactivity disorder. She has been working with Child A's Mother to get Child A an Individual Education Plan. During the on-site inspection, I walked from Ms. Jackson's office to the classroom where the incident occurred. From the door of the office to the classroom door, it is approximately 20 feet and only takes a few seconds to walk to. Child Care Staff Member 1 stated that she grabbed Child A's wrist when she was about to throw a chair. She grabbed her arm to stop her from throwing the chair. Child Care Staff Member 1 did not recall the incident described, but stated that type of behavior is common with Child A. While holding a child's arm, if the child drops down or twists their arm, Child Care Staff Member 1 will let go of the child's arm. She does not want to hurt the child's arm. Child A has a lot of behavioral concerns. She has a specialist that visits the classroom to assist. Sometimes when Child A is having a hard time, Child Care Staff Memb
