R 400.8125 · R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.
During my onsite inspection on June 27, 2022, I interviewed Tiffany Raby, licensee designee and program director. Ms. Raby explained that on June 21, 2022, the center called 911 and requested and ambulance because Child A (age 9 months) hit her head. She was "responsive but looked like she was sleeping". The 911 operator instructed the child care staff to lay Child A on a flat surface. Which they did. It took the ambulance about 15 minutes to arrive. Child A's Parents arrived at the center prior to the ambulance and observed the paramedics examination. Child A was then transported to the hospital where an MRI was conducted, which confirmed Child A had a concussion. I interviewed Alicia Jappah, child care staff member. Ms. Jappah stated that Child A was on her hands and knees rocking forwards and back. She lost her balance and fell forward. Her head hit a wooden child's chair. The impact propelled her backwards and she hit her head on the floor. Ms. Jappah was near the changing counter and had a clear view. Ms. Taylor was sitting near where Child A fell holding an infant. Ms. Taylor immediately sat the child down she was holding and picked up Child A. Ms. Jappah said Lila Crandal, child care staff member, was also in the room but was facing away from where Child A was sitting. She did not turn around until after Child A fell. Jacklyn Taylor, child care staff member, informed me she was sitting on the floor with an infant in her lap while watching other infants. Child A was on "all fours". She was rocking back and forth, and it appeared like she was trying to navigate to sit on her bottom. She lost balance and fell forward hitting her head on a small wooden chair. The impact sent her "backwards and off to the side". She hit her head again. Ms. Taylor said she immediately sat the infant she was holding down and picked up Child A. Her breathing appeared fine, but she was not waking up. The front desk was called so another child care staff member could come help in the room while she carried Child A up front to wait for the ambulance. Ms. Taylor said she checked Child A's head for bumps. She did not find any bumps, but there was a red mark. Ice was lightly held on the red area. Child A's Parents arrived before the paramedics. The paramedics checked out Child A and felt she should go to the hospital. The parents made the decision to have her transported by ambulance instead of them driving her. Ms. Taylor said she was made aware that Child A had a concussion. I did not interview Lila Crandall, child care staff member because she said she did not see anything and Child A was immediately carried from the room. Caitlyn McLemore, lead child care staff member, walked into the class just after Child A was picked up from her fall. During my telephone interview with Child A's Mother, she stated that the incident was an accident, and she can understand how it happened. She stated that her daughter pulls herself up and falls over at home. Child A's Mother said she was 3 impressed with the care the child care staff members provided. Due to having a concussion, Child A stayed home from child care for five days. Child B's Mother stated that she is very comfortable with the child care staff and the care her child receives. She said the child care staff are always diligent in calling her whenever something happens, even if it is minor. Child C's Mother stated that her family has used several child care centers and this is the best run center. The child care staff members are great at documenting the children's day and keeping parents informed of any incidents. Child D's Mother said her child has not had any injuries, nor have there been any incidents that concern her. She believes all of the child care staff members do a great job.
