    R 400.8125 · R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.

    On June 20, 2023; 1 spoke to Child A's Mother. She stated that on June 14, 2023, when Child A's Father picked up Child A from the child care center, Child A's Father was informed by Child Care Staff Member (CCSM) 3 that Child A put CCSM 1's in his mouth. CCSM 1 left her on her desk in the"cassroorn and Child A thought the ■ were jellybeans, put in his mouth but after realizing that it was not a jellybean. Child A's l o er questioned Child A about the incident. Child A told her that he took and put them in his pocket and went to the gym. While in the gym Child A put it with his and immediately when he realized a was not sweet and Child A's Mother stated that she monitored Child A at home throughout the night and did not take him for medical treatment. She also stated that he did not appear to be harmed by placing the . Child A's Mother stated that she was "frustrated" that she did not receive a telephone call from the child care center regarding the incident, as "they call about everything." She emailed Program Directors Breanna Cleeves and Maria Dionise-Kurdunowicz and expressed her concern about not receiving a telephone call regarding the incident. Child A's Mother does not have any concerns regarding the care and supervision provided by the child care center just that she was frustrated that she and Child A's Father were not called to be informed of the incident. On June 20, 2023, 1 attempted a telephone call to Child A's Father. I left a message requesting a return telephone call. On July 31, 2023, 1 spoke to Child A's Father. He stated that on June 14, 2023, when he picked up Child A from the child care center he was informed by CCSM 3 that Child A put CCSM 1's . CCSM 3 stated that CCSM I on her desk in the classroom and Child A thought jellybeans, put after realizing that it wa4no n. When Child s Father arrived ome, he told Child A's Mother. Child A told him that he put th and went to the gym. While in the gym Child A bu when he realized that it was not sweet and threw them on the floor. Child ter does not have concerns regarding the care and supervision provided by the child care center but stated that he did not like that he and Child A's Mother were not called to be informed of the incident. On June 23 2023, 1 completed an unannounced on-site inspection at the child care center wit 1 CCSM 3, Program Director Maria Dionise-Kurdunowicz, Program Director Breanna Cleeves and Child A. CCSM 1 stated on June 13, 2023, she began , which is a On June 14, 2023, she arrived toFwFr t 7:00 a.m, and brought to work with the intention on taking but became busy 'with prepping activities for field day, and she did the on her desk. When she left for the day 12:40 on June 14, 2023, at approximately p.m., she did not realize that she on her desk, as there were gift bags, food items and other things on the desk, and she did not see . She assumed that she put th in her bag. She received a text message at approximately 3:40 p.m. from CCSM 2 informing her that Child A and I wE tound on the gymnasium floor. CCSM 1 confirmed for CCSM 2 that ere were onl CCSM 1 stated that she was aware tha should not be within the reach of children. However she was not aware"Oao icy regarding the handling of child care staff member's at the child care center. She was not aware of any that may result from Child A CCSM 1 was tearful and remorseful that Child A put due to her leaving it accessible on he= CCSM 2 stated on Jun 14 2023 approximately 30 minutes after CCSM 1 left for the day she saw on CCSM 1's desk.
